Statement of Work SOCIAL WORK BEHAVIORAL HEALTH CONSULTANT TRAINING VISN 8 Network – Multiple Facilities Florida and Puerto Rico *Note that this sample has been revised from the source document on the Government Point of Entry as necessary to align formatting and applicable FAR procedures.* 1. Title of Contract: Social Work Behavioral Consultant Training 2. Background: The Department of Veterans Affairs (VA), VA Sunshine Healthcare Network, Veterans Integrated Network Service (VISN 8) is the nation’s largest system of hospitals and clinics serving a population of more than 1.6 million Veterans in a vast 64,153 square mile area spread across 79 counties in Florida, South Georgia, Puerto Rico and the Caribbean. VISN 8’s seven healthcare systems include eight Joint Commission- accredited VA Medical Centers (VAMCs) and 55 outpatient clinics. Approximately 23,800 employees work at these facilities, providing a full range of high quality, cost-effective medical, psychiatric and extended care services in an inpatient, outpatient, nursing home, and home care settings. In 2018, over 534,000 Veterans received their healthcare at VISN 8 facilities - more than any other VA network in the country. VISN 8 strives to ensure social work professional practice is consistent with practice standards defined by the Office of Care Management and Social Work in VHA and by recognized national social work professional organizations for the appropriate implementation of social work services at each VA health care system. AUTHORITY: Title 38 United States Code (U.S.C.) 7301(b). Social workers facilitate transition and coordination of care between the Department of Defense (DoD) and VHA; and between the VHA and the community through the development and utilization of resources and services in support of established treatment goals for Veterans, their families, and caregivers. VHA social workers are essential in the provision of mental health services across the continuum of care. In addition, social workers play a leading role in the provision of case management services for Veterans who are severely injured or ill, require discharge planning, or affected by severe and persistent mental illness, homelessness, suicidal ideation, and other risk factors. Comprehensive social work services are developed and implemented through a broad range of treatment programs offered in VA medical facilities. Social workers participate fully in the planning, implementation, and evaluation of treatment programs for Veterans receiving medical and surgical interventions, rehabilitation services, primary and ambulatory care clinics services, services in community settings and long-term care facilities. Social workers conduct psychosocial assessments as part of the initial interprofessional team assessment, identify psychosocial problems and stressors and contribute to interdisciplinary treatment plans. Social workers typically serve as the primary liaison between the interprofessional treatment team and family members and caregivers while overseeing provision of caregiver assistance and family support services. Social work training, education, licensure, and practice is based upon a person-in- environment framework, which makes social work uniquely suited for providing, participating in, and leading patient-centered care initiatives, such as whole health, integrated care coordination, and integrated case management. The Chief, Social Work Service or Social Work Executive must ensure these social work practice requirements are met, to include: 1. The provision of education, training, guidance, and consultation to facility leadership (e.g. VA medical facility Director, Associate Director, Chief of Staff, Nurse Executive), service chiefs, and all VA medical facility staff on social work practice, policy, and facility wide program impact. a. Confirmation that social workers practice at the top of their education, skill, and licensure level. b. The incorporation of social work services within VA medical facility, the healthcare system, and off-site VHA health care program policies. c. The provision of social work services in the following social work functions, as clinically indicated and in accordance with licensure level, scope of practice, credentialing, privileging, and clinical assignment. d. Support of social work training as part of VHA’s educational mission: (1) Psychosocial assessment. (2) Mental health assessment and diagnosis. (3) Psychosocial treatment and intervention. (4) Psychosocial rehabilitation. (5) Psychosocial case management and care coordination. NOTE: See VHA Handbook 1110.04, Case Management and Standards of Practice dated May 20, 2013, for details. (6) Advance care planning and goals of care conversations. (7) Resource referral and community services coordination. (8) Discharge or after care planning and coordination. (9) Community care and community resource linkage. (10) Interdisciplinary collaboration, coordination, and consultation. (11) Pre-admission planning. (12) Admission diversion services. (13) Patient and family education. (14) Client advocacy. (15) Mandatory reporting (abuse, neglect, or exploitation). (16) Suicide assessment, crisis intervention, and safety planning. (17) Individual, marriage, couple, family, and group counseling or therapy. 3. Scope: The contractor shall provide all labor, supervision, and other resources required to develop Social Work competency training and education programs. approved by social work continuing education by the Association of Social Work Boards (ASWB) through the Approved Continuing Education (ACE) program. This includes the plan, all aspects of the curriculum, and implementation The term curriculum refers to the subcomponents of materials, tools, content, printed materials, and learning activities. The contractor shall familiarize themselves with VA’s current state of Social Work competency and care-related education and materials. The contractor will gain a deep knowledge of the existing Social Work competency curriculum, videos, tools, materials, and on-line resources. The Social Work competency training and care education programs shall include an overarching strategy with each program consisting of curriculum, roll-out plan, project timelines (schedule, dependencies, milestones, etc.), implementation methodologies, (health study modules, video presentations, experiential learning activities, etc.), and evaluation process. Educational content shall be created with transferability for inclusion in other VA educational offerings, e.g., sub-modules on specific cross-related topics. Corresponding supplemental content will be available to Veterans in print and web-based formats will enhance learning application. Accreditation Compliance: The contractor will submit current proof that it is approved to as a provider for social work continuing education by the Association of Social Work Boards (ASWB) through the Approved Continuing Education (ACE) program. All documents rendered electronically are considered e-Learning products and will receive certification from the VA 508 Office or its delegate, without exception, prior to acceptance and final delivery. The VA 508 office utilizes the VA Section 508 Checklists. The contractor shall follow the VA 508 Review Request process, to submit the e-learning products for review and validation by the VA 508 Office or its delegate, unless otherwise designated by the Contracting Officer’s Representative (COR). The contractor shall utilize the Request for Section 508 Review with or without the assistance of the COR, when appropriate. Note: There are multiple Section 508 checklists; each checklist references different aspects of a product that needs to be 508 compliant; and several 508 checklists may pertain to a given product, such as an online course. For example, the Web Based, Functional, and Video and Multimedia checklists may all be needed for documents presented electronically. 4.0: Specific Tasks and Deliverables The contractor shall not commit or permit any act that interferes with the performance of work awarded to another contractor or with the performance of Government employees. In any case where, in the course of fulfilling the contract requirements, the contractor disturbs any work guaranteed under another separate contract, the contractor must restore such disturbed work. Task 4.1: The contractor shall develop and provide a comprehensive web-based training that includes curriculum to meet the deliverables. The web-based training shall belong to VISN 8 for future use. Deliverable 4.1.1: The contractor shall provide training for DSM V Diagnosis training on Personality Disorders, Depressive Disorders, Trauma and Stressor Disorders, Anxiety Disorders, Substance Use Disorders. Objectives for training: i. Participants will be able to recognize essential acute and chronic features of each disorder and be able to provide an accurate diagnosis for each disorder ii. Participants will be able to understand and execute brief treatment modalities for each disorder and know when to refer for longer term specialty mental health treatment. iii. Participants will be able to understand medications commonly used to treat each disorder Deliverable 4.1.2: The contractor shall provide Motivational Interviewing training. Objectives for training: i. Participants will be able to understand the techniques of Motivational Interviewing ii. Participants will be able to understand and discuss the stages of the change and match the appropriate intervention to the stage of change iii. Participants will be able to demonstrate how to apply Motivational Interviewing interventions Deliverable 4.1.3: The contractor shall provide Cognitive Behavioral Therapy Brief Treatment Interventions training. Objectives for training: i. Participants will be able to understand the techniques of Cognitive Behavioral Therapy brief treatment interventions ii. Participants will be able to identify the most common disorders Cognitive Behavioral Therapy brief treatment interventions is most effective iii. Participants will be able to assist Veterans to identify, challenge and change cognitive distortions; consequential thinking; most common and effective types of homework assignments; role playing; iv. Participants will be able to demonstrate how to apply Cognitive Behavioral Therapy Brief Treatment Interventions Deliverable 4.1.4: The contractor shall provide Solution Focused Treatment Interventions training. Objectives for training: i. Participants will be able to describe, understand and identify solution focused treatment and interventions ii. Participants will be able to demonstrate how to apply Solution Focused treatment interventions Deliverable 4.1.5: The contractor shall provide training for Crisis Intervention Modalities, to include threats of harm to self or others. Objectives for training: i. Participants will be able to describe crisis intervention modalities, to include threats of harm to self or others ii. Participants will be able to identify stages of crisis and appropriate treatment interventions for each stage iii. Participants will be able to demonstrate understanding of how to apply crisis intervention modalities iv. Participants will be able to demonstrate VA protocols to asses for threats of harm to self or others and recommend/initiate appropriate treatment/referrals Deliverable 4.1.6: The contractor shall provide Psychopharmacology training. Objectives for training: i. Participants will be able to describe most commonly used psychotropic medications (brand and generic names) and their side effects for each disorder ii. Participants will be able to describe most commonly used non-psychotropic medications (brand and generic names) and their side effects for each disorder iii. Participants will be able to describe most commonly used over the counter medications (brand and generic names) and their side effects for each disorder Deliverable 4.1.7: The contractor shall provide Trauma Informed Social Work Practice and Treatment training. Objectives for training: i. Participants will be able to demonstrate understanding of types of trauma and triggers over the life cycle. ii. Participants will be able to understand and discuss the guiding principles of Trauma Informed care. iii. Participants will be able to engage Veterans through Trauma Informed lens. iv. Participants will be able to understand and execute brief treatment modalities appropriate for trauma/triggers and know when to refer for longer term specialty mental health treatment Deliverable 4.1.8: The contractor shall provide Substance Use Disorders training. Objectives for training i. Participants will be able to recognize medical, psychological, social and functional indicators of addition disorders. ii. Participants will be able to raise the issue with Veterans in the context of their health. iii. Participants will be able to screen for substance use when Veterans present with commonly associated medical problems, including trauma or other injury. iv. Participants will be able to engage and assist Veterans with setting goals that are specific, measurable, achievable and realistic within a specific time period. Deliverable 4.1.9: The contractor shall incorporate an evidenced-based education evaluation assessment that tracks, assesses and evaluates the effectiveness of the Social Work competency training. The evaluation assessment will be a post course Practical Skill Assessment (PSA) for all participants within 30 days of the end of each class. This evaluation will address individual program evaluations, participant application and behavior change, and Veteran outcomes. Task 4.2: Provide educational content that is aligned with existing Social Work competency content and includes individual components that will be designed for use in multiple formats, including printed and electronic media. Deliverable 4.2.1: The contractor shall provide educational content for access by VA staff as the target audience. This content will be aligned with the Social Work competency training program. This will include individual components that will be designed for use in multiple formats, including printed and electronic media that is accessible to a wide audience of VA Social Workers and Behavioral Health providers. This content will be print friendly in entirety as a comprehensive manual and will also include individual PDF files for sections of content. The VA must have electronic rights to the graphics in these materials and the files must be 508 compliant. Content must be tailored to Veteran demographics, and be applicable across the continuum of health, condition, disability, and lifespan. Deliverable 4.2.2: The contractor shall provide, and keep updated, a comprehensive Social Work competency manual of educational content that supports the Social Work competency training program. The target audience for this manual is VA professionals who attend Social Work competency courses. This content will be designed for use in multiple formats, including printed and electronic media that is accessible to a wide audience of VA employees. This content will be print friendly in entirety as a comprehensive manual and will also include individual PDF files for sections of content. Web-based content will be 508 compliant. Deliverable 4.2.3: The contractor will provide a print copy of the comprehensive Social Work Competency Manual to each program participant. This will be in addition to any course-specific print material. 5. Period of Performance: The Period of Performance (POP) shall be for one (1) base year, or when all trainings have been provided and accepted by the government, and whichever comes first. 6. Place of Performance: Designated VISN 8 locations will be virtual using a web-based platform. If needed, contractor travel must be requested and approved in advance by the COR. 7. Government Recognized Holidays: There are 10 Federal holidays set by law (USC Title 5 Section 6103). Under current definitions, four are set by date: New Year's Day January 1 Independence Day July 4 Veterans Day November 11 Christmas Day December 25 If any of the above falls on a Saturday, then Friday shall be observed as a holiday. Similarly, if one falls on a Sunday, then Monday shall be observed as a holiday. The other six holidays are set by a day of the week and month: Martin Luther King's Birthday Third Monday in January Washington's Birthday Third Monday in February Memorial Day Last Monday in May Labor Day First Monday in September Columbus Day Second Monday in October Thanksgiving Fourth Thursday in November 8. Type of Contract: The Government anticipates award of a Firm Fixed-Price contract. B. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task and associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan. 2. All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms. 3. All educational documents will be delivered in an editable format. 4. All educational documents (non-administrative) rendered electronically are considered e-Learning products and will receive certification from the VA 508 Office or its delegate, without exception, prior to acceptance and final delivery. QUALITY CONTROL The contractor shall develop and maintain an effective quality control program to ensure that services are performed in accordance with this SOW. The contractor shall develop and implement procedures to identify, prevent and ensure non-recurrence of defective services. The contractor’s quality control program is the means through which the work will comply with the requirement of the contract. After acceptance of the quality control plan, the contractor shall receive the contracting officer’s acceptance in writing of any proposed change to his or her Quality Control system. D. Contractor Experience Requirements – Key Personnel: 1. Key Personnel: Certain skilled experienced professional and/or technical personnel are essential for accomplishing the work to be performed. These individuals are defined as "Key Personnel" and are those persons whose resumes are submitted and marked by the vendor as "Key Personnel." Substitutions shall only be accepted if in compliance with the “Substitution of Key Personnel" provision identified below. The Contracting Officer may notify the Contractor and request immediate removal of any personnel assigned to any task by the Contractor who are deemed to have a conflict of interest with the Government, or if the performance is deemed to be unsatisfactory. The reason for removal shall be documented, and replacement personnel shall be identified within three business days of the notification. Employment and staffing difficulties shall not be justification for failure to meet established schedules. 2. Substitution of Key Personnel: All Contractor requests for approval of substitutions hereunder shall be submitted, in writing, to the COR and the Contracting Officer at least thirty calendar days in advance of the effective date, whenever possible, and shall provide a detailed explanation of the circumstances necessitating the proposed substitution, a complete resume for the proposed substitute and any other information requested by the Contracting Officer necessary to approve or disapprove the proposed substitution. New personnel shall not commence work until all necessary security requirements have been fulfilled and resumes provided and accepted. The COR and the Contracting Officer will evaluate such requests and promptly notify the Contractor of approval or disapproval in writing. All substituted personnel shall have the equivalent or better experience, education, training, and credentials than those they replace, subject to Government approval. 3. Contractor Personnel Requirements: Key Personnel proposed shall have knowledge in the following: Senior Instructional Designer: Key personnel designs and develops instructional material for Social Work competency curricula that support large health care organizations. Individual demonstrates and utilizes effective needs analysis, project management, course development, and evaluation skills. Individual can independently develop advanced-level integrative medicine courses for health care clinicians. Individual is able to utilize multimedia technology and authoring tools. Individual determines needs of the course and appropriate method of delivery. Program Director: Key personnel will provide subject matter expertise, expert technical and managerial guidance, problem definition, analysis, requirements development, and implementation of complex projects and programs Subject Matter Experts for Social Work competency: Specific roles are identified as critical to the established Social Work competency Program. All key personnel must be highly skilled and able to provide clinical guidance and effective participant feedback to enhance skill and application. The trainers must also provide expert instruction and training facilitation. E. FORMAL ACCEPTANCE OR REJECTION OF DELIVERABLES The Government will review within 15 days each deliverable and provide feedback/comments. The contractor shall have five business days to incorporate feedback/comments and make appropriate revisions. The contractor shall provide the revised version of each deliverable within two days to the Contracting Officer’s Representative (COR). The COR will review and determine final acceptance by the Government. The COR will notify the contractor of final acceptance within five business days. F. CHANGES TO THE SOW Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the Contracting Officer shall be borne by the contractor. Only a warranted CO may alter, change, or delete contract terms. Any contractor acting on its own or under any other circumstances does so at its own economic peril. G. CONTRACTOR REPORTING REQUIREMENTS 1. The contractor shall submit a monthly progress report, due on the fifth workday, addressing the status of all active efforts to the COR and CO. The monthly report shall also serve as the official reporting for order compliance, and each one will cover progress made for the prior month. At the conclusion of each deliverable, the contractor shall provide a written memorandum documenting deliverable completion. 2. The monthly progress report shall also identify any problems that may have arisen and provide an explanation of how each problem was resolved, or where one was not resolved, a plan for how the problem will be resolved. 3. The contractor shall take minutes of all formal conference calls and/or meetings held with the Program Manager. Copies of these minutes shall be attached to the next monthly progress report. 4. Monthly progress reports shall contain, but are not limited to, the following: a) Status summary b) Change request status (new, open, closed since last report) c) Issue status (new, open, closed since last report) d) Schedule status e) Minutes of status meetings 5. The contractor shall notify the Program Manager, COR and the Contracting Officer, in writing, if problems arise that could have an adverse effect upon the performance of the contract. H. INVOICING Vendor Electronic Invoice Submission Methods: Invoices will be submitted via electronic form. Facsimile, e-mail, and scanned documents are not acceptable forms of submission for payment requests. Electronic form means an automated system transmitting information electronically according to the accepted electronic data transmission methods below: i. VA’s Electronic Invoice Presentment and Payment System – The FSC uses a third-party contractor, Tungsten, to transition vendors from paper to electronic invoice submission. The contractor can go to this website: to begin submitting electronic invoices, free of charge. ii. A system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) chartered by the American National Standards Institute (ANSI). The X12 EDI Web site (). Vendor e-Invoice Set-Up Information: The contractor may contact Tungsten at the phone number or email address listed below to begin submitting electronic invoices to the VA Financial Services Center for payment processing, free of charge. The preliminary and final deliverables and all associated working papers, application source code and other material deemed relevant by VA that have been generated by the contractor in the performance of this contract are the exclusive property of the U.S. Government and shall be submitted to the Contracting Officer at the conclusion of the contract. These deliverables will be provided to VA in an editable format. 2. The Contracting Officer will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this contract. No information shall be released by the contractor. Any request presented to the contractor for information relating to this contract shall be submitted by the contractor to the Contracting Officer for response. 3. Press releases, marketing material or any other printed or electronic documentation related to this project shall not be publicized without the written approval of the Contracting Officer. K. Security Requirements 1. The contracted personnel will not have access to VA information systems or sensitive information and will be escorted by VA authorized staff while onsite at VA facility. 2. The C&A requirements do not apply, and a Security Accreditation Package is not required. ................
