I. OBJECTIVE:

The objective of the Hazardous Materials and Waste Management Plan is to develop a system that addresses the identification, selection, handling, storage, use and disposal of hazardous materials and wastes.

II. SCOPE:

The Hazardous Materials and Waste Management Plan is applicable to all Premier Medical Group sites that use, store, and/or dispose of hazardous materials and wastes.

III. GOALS:

The goals of the Hazardous Materials and Waste Management Plan include the following: To provide education to personnel on the elements of the Hazardous Materials and Waste Management Program; To identify, evaluate and inventory hazardous materials and waste generated or used consistent with applicable regulations and laws; To provide adequate space and equipment for the safe handling and storage of hazardous materials and waste; To establish emergency procedures to use during hazardous materials and waste spills or exposures

IV. FUNCTIONS AND RESPONSIBILITIES

1. Develop and monitor the processes used in the selection, handling, storage, use, and disposal of hazardous materials from receipt through use and hazardous waste from generation to final disposal.

2. Ensure mechanisms exist for complying with regulations regarding the identification, evaluation, and inventory of hazardous materials and wastes used or generated by each department

3. Review of hazardous materials and waste spills and exposure events, as well as other problems related to hazardous materials or waste

4. Take necessary actions when educational efforts are indicated; ensure educational components required by regulatory and accrediting agencies, are completed.

5. Review and revise PMG hazardous materials policies and procedures 6.

VI. HAZARDOUS MATERIALS AND WASTE POLICIES AND PROCEDURES

Premier Medical Group has developed policies and procedures to carry out the objective and goals of the Hazardous Materials and Waste Management Plan. Policies and procedures are produced based on the hazardous materials. System-wide hazardous materials and waste policies and procedures are distributed to all departments and are posted on the Premier Medical Group Intranet site. Site leadership is responsible for ensuring enforcement of these policies and procedures. Each employee is responsible for following safety policies and procedures.

Hazardous Materials and Waste policies are reviewed at least annually or as necessary. Some policies/procedures will be reviewed more often if driven by the requirements of regulatory standards or company-specific or industry-wide incidents. Additional interim reviews may be performed on an asneeded basis. Hazardous Materials and Waste policies, procedures, and forms are listed in the Appendix A of this Plan.

VII. STAFF DEVELOPMENT, EDUCATION, AND TRAINING

All persons managing or handling hazardous chemicals, materials, or waste will be provided with appropriate orientation, proper use of personal protective equipment (PPE), spill response and appropriate disposal. Each site is responsible for training individuals handling hazardous materials and waste. Training will incorporate a combination of in-person and online training.

VIII. EMERGENCY RESPONSE

The Premier Medical Group Policies/Procedures Manual provides emergency procedures for responding to hazardous materials and waste-related events. Emergency procedures drills will be carried out and documented in accordance with the Hazardous Materials Spill Policy (EC 2014).

IX. INCIDENT REPORTING AND INVESTIGATON

The Hazardous Materials and Waste Management Plan requires documentation of incidents and accidents involving hazardous materials and wastes. All incidents must be reported in accordance with the Incident Reporting policy (EC 2000).

X. INSPECTION, TESTING, AND MAINTENANCE

To ensure the operation of the Hazardous Materials and Waste Management Plan the facility must engage in inspection, testing, and maintenance of its various components. Many components are observed on a regular basis, such as employees properly handling, disposal, identification, and labeling of hazardous materials and wastes. Other inspections involve ensuring proper engineering controls, policies, and personal protective equipment (PPE) are being used properly.

IX. PERFORMANCE IMPROVEMENT MEASURES

A. At Premier Medical Group, ongoing performance monitoring is conducted. The following hazardous materials and waste performance monitors have been established: 1. Incident reports submitted 2. Updated hazardous materials inventories and Safety Data Sheets

XII. REGULATORY/ ACCREDITATING COMPLIANCE

The Hazardous Materials and Waste Management Plan is written and implemented in accordance with regulatory and accrediting agency requirements.
