How to Earn Wellness PointsTo Participate in the Employee Wellness Program, please note that you must complete the yearly online Blue Cross/Blue Shield Health Assessment by April 7th, 2017 (Points are not awarded for this.) Completion of the Initial Confidential Online Health Assessment is mandatory to receive either 1) a reduction in your health insurance premium deducted from your paycheck OR 2) an additional $25.00 monthly contribution to your retirement plan if you opt out of the Health Department’s health insurance plan. To check on your points tally, go to the Intranet/Personnel/Employee Wellness Program/Resources/Employee Wellness Points Log. Find your employee ID number and your points and the amount of the financial incentive you have earned will be listed. Points will be updated on a monthly basis; however, the WebMD modules will not be updated until the end of the year.1-30 Points EachSmall Steps Calendar (1-31 points): Take small steps toward wellness with a different activity each day. Each quarter a new small steps calendar will be published by the Wellness Team. For each day that you participate, you can earn a point. (Activities can be completed anytime throughout the month, so they don’t necessarily need to occur on the day specified.) At the end of the month, turn your calendar into Lynne with the total number of points tallied. HDNW Training Today Wellness Modules (20 Points each): Complete any of 12 modules of your choice online. Many of these modules support communications in the workplace and align with our Dignity at Work Policy. Examples of topics include Effective Communication for Employees, Generational Diversity, How to Prevent and Respond to Bullying in the Workplace, Workplace Harassment-What Employees Need to Know and more. Modules that qualify for Wellness Points are indicated with an icon in the “Documents” column. Once you have completed the module and passed the quiz, you can print a completion certificate if you would like, however, this is not necessary. This is an opportunity to share healthy food at your office.? Bring in a?healthy snack (sliced fruit with yogurt for dip, or veggies and hummus???? ?) or a healthy dish to share with your fellow employees.? These can be featured in the coffee station area or other area where staff gathers.? We've all tried new things when someone else prepares them first--this is a great way to learn about new healthy foods (and that healthy foods can be tasty!).? Please include recipes and forward one to Lynne in order to earn points for your contribution to promoting wellness! If your food doesn't require a recipe, just note what you brought in.Attend a presentation (20 Points) on a wellness topic outside of work. Local hospitals, libraries, physical therapy practices, and community colleges offer a variety of opportunities for learning more about health and wellness. Topics may include osteoporosis, fitness assessment, cancer prevention or even a health fair. Forward the date, name of the presentation and organization sponsoring it to Lynne.30-200 Points EachWebMD Modules: Complete any of the 6 WebMD online health education modules of interest to you on your own time and at your own pace. To Access WebMD Modules: 1) Go to 2) Log in with your Member ID and Password3) Click on Healthy Living Tab4) Click on My Health Assistant5) Select from the menu:WebMD Eat Better 30 points WebMD Lose Weight 100 points WebMD Enjoy Exercise 30 points WebMD Conquer Stress 40 points WebMD Feel Happier 40 points WebMD Quit Tobacco (6 months tobacco free) 200 points Once you have completed your module(s), you don’t need to do anything more. A report will be generated in December that lists the number of modules you have completed. Points for this activity will not show up on the Employee Points Log until after the end of the year (the information comes from a report that comes from BC/BS).30 Points EachLunch & Learns: Twice a year, the Wellness Program provides lunch and a lecture on a health-related topic. Lectures will be scheduled for each of the 5 offices. Schedules are posted ahead of time and sign up is required to insure enough lunch is available. Make sure you sign an attendance sheet on the day of the event, as well, to earn points for attending.Flu Shot: Get your annual flu shot from the health department or another provider. You need to send an e-mail to Laurie Hughes to let her know you received it. Getting your shot at the HDNW does not automatically get recorded in the Wellness Point System, so please let Laurie know.Participate in Event: Send an e-mail to Lynne if you participate in an organized event such as Relay for Life, Bike for Breast Cancer, 5K Run or Walk, Kayak, Smart Commute, Marathon, X-Country Ski event, yoga or meditation retreat, etc. Wellness-focused potlucks: The Wellness Committee organizes 3 potlucks each year.? If your office, division, or anyone?would like to?schedule an additional Wellness Potluck at anytime, you can earn more points.? See below regarding recommendations for Wellness Potlucks.? Just forward a list of the attendees to Lynne.Some simple nutrition guidelines in order to emphasize healthy foods:Whenever possible, include fruits and veggiesIf you use dairy products, try to use low fat/fat free versionsAvoid fried foods.Emphasize whole grainsMeats should be leanLightly salted-emphasize the use of herbs, spices, nuts, fruits, mustards, vinegars for flavoringDesserts are welcome in small portionsBeverages options should include non-sugared options, such as sparkling water, unsweetened iced tea, spa water (water with fresh fruit), diet drinks, coffee, or tea50 Points EachFun & Fitness Contest: May-July (13 weeks) Individuals keep track of their own physical activity and record it in aerobic miles (log sheets and aerobic mile conversions are provided). Weekly totals are recorded on a log sheet in each office.? Office coordinators enter the weekly totals on an Intranet log sheet. The competition rewards an overall winner and runner up. Plus there are 3 random drawings throughout the competition and a winning office will be determined, based on the average aerobic miles per participant.? Make It Happen-Habit Change Challenge: August-November (13 weeks) Select one or more goals for yourself.? You choose steps to take toward accomplishing that goal.? You do your own recording and tracking (forms are provided if you would like to use them.)? At the end of each week, you indicate whether or not you worked on the steps you chose.? A sign-up sheet will be posted in each office where you can indicate your progress.? Your specific goals and steps are confidential; the only thing you share is whether or not you are working on it!?For 25 bonus points:? Create more accountability for yourself by reporting to a friend, a family member, or co-worker at least 4 times throughout the challenge. For example, form a small group that can commit to meeting regularly (10 minutes before or after work or at lunchtime).? Share your goals with each other and offer support in making changes. Celebrate successes and brainstorm challenges together! The log sheets in each office will provide a way to indicate that you are including the accountability component.?If you don’t feel the need to work on a health behavior, you can focus on another behavior, such as a financial goal, reducing clutter at home or work, or spending more time with people you care about.? After all, these may all aid in reducing stress!??Workout: This includes exercise outdoors or indoors, on?your own or in a class. For every 3 consecutive months of exercise that includes 12 days per month you can earn 50 points. If you do this for the entire year, you could earn all of your Wellness Points this way!? You will need to submit a record of the activity to Lynne in order to earn points.? A log sheet, Work Out Tracker is available for recording your activity.? You can keep a paper copy or save it as an electronic version on your computer.BC/BS Win by Losing Spring & Fall: BC/BS of Michigan hosts a weight loss competition among businesses twice a year (Feb-April & August-October).? Participating businesses compete against each other based on the number of employees who take part in the competition.? Our agency can compete as one business or smaller groups can form teams.? E-mails will be sent to notify employees about the start of the program. Nichole Flickema will be our team leader and will log our cumulative starting weight and our weekly weight for the 9 weeks.? Participants will need to phone or e-mail their weekly weights to Nichole. She will submit this to the contest website.? Fruit & Veggie Challenge: (November?– December)?The intent of this challenge is to promote maintaining weight over the holidays (Thanksgiving to New Year’s weight gains range from 5-8 or more pounds for adults!)? Filling up on fruits and veggies can be a helpful strategy in keeping the scale in check.Individuals keep track of the number of servings of fruits and vegetables they eat each day and record weekly totals on a team log sheet. The totals for each team will be tallied and the team with the highest percentage (based on each person eating their goal of 5 servings/day) wins. ?The challenge is 8 weeks long and the winning team will be rewarded.????????????????????????????????????????????????????????????????????????????????????????????????????????????????????????????????????????Participate in Weight Watchers: (for 3 consecutive months). For each 3 months that you participate in Weight Watchers () or other weight management program, Wellness Points will be awarded. Print out your WW registration form and when you have completed 3 months, forward the form and dates to Lynne. The points will be added to your tally. If you participate for a full year, you could earn all of your Wellness Points. ................
