In applying for this sanction (ed event), the Host, Cardinal Community Swim Club, agrees to comply and to enforce all health and safety mandates and guidelines of USA Swimming, Indiana Swimming, the State of Indiana, and local health department.

Dear Delaware Community School Board and School Officials,

Thank you for partnering with us, Cardinal Community Swim Club, this fall and allowing us the use of your pool so that we can continue to provide our community the sport of age group swimming. CARD is grateful for all that you have considered this fall as we, as a community, navigate the Covid-19 Pandemic. Today, we ask that you consider our proposal to host a one day meet later this month.

Amongst the swimming community, this meet is known as the CARD Dactacular. It originated under the original Delaware Aquatic Club (DAC) program and has been continued under our joined program for greater than ten years. Traditionally it is the last Saturday in October. It is our intention to host a healthy, safe and fun meet for our swimmers.

During the planning of this meet, CARD has strived to follow the recommendations from the Delaware County Health Department, Indiana Swimming and USA Swimming,

• The meet will be broken into 4 sessions by age groups.

• Anticipated number of swimmers: 50 per session

• Anticipated number of spectators: 36 max per session allowing for 6 ft distance between persons

• Anticipated number of volunteers: 30 per session: 20 parent volunteers; 10 officials

• Anticipated number of coaches: 10 per session

• Max number of persons per session: 130 persons

• All persons will wear masks unless they are swimming in the pool

• CARD will disinfect the swimmer and spectator areas between sessions.

In order to host a successful event, CARD asks permission to utilize the pool along with the east and west upper gym balconies. We would like to utilize Door 2 on the west side of the high school as our entry point and temperature taking area. Then up the stairs on the west balcony, swimmers and spectators are expected to sign in. Swimmers will report to the pool and then to the east balcony to store their gear. Teams will be socially distanced on the east balcony. Spectators are expected to socially distance and wear masks at all times. CARD will mark out acceptable seating. Swimmers are only allowed 1 spectator in attendance. At the conclusion of the session, spectators and swimmers will be asked to leave through Door 2b on the west side of the pool. There will be a 15-minute period between all sessions designated for cleaning and disinfecting the spectator and swimmer areas.

Included in this packet is our intended meet announcement as required by IN Swimming and USA Swimming, our governing bodies. This packet has also been submitted for approval through the Delaware County Health Department.

|Any swimmer entered in the meet, must be certified by a USA Swimming-member coach as being proficient in performing a racing start of must start each race from within|

|Swimmers are to remain in their assigned camping area until preparing for their heat and are required to wear a mask while not physically in the pool. |

|Safe Sport 360: |

|The Minor Athlete Abuse Prevention Policy prohibits Applicable Adults (as defined) from having one-on-one interactions with minor athletes which are not within an |

|observable and interruptible distance from another adult. Applicable Adults are required to abide by this policy in full during this meet. |

|§ Pursuant to USA Swimming Rules and Regulations and federal law, it is every member’s responsibility to immediately (i.e., within 24 hours) report any incident of |

|child abuse, including physical or sexual abuse, to law enforcement and the U.S. Center for SafeSport. Reporting must occur when an individual has firsthand knowledge|

|of misconduct or where specific and credible information has been received from a victim or knowledgeable third party. A report to the U.S. Center for SafeSport may |

Release and Hold Harmless Agreement

In consideration of being permitted to participate in this swim meet, and for other good and valuable consideration, the undersigned for himself, his successors and assigns, hereby releases and forever discharges the host club, and its Board of Directors, USA Swimming, Indiana Swimming and each of their respective officers, agents, employees, members, successors and any other persons in any way connected with this meet, from any and all liabilities, claims, demands, actions, or causes of action of whatever kind of character arising out of or in connection with said event. Further the undersigned shall indemnify and hold harmless the host club, USA Swimming, Indiana Swimming and the officers, trustees, agents, employees and members of the foregoing and all other persons in any way and claims arising out of or in connection with any injury, including death, or alleged injury of damage to property sustained or alleged to have sustained in connection with or to have arisen out of said event.
