ACADEMIC CONTRACT
For students in the Zuckerman College of Public Health
Students placed on academic probation must make academic progress (grades of A and B) in order to achieve good standing with the College and University. The probationary period will end once your cumulative GPA is a 2.0 or above. Please also note that the GPA required for admission to the public health professional major is 2.5.
1. Calculate how many B grades you need to earn this upcoming semester to reach UA and College target GPAs:
a) Visit your UAccess Student Center to get information regarding your current cumulative UA GPA and cumulative UA units taken towards your GPA,
b) Visit the following link: - scroll down to the section that says “Calculate your Target GPA”
I need ______units of B to raise my cumulative GPA to a 2.0.
I need ______units of B to raise my cumulative GPA to a 2.5 (required for admission to the public health major).
Note: Earning A grades (4.0) in some or all courses helps raise GPAs even quicker than earning B grades do. Earning C grades (2.0) in courses does not assist in raising a student’s GPA. Also, taking Pass/Fail courses does not assist in raising a student’s GPA.
2. Review the UA Grade Replacement Opportunity policy by following this link:
If you are eligible, list the courses that you can consider GRO’ing: _____________________________
3. There are many reasons students enter the academic probation process. These include lack of adequate attention to academics, financial or heavy work obligations, health struggles, family needs, or other personal issues. Attach a personal statement answering these questions: Why do you think you struggled academically this past semester? Because it is important for you to take steps to address the causes of your academic difficulties, what strategies do you plan to use this semester?
Advisor Notes: ________________________________________________________________________________
I understand that University policy is that a student may be disqualified from the College and/or University if the student has lower than a 2.0 GPA two consecutive semesters. I will meet with my academic advisor three times this upcoming semester in order to make plans, identify resources and report on progress.
