Illinois Screening, Brief Intervention, and Referral to Treatment II (Illinois SBIRTII)

The Illinois Department of Human Services, Division of Alcoholism and Substance Abuse (IDHS/DASA) has implemented SBIRT services to adult patients within several federally qualified health systems, two hospitals and several non-correctional community settings located in Rockford and in the Chicago metropolitan area. Our partners include:

• Access Community Health Network - FQHC

• Erie Family Health Center - FQHC

• Aunt Martha’s Youth Service Center - FQHC

• Rosecrance, Inc. at Swedish American Hospital

• Family Guidance, Inc. at St. Bernard’s Hospital

• Treatment Alternatives for Safe Communities, Illinois TASC – non-correctional community settings

The model in the FQHC’s involves screening by medical assistants, brief intervention by the medical provider, and referral by a case manager. The case manager conducts screening, provides brief intervention and makes referrals in the hospital and non-correctional community settings.

Illinois screens for substance abuse using the Alcohol Use and Alcohol Use Disorders Identification Test for alcohol and the Drug Abuse Screening Test (DAST-10) for other drugs. The first three questions of the AUDIT make up the AUDIT-C (Alcohol Use Disorders Identification Test - Consumption) and, along with the first question from the DAST-10, serve as a “pre-screen”. When indicated based on the results of the AUDIT – C, the Full AUDIT is conducted. When indicated based on the results from inquiring about a patient’s drug use using the first question from the DAST-10, the full DAST-10 is conducted.

The sites also conduct the two-question version of the Patient Health Questionnaire, the PHQ-2. When indicated based on the results of the Mental Health Screen, patients are referred to a community mental health provider. When indicated based on the results of a brief Tobacco Product Use Screening Question, patients are referred to tobacco cessation services.

TASC also conducts a gambling screen. All the clients seen by their SBIRT programs will receive a prescreen based on the Brief Bio-social Gambling Screen (BBGS), a three-question screen developed by researchers from the Division on Addiction, Cambridge Health Alliance, a teaching affiliate of Harvard Medical School.  This prescreen includes three questions, each referencing a “during the past 12 months” timeframe: have you become restless, irritable or anxious when trying to stop/cut down on gambling; have you tried to keep your family or friends from knowing how much you gambled; and did you have such financial trouble that you had to get help from family or friends?  Answering ‘Yes’ to one or more questions indicates the need for a more complete screen.  Staff at Illinois TASC will conduct the complete screen, based on the DSM, and if clients score a 4 or 5 will provide Brief Intervention regarding gambling.  Clients who score a 6 or above will be referred for gambling treatment.  Illinois has substance abuse treatment providers who are also trained and funded to provide treatment for Problem and Pathological gambling that utilize their existing contracts to serve SBIRT clients.
