Dear Exhibitor,

Expo Advantage USA, Inc., will be the Official Services Contractor for the upcoming Symposiums at the Hilton New York Hotel. We are pleased to have been selected

to serve you. Our objective is to make your participation at this show as successful as it can be.

To assist you, we have prepared the OFFICIAL EXHIBITOR'S MANUAL. The Manual has two (2) sections. The first describes the services available from our Company. The second is a series of order forms. Please take a few minutes to carefully read this information. Feedback received from Exhibitors has indicated that reading the manual first and then completing the forms is the most effective manner for understanding and ordering of services.

Please note payments for furniture and accessories are due with the order. All charges for services must be paid prior to the close of show. For your convenience, we accept Visa, MasterCard and American Express.

We will accept fax orders for all services. To be eligible for the discounted rates, we must have your check forty-eight (48) hours prior to the installation or a credit card charge authorization form faxed to us with the advanced orders. Orders received without advance payment are considered FLOOR ORDERS and will be billed at the floor order rates. Orders can be faxed to (631) 273-2371.

**When scheduling your carrier for showsite pick-ups, be aware that trucking waiting time may apply. Expo Advantage must return all empties to the show floor and materials must be packed prior to proceeding with outbound freight pick-ups. To assist in keeping freight movement orderly, all showsite carriers must sign in at the loading dock upon arrival and leave contact information.

Forced freight is subject to additional surcharges.

Special Notice

Due to the delicate nature of the medical equipment and special crating and/or handling involved, a company representative is requested to supervise the unpacking and packing of this equipment to ensure all parts are accounted for as well as securely and properly packed for return shipments. Should a company representative be unable to supervise, every attempt will be made to ensure the proper crating and handling of this equipment. Expo Advantage USA will not be responsible for missing parts and/or damages should they occur. It is the responsibility of the exhibitor to make sure their equipment and display materials are properly insured.

All medical equipment from the show floor must be removed during outlined move out. The hotel will force Expo Advantage USA, Inc., to clear the floor of all remaining materials and equipment at 4:00 a.m. on Saturday, 11/22/08, due to other obligations for the exhibit sites. Forced freight time subject to change without notice at the discretion of Expo Advantage USA, Inc.

It is imperative that each exhibitor makes arrangements with their representatives to ship their equipment within the outlined time frame.

Exhibitors cannot move their equipment in the Hotel’s passenger elevators. Passenger elevators are not rated for the excessive weight of equipment/freight and the Hotel will not allow them to be used for this purpose. All equipment/freight must be moved using the designated freight elevators. Freight elevators, in accordance with the Hotel, can only be operated by authorized personnel. Due to safety precautions, exhibitors will not be allowed to use or ride the freight elevators.

*** IMPORTANT ***

Expo Advantage will have priority use of elevators. All companies wishing to use other companies to load in or load out their materials by use of the Hilton New York Hotel Elevators, must coordinate with Expo Advantage.

Limits of Liability & Responsibility

Please Read Carefully

1. Expo Advantage USA, Inc. and its subcontractors shall not be responsible for damage to materials improperly packed, uncrated materials, glass breakage, and/or concealed damage.

2. Expo Advantage USA, Inc. shall not be responsible for loss, theft, or disappearance of Exhibitor's materials after same have been delivered to Exhibitor's booth.

a. Relative to inbound shipments: there may be a lapse of time between delivery of the shipment(s)

to the booth by Expo Advantage USA, Inc. and the arrival of the Exhibitor's Representative at the booth. During such time, the shipment(s) will be left unattended. Expo Advantage USA, Inc. shall not be responsible for any loss, theft, or damage that may occur during such period.

3. Expo Advantage USA, Inc. shall not be responsible for loss, theft, or disappearance of Exhibitor's materials pending loading out after the close of show. All bills of lading for outgoing shipments, which are given to Expo Advantage USA, Inc. by Exhibitors, will be checked at the time it is removed from the show floor, outbound to the loading dock. Corrections will be made where discrepancies exist. Expo Advantage USA, Inc. does not provide security on the show floor, storage areas or loading dock. Exhibitors are responsible for their exhibit materials until received by their designated outbound carrier.

a. Relative to outgoing shipments: there will be a lapse of time between the completion of packing

and the actual pick-up of materials for loading onto a carrier. During such time the shipment(s) will be left unattended. Expo Advantage USA, Inc. will not be responsible for any loss, theft, or damage, which may occur during such period. Expo Advantage USA, Inc. will adjust quantities and weights on any bill of lading to conform to the actual count and weight of items in the booth at the time of pick-up.

4. Expo Advantage USA, Inc. shall not be responsible for loss, damage or delay due to strikes, lockouts, equipment failure or work stoppages of any kind including stoppages due to normal hotel functions.

5. Expo Advantage USA, Inc. shall not be responsible for ordinary wear and tear in handling of materials nor for loss due to fire, windstorm, water, vandalism, acts of God, mysterious disappearance, or other causes beyond its control.

6. Expo Advantage USA, Inc. shall not be liable to any extent whatsoever for any actual, potential, or assumed loss of profits or revenues, or for any collateral costs, which may result from any loss or damage to any Exhibitor's materials which may make it impossible or impractical to exhibit same.

7. Expo Advantage USA, Inc. liability shall be limited to any loss or damage which may result solely from Expo Advantage's negligence in the actual physical handling of the items comprising a shipment and not for any other type of loss or damage.

8. Expo Advantage USA, Inc. is not an insurer. Insurance, if any, shall be obtained by the Exhibitor. It is

suggested that Exhibitors arrange all risk coverage. This is done by adding "riders" to existing policies.

BE SURE YOUR MATERIALS ARE INSURED from the time they leave your firm until they are returned after show. Be sure your liability insurance is in effect at the showsite. Contact your representative for further information.

9. The amounts payable to Expo Advantage USA, Inc. hereunder are based on the value of the material handling services and the scope of liability herein set forth. These amounts are unrelated to the value of the Exhibitor's property being handled. Because it is impractical and extreme difficult to fix the value of each shipment handled by Expo Advantage USA, Inc., Expo Advantage DOES NOT PROVIDE FULL LIABILITY should loss or damage occur. Should Expo Advantage USA, Inc. be found liable for loss or damage, the liability shall be limited to the specific article which was physically lost or damaged. Such liability shall be limited to $0.10 per pound, per article with a maximum liability of $50.00 per item or $1,000.00 per shipment, whichever is less, as the exclusive remedy and agreed upon damages and not as a penalty. The irrespective of cause or origin, results directly or indirectly to property from the performance or non-performance of obligations imposed by the offering of material handling services to exhibitors.

10. Expo Advantage USA, Inc. shall not be liable to any extent whatsoever for loss or damage to Exhibitor's materials while same are consigned to "empty" storage. This shall apply to any and all materials removed from storage. This shall also apply to any and all materials removed from the show floor and stored during the show. Expo Advantage USA, Inc. does not supply security unless specifically requested in writing. Such requests will be accommodated with all charges incurred invoiced to the requesting Exhibitor(s) or their designated contractor(s), which ever may apply.

11. Claims for loss or damage must be submitted to Expo Advantage USA, Inc. by the close of show. No suit or actions shall be brought against Expo Advantage USA, Inc. more than one (1) year after the actual date of the close of show.

12. In connection with the receipt, handling, temporary storage and reloading of Exhibitor's materials, Expo Advantage USA, Inc. will provide services as the agent of the Exhibitor, and not as bailee or shipper. If any employee of Expo Advantage USA, Inc. shall sign a delivery receipt, bill of lading or other document, they will do so as the Exhibitor's agent and the Exhibitor accepts the responsibility thereof.

13. In order to expedite the removal of materials from the showsite, Expo Advantage USA, Inc. shall have the authority to change Exhibitor designated carriers in the event that such carriers do not pick-up on time or refuse the shipment. Where no disposition is made by the Exhibitor, materials will be taken to Expo Advantage USA, Inc. warehouse to await Exhibitor's shipping instructions. The Exhibitor is responsible for charges relating to such handling and for making all the necessary arrangement's for their carrier to pick-up their shipment. No liability will be assumed by Expo Advantage USA, Inc. as a result of such re-routing or handling.

14. All questions relating to classification of Exhibitor's materials, rates charged or weights used to determine material handling charges shall be submitted to Expo Advantage USA, Inc.'s office on the invoice within thirty (30) days of the date of the invoice. Complaints received after such period shall not be considered and payment of the invoice shall be made in full.

15. Expo Advantage USA, Inc. shall have control of all traffic into and out of the showsite to prevent tie ups in loading areas, to keep aisles clear, and to provide an orderly and efficient operation for the show as a whole.

16. Expo Advantage USA, Inc. will not be held liable for delays caused by building equipment failures such as loading dock equipment, elevators and escalators.

PLEASE NOTE THE FOLLOWING . . .

The consignment or delivery to Expo Advantage USA, Inc. by an Exhibitor or by any shipper on behalf of the Exhibitor, shall be construed as an acceptance by such Exhibitor (and/or shipper) to the terms and conditions set forth herein Sections 1 through 16. Claims for loss or damage cannot be processed until all invoices for the services performed by Expo Advantage USA, Inc. have been paid in full.

Labor Union Guidelines for the New York Area

There are four (4) major unions that have jurisdiction over trade shows. The following should help guide you in conforming to union jurisdiction and its adherence to them when required.

TEAMSTERS UNION

Teamsters handle freight at the exhibit hall. They unload trucks or vehicles. They also provide rigging of machinery, moving services and spot machinery in your booth. The normal rigging crew consists of three (3) men. Customarily, this service must be ordered as needed at the exhibitor's expense.

CARPENTERS UNION

Carpenters handle the erection and dismantling of display and exhibit booths. This also includes all display work. Carpenters uncrate and re-crate machinery and/or equipment.

EXPOSITION WORKERS

Exposition workers deliver freight to exhibitor's booth after it has been unloaded by teamsters. They also deliver furniture and floor covering. They may assist carpenters in the erection and dismantling of exhibits and displays. The first two (2) men required for this work must be carpenters and the third man, if needed, an exposition worker.

ELECTRICAL UNION

Electricians handle all electrical work, which includes supply power lines to your booth, connecting equipment to outlets and installing signs or headers that are lighted, unless they are permanently a part of the exhibit backwall. Electrical union labor is supplied by the hotel.

WHAT CAN EXHIBITORS DO WITHOUT UNION PERSONNEL?

Contrary to popular belief, exhibitors are allowed to do some work within the privacy of their own booths. The following list consists of services that do not require union personnel:

• Exhibitors are allowed to unpack and re-pack their own product (if in cartons, not cases).

• Exhibitors can carry into their booth merchandise that one person can hand carry in. This does not mean that if an exhibitor has fifty (50) cartons in a truck that the exhibitor can make fifty (50) trips to his booth.

• Exhibitors are allowed to do the technical work on their machines, such as balancing, programming, cleaning, etc.

• Exhibitors are allowed to set-up and dismantle their exhibit if it is a TABLE TOP display or a POP-UP display – provided it is no larger than 10’ and can be set/dismantled by one (1) exhibiting company employee in 1/2 hour – any other type of exhibit requires union labor.

Set-up & Dismantling Labor

In the interest of prompt and efficient processing of exhibitor's labor requirements for set-up and dismantling of exhibits, it is recommended that advance notice be provided using the provided form. Labor requests received at showsite will be filled after advance orders in all cases. Orders placed on the show floor, without prior notification via the provided form, are subject to a twenty-five percent (25%) surcharge.

Refer to the Installation/Dismantle Schedule located in the front of this manual for specific time/dates for your exhibit labor.

Notes:

• You must call for labor at the service desk and bring display person(s) back to the service desk when finished.

• Expo Advantage has exclusive right to force the Overtime or Double Time rate.

• The minimum charge for labor is one (1) hour per person, one-half (1/2) hour increments thereafter.

• When scheduling dismantle labor thru Expo Advantage or your EAC, be sure to allow sufficient time for empty containers to be returned to your booth.

• Labor must be cancelled in writing, 24 hours in advance to avoid a one (1) hour cancellation fee per worker.

ADVANCED ORDER LABOR RATES

CARPENTERS

Straight Time (8:00 a.m. to 4:30 p.m. Monday thru Friday) $135.00 per man/per hour

Overtime (4:30 p.m. to 8:00 a.m. Mon thru Fri, All Day Saturday) $195.00 per man/per hour

Double Time (All Day Sunday & All Holidays) $240.00 per man/per hour

Non-Official Contractors

A non-official contractor is any company (other than the designated official contractor) that an exhibitor wants to use inside the exhibit hall before, during, or after the show. Permission to use a non-official contractor will not be given for the performance of the following services: Electrical, Plumbing, Telephone, Drayage, Rigging, Booth Cleaning, or Catering.

How to obtain authorization for use of a Non-Official Contractor.

The three steps to obtain authorization to use a non-official contractor are:

- The Exhibitor must fill out the Non-Official Contractor Request form and return it to Expo Advantage

USA, Inc. A Non-Official Contractor fee of $200.00 must accompany all non-official contractor requests.

- Written acceptance of show rules from the Non-Official Contractor.

Written acceptance in the form of a letter from the Non-Official Contractor to the Official Contractor, Expo Advantage USA, Inc., specifying that the contractor will abide by all rules and regulations and that the Non-Official Contractor maintains the appropriate union contracts for the jurisdiction and is in good standing with these unions.

- Certificate of insurance from the Non-Official Contractor.

Authorization will be given to provide installation and dismantling service to the exhibiting firm upon receipt of a certificate of insurance naming the show manager and naming the official contractor and the exhibition hall as additional insured. The non-official contractor will provide the following types of insurance and minimum values:

- Workers Compensation minimum coverage $100,000.00.

- Comprehensive general liability minimum coverage $1,000,000.00.

- Automobile liability should include all owned and hired vehicles with limits of $500,000.00 bodily injury and $500,000.00

property damage.

Rules and regulations governing Non-Official Contractors

1. The Non-Official Contractor will refrain from placing any undue burden on the Official Contractor, including specifically not interfering in any way with the Official Contractor's work.

2. The Non-Official Contractor will not solicit business on the show floor during installation, dismantling or at any other time. Violation of this rule will automatically result in removal of the Non-Official Contractor's employees and agents, as well as forfeiture of the approval previously granted. Any work not completed will be performed by the Official Contractor and the Exhibitor will be responsible for all services provided by the Official Contractor.

3. The Non-Official Contractor will share with the Official Contractor all reasonable costs incurred in connection with his operation, including overtime pay for stewards, security, restoration of exhibit space to its initial condition, and marking of exhibit floor.

4. The Non-Official Contractor will cooperate fully with the Official Contractor and will comply with existing labor regulations or contracts as determined by the commitment made and obligations assumed by Show Management in any contracts with the Official Contractor.

Show management reserves the right in its sole discretion, to withhold approval or dismiss from the show, any Non-Official Contractor whose participation in the show may, in the opinion of show management, lead to strikes, pickets, or any other labor action directed at the show or other disruption of the show, or unreasonable interference with or inconvenience to the show or any of the exhibitors.

**IMPORTANT NOTICE**

Special Movement Requirement:

Any exhibitor who requires special services due to location of exhibit, movement outside the schedule listed in the show manual or any show management/hotel mandated moves will be charged additional installation and dismantle labor (at the prevailing rate) as well as a showsite storage rate of $80.00/CWT based upon incoming weight with a 200 lb minimum.

RE: Showsite Drayage

Warning: Failure to read this notice can lead to unanticipated expenses

in your material handing charges.

The Hilton New York Hotel’s package-receiving department has limited capacity for shipments of materials. Be advised that an additional package room charge will be applied to your invoice for any and all materials that go through the Hilton New York Hotel.

All items received in the package room will be released to Expo Advantage for delivery. Therefore you will be charged for the Showsite Material Handling and the applicable Hilton New York package room charge listed below:

Boxes up to 15 lbs $4.00

Boxes 16 to 30 lbs $6.00

Boxes 31 to 50 lbs $12.00

Boxes 51 to 75 lbs $18.00

Boxes 76 to 99 lbs $25.00

Boxes 100 lbs PLUS $25.00 plus $0.20 per pound over 100 lbs

Plastic Pop-up Display Cases $35.00

Pallets $60.00 per pallet

**these rates are the determination of the Hilton New York Hotel and are subject to change without notice

The Hilton New York Hotel’s package room will not accept crates or any items received damaged during shipping, considered unsafe or too large. Expo Advantage USA will not be responsible for the condition or quantity of any materials sent to the Hilton New York Hotel’s package room.

Follow these guidelines to avoid these additional charges:

• Ship all materials to the Expo Advantage Warehouse by Friday, October 31, 2008. Listed Advanced Warehouse Drayage charges will apply.

• When shipping directly to the showsite, do not ship via Federal Express, UPS, DHL or similar package carriers. These items will go directly to the Hilton New York Package Room first, thus package room charges will be assessed.

• When shipping directly to showsite, schedule your shipments to arrive only during Exhibitor Installation. Anything arriving before may be refused by the hotel or if accepted will be subject to package room fees in addition to showsite material handling.

Material Handling / Drayage

Special Showsite Considerations:

**Service doors on the 3rd floor measure 11’”W x 12’H**

** Elevator to 3rd floor measure 26’L x 10’11”W x 13’11”H**

**Call for Pavilion Elevator Access Sizes**

Expo Advantage USA, Inc. is the official drayage handler for the conference being held at the Hilton New York Hotel. Expo’s services must be used to bring all materials into the hall except those that can be hand carried by one person in one trip. Be advised, Expo Advantage USA, Inc. will not accept any collect shipments. All common carrier & truck shipments must have a certified weight ticket when delivered. Failure to provide such a ticket may result in surcharges to have Expo Advantage USA take the shipment to be weighed. Be advised, on shipments without proper paperwork, the receiver’s decision to obtain a certified weight ticket or their estimate of weight will prevail. It is recommended that all shipments be sent to the advanced warehouse.

PACKAGE A – Advanced Warehouse Shipments

In order to be as efficient as possible, we strongly urge Exhibitors to advance ship their freight to Expo Advantage USA's warehouse by Friday, 10/31/08, 3:30 p.m. Shipments will be received from carriers prior to move-in with free storage up to thirty (30) days prior to show. Freight will be delivered from our warehouse and delivered directly to the Exhibitor's booth. Includes removal, storage, return of empty packing cases when necessary and delivery from the Exhibitor’s booth to the loading dock for outbound shipping from the hotel or Expo Advantage USA's warehouse.

Loose, Skidded and/or Crated Materials: $247.00 per CWT

Minimum Charge: 200 lbs.

Shipments received after the deadline of 10/31/08, are subject to a 30% late charge.

PACKAGE B – Direct Showsite Shipments

Shipments received at the convention site from owner’s vehicle or outside carriers will be unloaded and delivered to the Exhibitor's booth. Includes removal, storage, return of empty packing cases when necessary and delivery from the Exhibitor’s booth to the loading dock for outbound shipping from the hotel or Expo Advantage USA's warehouse.

NOTE: Do not ship to the exhibit area (hotel) prior to the first day of move-in. The hotel has limited space to store exhibit material prior to installation. Any freight sent to the hotel prior to exhibitor move in will be subject to additional surcharges. All Freight should be sent to the advance warehouse to avoid delays and possible refusal at hotel.

Loose, Skidded and/or Crated Materials: $295.00 per CWT

Minimum Charge: 200 lbs.

Due to hotel loading dock limitations, ALL exhibitors desiring to make showsite deliveries MUST provide advanced, written notice. After receipt of showsite drayage request, Expo Advantage USA will forward confirmation with a delivery date and time window. Exhibitors who miss their confirmed

delivery window or neglect to contact Expo Advantage USA with their showsite delivery request will incur an additional 25% surcharge to the above listed showsite rates.

PACKAGE C – Non-Scheduled Materials Left on Show Floor at the Close of Show

At the close of the show, any materials that were not subject to Advanced Warehouse Drayage or Showsite Drayage will be considered part of Package C. These materials will be delivered from the Exhibitor’s booth to the loading dock for outbound shipping from the hotel or Expo Advantage USA's warehouse, the charge will be:

Package C: $75.00 per CWT

Minimum Charge: 200 lbs.

Every effort will be made ensure timely outbound shipping of your exhibit materials from the

Expo Advantage warehouse. However, exhibitors must be aware that potential delays occur due to removing freight from exhibit halls and returning to warehouse, logging in at warehouse, and preparing for outbound pickup by carrier. Expo Advantage will not be held responsible for any shipments that are delivered late. Any shipment not marked with necessary arrival dates will be treated as a non-priority shipment. Expo Advantage can, at its discretion, ship any shipment COLLECT.

When scheduling your carrier for showsite pick-ups, be aware that trucking waiting time may apply. Expo Advantage must return all empties to the show floor and materials must be packed prior to proceeding with outbound freight pick-ups. To assist in keeping freight movement orderly, all showsite carriers must inform check-in personnel on the loading dock of their arrival and than proceed to the Expo Advantage service desk to sign in.

Make sure all shipping agents know the Exhibiting Company’s Name.

LABELING

Outbound freight that is not properly labeled by the Exhibitor or the Exhibitor’s Designate and/or the outbound bill of lading is not properly filled out and authorized by the Exhibitor will incur the additional charge of $18.00 per CWT, 200 lb minimum.

Note: Material Handling/Drayage charges are based on a 200 lb minimum per shipment.

Shipping Instructions for Drayage Orders

All shipments, by whatever the means of transportation, MUST BE PREPAID. Collect shipments will not be accepted. Shipments should be addressed as shown below. Shipments arriving without advance written order will automatically be handled as Package B, Direct Showsite Shipments and the delivery of said freight will be construed as acceptance by the Exhibitor of the terms and conditions set forth in Package B.

Do not ship to the exhibit area (hotel) prior to the first day of move-in. The hotel has limited facility to store exhibit material prior to installation. All Freight should be sent to the advance warehouse to avoid delays, possible refusal and surcharges at hotel.

A Bill of Lading must be prepared for all inbound and outbound shipments whether using

Package A or Package B. Included on this bill of lading must be: number of pieces; weights of pieces (inbound only); certified weight ticket (inbound only); carrier name; shipper information; and consignee information. Upon shipment, a copy of the inbound bill of lading must be mailed/fax to Expo Advantage USA, Inc. If certified weight ticket not provided, the receiver's estimate of the weight will prevail or a surcharge may be imposed to take materials to be weighed. An outbound bill of lading must be provided at the close of show. All outbound freight must be properly labeled by the Exhibitor or their designated contractor. Outbound freight that is not properly labeled and/or the outbound bill of lading is not properly filled out will incur a surcharge as outlined on Paqe 14. Labels and outbound bills of lading will be available at the Expo Advantage USA, Inc. service desk.
