Regional Forum on Hospital-Community PartnershipsDate and time: Tuesday, October 28, 2014; 10:00 a.m. – 1:30 p.m.Location: The Greater Waldorf Jaycees Community Center3090 Crain Hwy, Waldorf, MD 20601Purpose: The forum will highlight several innovative partnerships and cross-setting collaborations that are under current implementation and discuss how these programs fit into the larger context of the state’s new Medicare All-Payer Waiver, State Innovation Model/Community Integrated Medical Home, and promote ongoing population health improvement efforts. Outcome: To replicate, expand, and sustain innovative hospital-community partnerships.
