This presentation takes the next step forward, providing community case managers with information to assist their Veteran clients in navigating the complex but amazing benefits available through the VA. “Presumptive” conditions and disability benefits will be identified. The Mission Act and how it improves Veterans access to healthcare will be described and there will be a discussion of additional VA programs and benefits that assist case managers in directing not only Veterans healthcare, but also their general well-being. She is an ANCC Certified Case Manager since 2005, a Case Management Society of America (CMSA) member since 2004 and published author. She is active in the Central Virginia Chapter of CMSA (Past President) and is Past DoD/VA Advisor to the National CMSA Board. She served as the Co-Chair for many years for the CMSA Military/VA/DoD Conference Planning Committee. She has been a presenter at both the local and National CMSA levels and in 2010, received the National CMSA Award of Service Excellence. She is serving as an ANCC Nursing Case Management Content Expert Panelist from 2018-2021. In Mar 2018, she was the recipient of the American Nurses Credentialing Center’s 2018 Certified Nurse Awards in the Nursing Case Management category and in May 2018 was selected as the RICVAMC 2018 recipient of the Department on Veteran’s Affairs Secretary’s Award for Excellence in Nursing in an Expanded Role. After 25 years serving as a case manager for the Department of Defense (DoD) and the Veterans Administration (VA), she retired in January 2019 from Federal Service. This presentation will deconstruct the components of the MHS and explain how the various parts contribute to the overall care and case management of a military patient. Her excitement and confidence in the principles of case management inaugurated the Air Force’s first independent Nurse Managed Clinic staffed exclusively by case managers, where patient outcomes were game changers for medical management and population health.
