What is the Ages and Stages Questionnaire (ASQ) System?

Ages and Stages Questionnaire is comprised of the following:

• Parent/Caregiver completed screening tools

• Series of questionnaires for children 4 months to 5 years

• Periodic assessments at 6 month intervals up to age 3, then annually

• Identifies children in need of further assessment

• Encourages parent involvement

• A brief assessment procedure designed to identify children who should receive more intensive diagnosis or evaluation

• Early intervention (EI/CVRC/EPU)

• Early childhood special education (ECSE)

• Mental health/social service

• Health systems
