APPLICATION FOR CLINICAL TEACHING INSTRUCTIONS

Please return all forms to the Certification Officer by Wednesday, October 30, 2019.

The Clinical Teaching Application and its associated forms will be a direct reflection of your professionalism. Please complete the forms carefully.

The forms you must complete and return are:

1) Application to the UD Department of Education, Education Preparation Program with Resume and Essay. Please review the included instructions and rubric for the essay. – For permanent File.

2) Clinical Teaching Assignment Request Form – For permanent File

3) Final Exam/Break (Winter/Summer) Schedule Form

4) Personal Data Form – Type or use manuscript. Please be comprehensive and thorough in filling out this form. Do not submit without A COLOR PHOTO which you will adhere to the forms.

5) A Criminal History Information Release Form with Copy of Driver’s License and Resume for Criminal Background Check. Please note that if you are not placed with Irving ISD then you will be required to complete a criminal background check for appropriate school &/or school district. You will be notified via email by the Certification Officer if this is required. – Required by State of Texas for any occupational license.

6) Student Demographic Form for TExES Testing Authorization

7) Unofficial transcript as proof of GPA – You may download transcript from Banner.

TEA Requires the completion of 12 content hours for all certifications fields except Math and Science grades 7 or higher which must complete 15 hours.

8) Basic Skills Requirements or proof of exemption – Please see requirements for passing scores and if you are unable to submit passing scores, please contact the Certification Officer for exemptions.

9) Reference Forms (2) with Application to Teacher Certification –

a. For EC-6 Certification – Please ask two current or former EDUCATION professors to complete and return the reference forms by required due date to the Education Department.

b. For Middle or High School Certification - Please ask one professor from your discipline (content area of certification) and one from the Education Department. For Permanent File

10) Graduate Applicants you must complete your graduate application and admission requirements BEFORE you begin student teaching. A letter of acceptance must be received from the Braniff Graduate School and included in your permanent file with the Education Department.

Clinical Teaching Orientation for Fall 2020 applicants will be on April 16, 2020 from 4:30 – 5:30 pm.

If you choose to leave the University of Dallas, Educator Preparation Program, after being accepted, you are required to notify the Certification Officer in writing within one business day of leaving.

Students who have not completed all general education requirements may be accepted on a one semester conditional acceptance. You will be notified in writing by the Certification Officer if this occurs.
