However, we can also provide toys and treats here at the facility.Food should be brought preferably portioned in zip-lock baggies, 1 bag for each feeding. If we need to add canned food, raw food, medication, or supplements, we must have specific feeding & medication instructions. We highly recommend that your dog stays on their normal diet so that their stay can be as consistent as possible. Changes in diet can lead to upset stomachs and lack of appetite. All of your dog’s belongings must fit in our bins. No large bags or plastic containers. If food is not portioned into single feedings, one large zip lock bag may be used.We can provide food for your pet’s stay at an additional charge. Some medications (Insulin, Fluids etc.) can be administered for an additional charge. Please have them properly labeled with instructions and your Veterinarian’s Information.Collars with proper identification and a leash may be left with your pet(s).*No retractable leashes or shock collars please. We consider them unsafe and we will not use them on your pet*Day Socialization:Please do not bring your dog’s toys, as they may not want to share, we have plenty to keep them occupied. If your dog needs a feeding, please bring each feeding in a Zip-lock baggie with your pet’s name.Collar with identification and a leash can be left with your dog.*No retractable leashes or shock collars please*Initial: _____________Health & Sickness*ALL Vaccinations required to stay at the facility*Rabies, Bordetella, Distemper (or your Veterinarian’s version of this vaccine), yearly Fecal test and Flea preventative are REQUIRED. If your pet receives a live Bordetella Vaccine they cannot come to the facility for 10 days after the vaccine. (Non-Live are available ask your Vet).A onetime Waiver is available for Bordetella and a waiver for Distemper is available (with valid medical reason from your veterinarian, such as Distemper allergy). The health and wellbeing of all the pets in our care is our top priority. Just as when children or adults are around a large number of others, such as a school or office setting, sickness, viruses, & injuries can occur. Vaccinations are a precautionary measure but will not guarantee your dog will not become sick. Bordetella, or kennel cough, is a prime example, as the vaccine only covers a small number of the many strains of the virus just like the flu vaccine for people. Just as in people, an animal can be a carrier without showing any symptoms. There may be an incubation period where the dog is not showing symptoms but may have an illness. If your pet is showing any signs of illness such as coughing, diarrhea, discharge from eyes or nose, or does not seem to be in normal health, please do not bring your dog to the facility. We would ask that you call to let us know the symptoms and treatment your dog is having so we may best protect the other pets in our facility.Health & Sickness ContinuedInjury Whenever multiple dogs are together there is always a chance of injury. We do our utmost to be sure the dogs are safe and play is appropriate. If an injury does occur you will be notified to discuss what action you would like us to take. If you are not reachable, your veterinarian on file will be contacted to discuss action needed, or if no Veterinarian is on file or is not reachable we will contact a veterinarian of our choice. We thank you for your cooperation in keeping your pet as well as others here at the facility safe and healthy.Expectations of Normal Dog PlayRemember that normal dog & puppy play does include mouthing, slobbering, using their paws, jumping, wrestling, & vocalization. Dogs or puppies with very short coats or light or pink skin are more prone to and may get scratched or nipped during normal dog play. We do our very best to monitor and keep play appropriate but these types of issues will always be a possibility during normal dog play.Initial: ____________ ................
