Outpatient Infectious Diseases Clinical Pharmacy SpecialistPosition Start Date: Position available now.Salary: Based on experiencePosition Type: Clinical Pharmacy SpecialistPosition Description:Parkland Health and Hospital System (PHHS) seeks a motivated individual to provide infectious diseases focused services within our Outpatient Parenteral Antimicrobial Therapy (OPAT) and Infectious Diseases clinics. The individual selected will work closely with a multi-disciplinary team, including ID physicians, medical residents, and nurses, in the outpatient setting to optimize the transition of care for patients requiring home IV antimicrobial therapy as well as providing antimicrobial stewardship and evidence-based antimicrobial therapy recommendations.Major Job Responsibilities: Provide patient-specific pharmacotherapy recommendations based on patient assessment and response to antimicrobial therapy, identifying medication adherence, acquisition, and administration barriers, and developing plans to optimize antimicrobial therapyMaintain seamless transition of care for patients discharged on home IV antimicrobial therapyPromote rational, safe, cost-effective antimicrobial therapy for infectious diseases patients through participation in quality of care and patient safety initiatives via the antimicrobial stewardship and antibiotic subcommitteesAssist inpatient clinical pharmacy specialists in evaluation of patient eligibility for OPAT clinicParticipate in infectious diseases related quality initiative and research projectsWork with peers to achieve team, departmental, and health system goalsSupport the educational mission of PHHS by precepting PGY1/PGY2 pharmacy residents and pharmacy students on OPAT rotations and participating in staff development initiatives within PHHS Engage in professional development via participation in local, state, and national professional organizationsRequirements:Pharm.D.; 2+ years of clinical practice experience preferred; PGY2 residency or comparable experience in infectious diseases or ambulatory care with experience in ID is required; Licensed or eligible to practice pharmacy in Texas or able to obtain Texas licensure within 90 days of hire.Facility:Clinical pharmacy services at PHHS include involvement in infectious disease, critical care, oncology, general medicine, antimicrobial stewardship programs, medication reconciliation, comprehensive ambulatory care services, emergency medicine, pain management and HIV.Contact Information:Steven S.
