|Job Purpose: |

|This role encompasses Clinical management of a group of patients within the Assessment and Treatment Centre (ATC). The post holder has |

|responsibility for triage of patient referrals, timely patient interventions and discharge. |

| |

|They will provide leadership for nursing within the clinical area and support nursing staff on all nursing issues. |

|The post holder will initiate and undertake / supervise specific projects / audits related to the professional development of nursing, monitoring|

|standards of care and acting as an agent for change achieving a culture that is flexible and responds positively to the demands and future |

|development of the service. |

|The post holder will initiate and undertake specific investigations (according to NMC Code of Conduct) related to the management of the patient. |

|To develop patient pathways/protocols to improve patient clinical management. |

| |

|The post holder will play a pivotal role in the day-to-day patient management, leading patient assessment processes and initiating treatment as |

|per pre agreed protocols and pathways. |

| |

|The post holder will be working in an unpredictable environment within the Clinical area (e.g. violence and aggression, Clinical emergencies) |

|Registration with the Nursing and Midwifery Council (NMC) must be maintained |

|Be fully conversant with the trust major incident plan, and ensure that all nurses on the ward understand the procedure. |

|Clinical Skills |

| |

|To act as an expert clinical practitioner, role model and advisor, undertaking advanced nursing skills within their defined clinical area. |

|Be responsible for assessing, planning, implementation and evaluation of care to a group of patients from admission through to discharge on the |

|unit and be required to teach nursing and non-nursing staff. |

|Advise on the promotion of health and the prevention of illness. |

|Contribute to service developments using evidence based practice |

|Set and monitor standards of care that encourage innovation and involvement of all nursing staff working on the unit. |

|Carry out those activities necessary to conduct a comprehensive assessment of a person’s nursing requirements. |

|Recognise and avoid situations that may be detrimental to the health and well being of the individual and team |

|Act as the patient’s advocate. |

|Complete patient discharge summaries to facilitate timely discharge |

|Will develop and disseminate knowledge of services available in the community to facilitate the planning of effective discharge |

|Provide information and specialist knowledge to patients and carers in making informed choices regarding their health and social well being |

|Communication Skills |

|Act as a liaison role for the /unit by providing effective communication, direction and support to staff, associate groups, patients and |

|relatives on a daily basis, to ensure patient care is conducted in a timely manner. |

|Facilitate and maintain an efficient communication system that will ensure the dissemination of medical staff instructions for improvements in |

|patient care. |

|Liaise with inter-hospital departments/personnel as required |

|Liaise with external agencies e.g. Primary care trusts, Social services Universities as required |

|Persuade and motivate patients and relatives in aspects of their care as appropriate |

|Attend appropriate multidisciplinary team meetings. |

|Maintain effective channels of communication with the patient’s relatives and all other staff involved in the patient’s care. |

|Offer polite and courteous attitude to patients, visitors and members of the multi-disciplinary team |

|Receive and deal with sensitive information according to trust policy |

|Offer support to patients and relatives in a sensitive and empathetic manner when distressing news has been received, or when nursing terminally |

|ill patients |

|Liaise with senior members of the trust team e.g. Site Manager, Urgent Care Matron, Discharge Manager and other operational Managers with regard |

|to the timely admission and discharge of patients. |

|Liaise with senior members of NHS North Somerset team. |

|Management Skills |

|Manage the care of a patients throughout their stay in the unit, ensuring timely discharge is achieved |

|Participate in the development of professional nursing policies and protocols and represent the unit at professional meetings. |

|With the Medical staff develop patient condition specific management plans to improve patient clinical management. |

|Monitor all patient related documentation to ensure it is completed correctly and accurately. |

|Participate in the selection, recruiting and retention of staff within the clinical area |

|Perform and oversee individual performance reviews on an annual basis of the nursing team and act upon individual needs. |

|To co-operate fully in the teaching of medical students and trainees from the Professions Allied to Medicine |

|Ensure there is a robust system for transferring information between staff and external teams in order that such information is transferred |

|smoothly and effectively with due regard for clinical accuracy and patient confidentiality |

|Work closely with the Divisional teams to develop services to meet Trust objectives and national targets |

|Competent at advanced nursing practice appropriate to area |

|IT literate |

|To provide guidance, supervision and support to junior members of staff and students. |

|Develop local induction programme and orientate new members of staff to their duties, ensuring that job descriptions are discussed and that they |

|are familiar with the Trusts policies and procedures. |

|Take every opportunity to maintain and improve knowledge and competence, and take steps to keep up to date with Clinical and Nursing |
