Title: Challenges of Treating ARFID (Avoidant Reactive Food Intake Disorder)

Mary J. Sanders, Ph.D.

Clinical Associate Professor

Stanford University chool of Medicine, CA 94305, USA

The purpose of this paper is to present the necessity of a team approach for the assessment and treatment of ARFID as well as propose a therapy approach that incorporates fear externalization, exposure, thought challenges, and family-based support.

ARFID is a new diagnostic category in the DSMV. It describes a situation in which someone avoids foods due to fears other than body image that results in nutritional deficiency and/or emotional distress. The fears may range from a fear that the food will not taste good to fears of choking, nausea, allergies, illness, etc.

Dr. Sanders will present an overview of a team treatment of ARFID, which involves assessing the food fears, and involves a team approach to rule out medical issues and increase nutritional balance. She will present a treatment model with incorporates externalization of the fears, and fear challenges through a combination of food chaining, ERP (Exposure Response Prevention, CBT (Cognitive Behavioral Therapy) and Family-Based support.

She will address the importance of medical evaluations that may be required to rule out possible medical etiology of symptoms (GI, allergies, food sensitivities, swallowing studies, IBS, celiac, etc.) as well as discuss the self-fulfilling prophecy of how malnutrition may contribute to symptoms, such as nausea and constipation.
