IFTA Renewal Notification

IFTA licenses must be renewed no later than December 31, 2016. To ensure timely issuance of your IFTA License, the following renewal requirements must be submitted along with the IFTA Filing Fee of $10.00.

Renewal with No Changes:

• IFTA License Renewal Application, form # 96-0424

• Record Keeping Agreement, form # 96-0537

• Power of Attorney: Motor Carrier, form # 96-0441 (if applicable)

Renewal with Changes:

• IFTA Account Changes Application, form # 96-0430

• Record Keeping Agreement, form # 96-0537

• Power of Attorney: Motor Carrier, form # 96-0441 (if applicable)

• Copy of the current registration (for each additional vehicle requiring IFTA decals)

• IFTA Lease Agreement Certificate, form # 70-0901 (for each additional vehicle requiring IFTA decals)

• When requesting an address change for the business location, verification of the physical Arizona address is required. Include a current copy of one of the following acceptable documents (the applicant name must be indicated on the document):

o Real-estate ownership document

o Real-estate lease agreement

o Rent or mortgage payment receipt

o Utility bill with applicant's name on it

o Current copy of the IRS form 2290

o Copy of your Articles of Incorporation

Please Note: The IFTA application process has been updated to reflect the following changes:

• Identify the fuel types for which you will file quarterly returns

• Assign e-mail addresses to designate contact information for: IRP, IFTA, Primary, Audit and Record Keeper
