However, additional hours may be required to complete duties.POSITION SUMMARY: The Health Coach is part of the care team and under the supervision of the Clinical Pharmacist. The Health Coaches will work in conjunction with the Clinical Pharmacist to assist the care team with direction and resources regarding pharmacotherapy aimed at meeting the needs of the patients and assist in meeting organizational goals. The Health Coach is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of care.SPECIFIC DUTIES AND RESPONSIBILITIES:Coordinate communication between patients, families, and caregivers with the multidisciplinary care team in order to identify barriers and support care plan.Provide education, training, and support to the care team and patients on medication therapy, self-care, utilization of home equipment and disease states.Assists in the development, implementation, and communications of programs or projects to promote preventative screening, population management, and high risk population management.Assist in collection and entry of patient specific data into the EMR.Ensure patient confidentiality.Ability to properly process data and compile reports on various projects.Notification and recall of patients regarding missed appointments and care gaps.Ability to communicate with patients and staff daily in a courteous and professional manner, address questions and direct calls promptly.Serve as part of the care team; following and implementing workflows that support and sustain MHSI Patient Centered Medical Home model of care.Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI.Participates in all Risk Management activities as deemed appropriate. Additional duties or responsibilities as deemed necessary.SKILLS & ABILITIES:Education: Associates Degree in Nursing Required or Bachelor Science in Nursing preferred Experience: RN with one year or more related experience preferredComputer Skills:Computer skills with capabilities to utilize Excel spreadsheets, graphs, and Word documents preferred. Ability to learn and utilize electronic record keeping systems such as electronic medical record required. Certificates & Licenses:Current licensure as a registered nurse required by Arkansas State Nursing Board.BLS and ACLS certification required. Other Requirements:Willingness to attend and/or complete continuing education related to this position and /or any other topic deemed necessary to MHSI. Detail oriented, positive attitude, and the ability to take direction and work well with a team but also self-motivated. REPORTING RELATIONSHIP:The Health Coach reports to and is supervised and evaluated by the Clinical Pharmacist.EVALUATION:The evaluation of work performance will be ongoing, and will be carried out by the Clinical Pharmacist. It will include specific duties and responsibilities of this position description plus employee attitude and general working behavior.
