DOCUMENTS REQUIRED TO APPLY FOR A SCHENGEN VISA

(C visa → 1 to 90 DAYS)

PURPOSE OF TRAVEL: TOURISM OR TRANSIT

Document check-list

1. Schengen Visa application form: all relevant fields in the form must be filled, the names appearing on the passport must match the ones on the form. The applicant must date and sign the form before the Visa Officer;

2. Recent passport size picture (3.5 x 4.0 cm): white background, full face and front view. Scanned or photocopied photos will NOT be accepted;

3. Passport and a photocopy of its main pages, as well as of any other pages with visa stickers and Airport Authorities’ stamps;

NOTE: your passport must be valid at least for 90 days beyond the expiry date of the visa, must have at least 2 blank pages and must have been issued not more than 10 years before.

If you are currently a Permanent Resident (PR Card Immigrant category CR8/RC8) but initially arrived in Canada as a refugee, you must submit a Travel Document for Refugees issued by the Canadian authorities.

Please note that the Canadian certificate of identity is not recognized as valid travel document.

4. Your Permanent Resident Card ( original plus photocopy of both sides) or your work or study authorization (original plus photocopy) NOTE: your work or study visa must be valid for 90 days beyond the expiry date of the Visa, and must have been issued for multiple entry;

5. Your Driver’s License (original and Photocopy) or any other proof of residency valid as proof of residence in the jurisdiction of this Consulate (Quebec, New Foundland and Labrador, Nova Scotia, New Brunswick, PEI, Nunavut);

6. Confirmed flight reservation showing complete itinerary. If you plan on visiting more than one Schengen country, please provide means of transportation between countries. Please be informed that if your application is approved you might be asked to provide the E-ticket ;

7. Proof of lodging for the entire duration of the trip and for all the countries you intend to visit: hotel reservation (original plus photocopy) with name and address of hotel, name and number of guests and duration of stay. For organized trips and cruises please provide vouchers with all the information on the places to visit and length of stay

OR

Letter of invitation (check the OTHER FORMS box) from a relative or friend who is a permanent resident in Italy. Remember to include a photocopy of your host’s valid photo ID (if EU citizen) or if needed host’s “Permesso di soggiorno” (Residency Permit);

8. Verification of employment: a letter from your employer, maximum one month old, stating your full name, date of birth, address, position with the company, date of employment, monthly salary and approved vacation time;

If you are self-employed: company’s Certificate of Registration and the most recent Income Tax (T4) receipt;

If you are a student: official letter from your school/University stating you are a full time student;

9. Proof of financial means: Your bank statements for the last three months (originals plus photocopies). If you are self-employed, you must also provide a business bank statement. Online banking print-outs will not be accepted;

10. If you are financially dependent on your mother/father/spouse, you must provide an affidavit of support (check the OTHER FORMS box) signed by mother/fathers/spouse before the Visa Officer OR duly authenticated by a Commissioner for Oaths, along with your mother’s/father’s/spouse’s bank statements for the last three months and a letter from his/her employer, maximum one month old, stating full name, date of birth, address, position within the company, date of employment and monthly salary. Remember to include a photocopy of your mother’s/father’s/spouse’s valid photo ID and a photocopy of birth/marriage certificate

11. If you are under the age of 18 and are traveling with one of your parents the “parental consent form” (check the OTHER FORMS box) duly completed and signed, is also required. If you are traveling without your parents the “assumption of responsibility for the minor in Italy form” (check the OTHER FORMS box) will also be required

12. Proof of medical travel insurance (minimum coverage required is 30.000 Euro = CAD 50.000) Please provide insurance documentation (original and photocopy) in which is clearly specified your name, time period and maximum amount of coverage for medical emergencies, hospitalization and repatriation;

13. Visa fee (non refundable) in the form of cash or Money Order, payable in Canadian Dollars to the Consulate General of Italy in Montreal, with applicant’s name and address as the remitter. Personal Checks will not be accepted;

14. Return envelope: If you prefer to have your Passport with the visa mailed to you, please provide a Pre-paid self addressed Express Post envelope (available at Canada Post offices).

PLEASE NOTE:

The Visa Office has full authority to evaluate and request any other documents in addition to those submitted by the applicant. The applicant is hereby informed that submitting the documents does not guarantee issuance of the visa.

INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED.

Rush processing is not possible.
