Managed Care Division - United States Army

Population Health Center

To Our Valued Patient,

Thank you for completing your colorectal cancer screening last year! Fecal occult blood testing (FOBT) is recommended annually and according to our records, it is time for you to repeat the screening procedures. Colorectal cancer is the second leading cause of cancer-related death in the United States, and both men and women are at risk. By getting colorectal screening tests regularly, colorectal cancer can be found and treated early when the chances for a cure are best.

For your convenience, the staff has entered an order for you for FOBT (the only test you can do at home.) Please review carefully all the directions for testing, including dietary restrictions, that are included in the kit. Bring the samples back to any CRDAMC laboratory. DO NOT MAIL SAMPLES back to the hospital! If you have not been contacted with your test results within 3 weeks of returning the samples, please call (xxx)xxx-xxxx to obtain them. If your test is positive, further testing will be recommended.

At your next scheduled appointment, you should ask your provider about other screening tests that may be recommended for you such as: screening for osteoporosis, risk factors for heart disease, and other cancers.

If we have identified you incorrectly and you have had a recent colorectal screening test, please contact the Population Health Center at (xxx)xxx-xxxx so that we may update your information in our system.

Thank you for allowing us to be of service to you.

Sincerely,

Colonel, US Army

Deputy Commander for Clinical Service
