Register and submit your initial claim for disability online at:

Filing an initial disability claim is a two-step process:

Step 1: Complete the one-time online registration. Step 2: File an online disability insurance (DI)

claim.

Once your claim has been submitted online please provide us the following:

1. Name and Kaiser Permanente Medical Record Number

2. Your Patient Receipt Number (provided online by California Employment Development Dept.{EDD}

3. For EDD extensions provide Kaiser Permanente with your claim ID number

4. Include timeframe, start and end date, and condition of your disability.

5. Contact Phone Number.
