Duke –Affiliated SRAHEC Residency annual report 2017-2018Overall statusThe 2017-2018 year of our residency was well supported and thrived under the guidance of our DIO Dr. Deborah Teasley and the program director, Dr. Sushma Kapoor. Dr. Deborah Teasley retired effective June 29th 2018 and Dr. Sushma Kapoor stepped into the role of the President and CEO of SR-AHEC. She also serves in the capacity of the DIO and Interim Program Director. We continue to grow as a family medicine residency program that specializes in integrated care and personalized education.Our Family Medicine Residency believes the heart of healthcare education is service to others by training outstanding family medicine physicians in our Program. Resident Training and education and faculty development are a priority, as is meeting the requirements of the ACGME, RRC, and NAS guidelines. We are on target with submission of our ADS data and our self-study date is October 1st 2020 (approx.).We relinquished our AOA Accreditation and have now received the Osteopathic recognition status.The Program Evaluation Committee met and performed the required Annual Program Review March 2 – April 23, 2018. The report was submitted to Dr. Kapoor on May 3, 2018. Dr. Kapoor to present the report to the residents and faculty on June 8th.The report was approved by our DIO on June 8th 2018.The Residency has made major advances in learner evaluation, longitudinal and developmental curriculums QI and Safety education. We have continued to strengthen our relationship with CUSOM and continue to be the Core teaching site for the VCOM MS3 students. We celebrated our first anniversary in our new building on June 19th 2018.The staff continues to be excited and impressed by the building and it has helped us tremendously in our recruitment as well. We have continued to expand our clinical services including the ADHD Clinic and expanded our diabetic services as well. These services help meet the needs of the community and also additional learning opportunities for our residents. We continue to incorporate peer-peer evaluations; face-face evaluations of the in-patient hand off process and transition of care for patients released from the hospital and emergency department. New Innovations Resident Management System has facilitated improved evaluations that are standardized and returned within 14 days for improved feedback. The information is compiled with procedures documentation, ITE results, participation in didactics, and formative and summative evaluations that are reviewed in quarterly faculty and Clinical Competency Committee meetings. The information is used to make changes to the FMR, FMC, and individual resident’s progress. Residents continue to log their procedures. In addition, all DO residents now log in their ambulatory OMM patient visits as well and we track them during our quarterly FMGMEC meetings. And pediatric patient encounters in hospital and emergency settings. Residents also attend the Patient –Safety committee meetings at CFVHS during their Intern year ICC rotation.Scholarly activity Curriculum/QI ProjectQuality ImprovementThe multi-disciplinary QI Committee and its two work groups (PCMH and MACRA/MU) continued to meet monthly. A SharePoint collaboration web site is used by the Committee and its work groups.The Committee’s focus has been on:PCMH implementation and recognition (aided by its PCMH Work Group)Meeting MACRA/MU requirements (aided by its MACRA/MU Work Group)Resident QI projectsResidents work as cohort teams and alongside research mentors in conducting research projects each year. A faculty Research Coordinator will assist residents during each project phase toward completion. Research mentors will provide structured evaluation of each project. As a result of this curriculum, residents will develop research skills, complete scholarly projects, and present findings to a professional audience. After completing this three year curriculum, residents will be able to:Develop realistic and meaningful research questions for empirical studyChoose appropriate research methodologies for a particular research questionUnderstand data extraction and analysis processes and proceduresUnderstand how to prepare an abstract or manuscript for publicationPractice effective quality improvement strategiesUnderstand basic requirements of Meaningful Use and Patient-Centered Medical HomeUnderstand Institutional Review Board process and important of protecting human subjectsThe projects for 2017-18 were as follows:PGY1: Project involves how to effectively implement the requirements of the impending STOP Act in the FM clinic. The project has been implemented, and they are currently waiting for results to be compiled. PGY2: Project involves reducing obesity and encouraging weight loss among patients in the FM clinic. Project has been implemented, currently in the data collection phase. PGY3: Project involves improving diabetic screening protocol for FM clinic patients by discerning most sensitive indicators, e.g. diabetic foot exam. Outstanding ResidentsOutstanding residents from each class were selected to represent the program at the NCAFP Annual Winter Meeting in Dec 2017. This year’s outstanding residents are Dr. Sterling Riddley, Dr. Ayesha Safo and Dr. Allie Whitley.CLER VISIT Our initial CLER visit was completed in August 2017.The response to the visit is attached and below is the follow up on the visit.1-One of the action items noted by the review team was the need to have an anonymous reporting system for adverse events, and near misses. In the past we utilized our Safety/Security Trending Analysis Report for this purpose, but it wasn't anonymous, and wasn't used for near misses.To comply with this action item we have developed the ADVERSE EVENT/INCIDENT/NEAR MISS REPORT FORM for staff to anonymously submit these type of situations. This form is located on our web site and is password protected to keep anyone who just comes across this form from entering information. There is a link to the page in New Innovations, along with the simple password that we have put in place.2-Mock code at SRAHEC completed 04/25/2018 11-1130am. Scenario of fallen patient. Overall, evaluation positive. Things to work on: response by providers to code 100. Evaluations sent to internal safety committee for review and recommendations to leadership --pending. This satisfies portion of CLER visit improvement plan. Also involves the clinic staff along with the providers.Faculty We have our new Behavioral Health Director Dr.Michael Shapiro.One of our graduating residents Dr.Sterling Riddley will stay with us as a Diabetes Fellow and Clinical faculty.Dr. Emily Ghassemi is our new Pharmacy Faculty from Campbell School of Pharmacy.We are actively recruiting for a Program Director for our Family Medicine Residency.Dr. Gardner will retire from the inpatient service attending role but will continue to provide patient care in the Specialty Clinic and also precept our Family Medicine Residents in the Clinic. ................
