The vacancies have arisen due to the need for additional consultant resource to introduce 7 day respiratory medicine cover, and to expand respiratory ambulatory care.The successful candidates will work as part of a busy, dynamic chest team. The duties of the successful candidate will include respiratory, sleep and lung cancer outpatient activity with a commitment to the Lung Cancer MDT, respiratory ambulatory clinics, inpatient care and endoscopy. Sessions will be provided for patient-related administration and supporting patient activities. Successful applicants will be encouraged to develop a sub-speciality interest that complements the existing team.Currently the Respiratory Physicians participate in the general medical on call rota (1:12) It is hoped that the appointment of the successful candidates will facilitate specialised respiratory on-call (1:8)Respiratory ServicesThe Wirral Lung Unit has an 8 bed “Lung Support Unit” that offers Non-Invasive Ventilation, High Flow Oxygen, enhanced monitoring facilities and skilled nursing support. There are an additional 37 standard respiratory beds. The Consultants share a junior staff complement of three Specialist Registrars, three Senior House Officers and two F1 posts. The Respiratory Department is also supported by a Consultant Nurse, two Advanced Nurse Practitioners, two Respiratory specialist nurses and two Macmillan Lung Cancer Nurses. There is a well-equipped respiratory laboratory including cardio-respiratory exercise testing. Within our unit we have facilities for overnight sleep studies and provide CPAP to patients with sleep apnoea. We have an advanced pleural service including provision of indwelling pleural catheters and local anaesthetic thoracoscopy.The Chest team see approximately 400 new cases of Lung Cancer each year. We are supported by a comprehensive weekly Lung Cancer & Mesothelioma MDT which is soon to be sectorised with the Countess of Chester Hospital. There is a weekly interstitial lung disease MDT / non cancer radiology meeting. We also have a TB MDT and the Unit contributes to a North West Severe Asthma MDT. There is a large community COPD and Oxygen team who provide an exemplary service in chronic COPD management, oxygen assessment and provision, pulmonary rehabilitation and early supported discharge. We have an active research portfolio with a dedicated respiratory research nurse. Successful candidates will be encouraged to expand our research profile. General ProvisionsThe appointee will have responsibility for the training and supervision of staff, including junior medical staff who work with him/her, and time will be devoted to this activity on a regular basis. If appropriate the appointee will be named in the contracts of doctors in training grades and/or nurses as the person responsible for overseeing certain areas of their training. The study leave entitlement of this post reflects the national Terms and Conditions for Consultants 2003.Access to a personal computer (including Internet access), appropriate IT support and training will be available if required. There will be support from doctors in training and specialist nurses. In addition there will be support from ward nursing staff, pharmacy and outpatient clinic staff, including nurses and dieticians. A nominated mentor will be available for the new appointee.Duties and ResponsibilitiesProvision to the Trust with other medical colleagues of a service in General Medicine. There will be responsibility for the prevention, diagnosis and treatment of patients with general medical illnesses, in particular those with respiratory conditions.The post holder will be responsible for respiratory and other medical referrals within the Trust and continuing care of patients who are not located on the respiratory ward.Professional supervision and management of staff in the department, for example doctors in training and Specialist Nurses. The post holder will participate in the training of those doctors in training. The post holder will also be required to participate in teaching. 4. The post holder will be required to attend audit days. The appointee will also be required to personally participate in Clinical Audit, Clinical Governance and Continuing Professional Development. The Trust supports the requirements for Continuing Medical Education as laid down by the Royal College of Physicians and is committed to providing time and financial support for these activities. Involvement in research is strongly encouraged. The LocationThe Wirral is commonly known as the “Leisure Peninsula” and is situated between the Mersey and Dee estuaries. The catchments’ population of approximately 357,000 consists of a mixture of semi-rural and urban communities. There are many pleasant locations to live, within very easy reach of the hospital. The schools in the Wirral are amongst the best in the country. Wirral is close to the major cities of Liverpool and Chester and conveniently located for North Wales and the Lake District. Sailing, windsurfing & hiking are popular pastimes; in addition there are 15 golf courses, numerous sports centres and tennis courts. Wirral is ideally placed for visits to the theatre (Liverpool, Chester, Manchester, Mold). There are also numerous cinemas and two bowling alleys. A wealth of historic monuments and museums exist within close proximity to Wirral. The shops in Liverpool, Chester and Manchester are within easy reach; in addition there is a large designer shopping centre at Cheshire Oaks near Ellesmere Port. The peninsula is served by the M53 central motorway spine, connecting to the M56 and M6 (Lake District - 1? hours travel time; Snowdonia – 1 hour; Manchester – 45 minutes; Chester - 20 minutes; Liverpool – 15 minutes). The Wirral has many features/facilities of a major city without the associated problems of transport anxiety. The hospitals are situated within 15 minutes of nearly all housing locations. Good rail links to the rest of the country are provided via Runcorn, Warrington or Liverpool stations. Wirral is also conveniently located for Manchester International Airport and Liverpool (John Lennon) Airport.Wirral University Teaching HospitalWirral University Teaching Hospital (NHS) Foundation Trust is based on two principal sites and consists of 820 inpatient beds. The hospital became a Foundation Trust in 2007. The acute services and intensive care facilities are concentrated at Arrowe Park Hospital site (724 inpatient beds), where there is also a busy Accident & Emergency Department which covers the whole of the Wirral Peninsula. The hospital contains all the major specialities and there is full pathological and radiological support including Angiography, CT scanning, Nuclear Medicine and MRI scanning services. Specialities on the Arrowe Park site include General Surgery, Urology, Orthopaedics, ENT, Ophthalmology, Maxillo-Facial Surgery, Haematology, Genito-Urinary Medicine and Critical Care (ITU and HDU). The Medical disciplines (including Medicine for the Elderly), Paediatrics and Women’s Services are also on this site, along with a dedicated Cardiac Catheter Laboratory, Haemodialysis Unit Coronary Care Unit and Lung Support UnitClatterbridge Hospital (5 miles down the M53 motorway) has a total of 96 in-patient beds. Services at Clatterbridge include elective surgery (Orthopaedic and General/Urology), the Limb Centre, Stroke Rehabilitation, Neurological Rehabilitation Unit, Dermatology and a satellite Dialysis Unit. The hospital shares the site with (1) the Clatterbridge Cancer Centre (CCC) a supra-regional oncology centre which is administered as a separate trust, (2) the Psychiatric Unit which is administered by the Cheshire and Wirral Partnership Trust and (3) Wirral Hospice St. John’s. The Trust’s experience and innovation in the use of information technology is recognised in its selection as a Global Digital Exemplar site. Arrowe Park houses a modern well equipped Education Centre with excellent lecture theatres, library and on-line facilities.The Chief Executive of the Trust is Mr David Allison, and the Medical Director is Dr. Susan Gilby.The Medical DivisionThe Division of Medicine comprises of medical specialities across adult and elderly care. The Division is led by Dr Ranjeev Mehra as Divisional Medical Director and Ms Nadine Armitage as Divisional Director. In total there are 41 Consultants in the Division.The Medical Division is comprised of 514 beds including, a 7-bedded Coronary Care Unit 8-bedded Lung Support Unit, and twenty four dialysis stations at Arrowe Park Hospital.There are also thirteen Consultant Geriatricians, five Consultants in Palliative Medicine and one Consultant in Rehabilitation Medicine. This would include teaching and training of junior doctors, CPD, audit and governance. There are regular good practice meeting replacing routine clinical work which has been factored into the allowance. In practice this amounts to 0.5 PA a week and the expectation, therefore, is that the routine week will consist of 7.5 PAs allocated to direct patient care and 2.0 PA for supporting professional activity. It is expected, that following the successful appointment of these 2 additional posts, a specialised respiratory on-call rota will be implemented on a 1:8 basis (with prospective cover) which would involve advising on respiratory patients, with review if appropriate, out of hours. The on-call commitment currently involves a 1:12, covering a mixture of unselected medical take. Weekend work (9.00am-1.30pm) will involve review of patients on the Lung Support Unit, and within the respiratory bed-base. This will receive an additional payment or time off in lieu. Consultants are expected to take phone calls in evenings, which would attract a 3% uplift. Consultants are very rarely called back to the Trust but are expected to in case of emergencies. TimetableThe provisional job plan is attached. It is likely that there will be a rotational basis to the cover provided by the 8 respiratory consultants to maximise job satisfaction, this would involve a week for ward work solely. Information TechnologyWirral University Teaching Hospital is a Global Digital Exemplar site. This includes inpatient noting, electronic prescribing, and electronic requests and results. Outpatient noting will soon be introduced. Disease registries, linking primary and secondary care, have been established for asthma and COPD. Wirral healthcare economy is working towards an integrated care record system. This will link not only primary, secondary and tertiary care, but also community healthcare workers, e.g. Opticians, Pharmacists and Social Workers. All successful applicants, whatever their position in the hospital, are expected to embrace and participate in digital technology. Stand alone data systems will be actively discouraged. An understanding of the technical aspects of computers is not necessary and appropriate training will be provided. Main Conditions of ServiceThe successful applicant will be covered by the national Terms and Conditions, Consultants (England) 2003. EducationThe Wirral University Teaching Hospital has extensive facilities for postgraduate education with an excellent and active Education Centre at Arrowe Park Hospital. In addition to fully equipped Lecture Theatre and Seminar Rooms for teaching purposes, the Centres has a well-stocked Medical Libraries with full-time staff. There is access to a wide variety of databases including Medline and the Cochrane Library for literature searches, and UpTo Date.Each Wednesday lunchtime, there are medical clinical meetings with contributions from all medical specialities. Each Friday morning there are respiratory speciality meetings including case presentations and journal reviews, there is no expectation to attend and they are not compulsory but the successful applicant will be encouraged to attend, due to the Trust’s commitment to junior doctor teaching. Second, third, fourth and fifth year medical students from Liverpool University undertake part of their training at the Wirral University Teaching Hospital each year and there is an active Undergraduate Education Centre in the hospital with seminar rooms, and IT suite and library facilities. The successful applicant will be expected, whilst undertaking clinical duties, to take an active role in the teaching of postgraduate medical staff and in undergraduate teaching where applicable. He/she should also expect to be accompanied by a doctor in training for at least two programmed activities. Consultants will also contribute to the direct teaching of trainees, in collaboration with colleagues. Funding is available for appropriate study leave and Continuing Professional Development. Continuing Professional Development (CPD)There are monthly CPD days (alternating half and full day) which are set aside throughout the hospital. These include clinical audit and clinical governance sessions and are often held jointly with other specialties. All consultants at Wirral University Teaching Hospital undergo an annual appraisal with a trained appraiser and the Trust is completely committed to annual appraisal and revalidation’Annual AppraisalThe Consultant Appraisal System at WUTH has been established in accordance with Department of Health’s National Appraisal Scheme for Consultant Medical Staff. All appraisers have been trained in appraisal techniques. Annual appraisals include monitoring of attendance at mandatory training and CPD . The appraisal reports are submitted to the Divisional Medical Director, Chief Executive and Medical Director. In case of any issues which cannot be reconciled by the DMD these will be referred to the Medical Director.Annual and Study LeaveThe annual leave entitlement is six weeks and two days (32 days) per year pro rataplus public holidays (or days in lieu) observed by the Trust. All periods of leave are granted subject to the exigencies of the service. Applications are made on standard forms and will need to be approved by the clinical lead.The study leave entitlement is 30 days in any period of three years in relationto leave with pay and expenses in the United Kingdom.
