Job DescriptionPosition: Veteran Services Case Manager Reports to: Veteran Service Lead Case ManagerSalary Range: $36,000 to $38,000 Classification: Exempt, professionalPurpose:To provide support and case management services to Passage Home Veteran clients and their families with a goal of self-sufficiency.Summary: The Veteran’s Services Case Manager will provide and assist with holistic and comprehensive case management services with all clients including intake assessments, benefit assessment, goal setting, long-term case development, weekly education, advocacy and referrals both within the Passage Home services system and among mainstream and public service providers. Case Manager duties include but are not limited to:Maintain Case load of 20-30 families/individuals, helping clients accomplish their goals and work towards self-sufficiency; maintain regular face-to-face contact with enrolled veterans. Conduct interview/assessments all pre-screened veterans/families to determine readiness for services.Review program documents with families; thoroughly explain program expectations and all related rules and requirements; ensure the veteran’s understanding and willingness to comply.Evaluate Veteran’s strengths and needs, determine immediate and long-term needs, and identify key barriers or assets in client housing stability.Assist Veteran to develop an individualized Housing Stabilization Plan, addressing housing sustainability, increasing household income, obtaining benefits, and increasing community supports. Collaborate with intra- and inter-agency staff to coordinate a program of life skill services through group workshops, individualized programs and support groups as needed to assist the Veteran with self-sufficiency.Provide education to Veteran(s) in areas of tenant rights and responsibilities and other topics in groups and individual settings.Conduct inspections on Veteran(s) housing to assure the home is safe and habitable. Collaborate with Veteran Administration Liaison and other VA Staff. Engage Veteran in ongoing assessment process, reviewing and amending Housing Stabilization Plan and level of care as necessary.Monitor family progress through phone calls, site visits and case management appointments. Act as liaison between landlords, court/criminal justice system and treatment services; maintain ongoing contact with service providers. Maintain case documentation in prompt and timely manner, including progress notes, database entries, and all other relevant information to assist clients and to comply with program requirements; Prepare discharge plans, time logs and statistics. Maintain professional knowledge in applicable areas and keep abreast of changes in job-related rules, statutes, and laws; make recommendations for the implementation of changes; read and interpret professional literature; attend training programs, workshops and seminars.Participate in regular staff meetings, staff training, supervisory sessions and accept responsibility for aiding the development of positive team relationships as requested.Adhere to agency policy, procedures, safety standards and professional code of ethics.Qualifications: Four-year college degree or comparable work/life/education experience. Experience working with homeless population in homeless prevention, rapid rehousing, and transitional supportive services (preferred). Must be proficient in using Microsoft Office tools including word, excel and power point.Demonstrated capacity to communicate professionally verbally and in writing. Ability to accurately document client records and files both on paper and in computer aided record keeping software.This position requires a flexible schedule. It is a professional salaried position and requires that case managers be available on call, over the weekend, and in the evenings as needed by clients and staff related support systems.
