National Resource Center for Nutrition and Aging Targeted Organizations: Nonprofits, state and local governments, tribal organizations, community-based and faith-based organizations Proposal Due: May 16, 2017 Granting Agency: Administration for Community Living ID/CFDA #: 93.048 Summary: To provide for training, technical assistance and support to the States; Units on Aging, Area Agencies on Aging and Nutrition Program providers in every State, Territory and the District of Columbia.

Promoting Integration of Primary and Behavioral Health Care Targeted Organizations: States Proposal Due: May 17, 2017 Granting Agency: Substance Abuse and Mental Health Administration ID/CFDA #: 93.243 Summary: To: (1) promote full integration and collaboration in clinical practice between primary and behavioral healthcare; (2) support the improvement of integrated care models for primary care and behavioral health care to improve the overall wellness and physical health status of adults with a serious mental illness (SMI) or children with a serious emotional disturbance (SED); and (3) promote and offer integrated care services related to screening, diagnosis, prevention, and treatment of mental and substance use disorders, and co-occurring physical health conditions and chronic diseases.

Targeted Capacity Expansion-HIV Program: Substance Use Disorder Treatment for Racial/Ethnic Minority Populations at High Risk for HIV/AIDS (Short Title: TCE-HIV: High Risk Populations) Targeted Organizations: Nonprofits, state and local governments, tribal organizations, community-based and faith-based organizations Proposal Due: May 3, 2017 Granting Agency: Substance Abuse and Mental Health Administration ID/CFDA #: 93.243 Summary: To increase engagement in care for racial and ethnic minority individuals with substance use disorders (SUD) and/or co-occurring substance use and mental disorders (COD) who are at risk for HIV or HIV positive that receive HIV services/treatment. The program also aims to contribute to the nation's achievement of the 90-90-90 goals regarding HIV status and treatment.

Community Economic Development Healthy Food Financing Initiative Projects Targeted Organizations: Nonprofits Proposal Due: April 28, 2017 Granting Agency: Administration for Children and Families ID/CFDA #: 93.570 Summary: To fund projects that implement strategies to increase healthy food access, foster self-sufficiency for individuals and families with low-income, and create sustained employment opportunities in communities with low-incomes.
