Requirements For Design/Build Services

Project 626A4-12-301

GI Clinic Pressurization & Monitoring

1. GENERAL SCOPE OF WORK & BASIC PROJECT OBJECTIVES:

Provide design and construction services for “GI Clinic Pressurization & Monitoring” at VA Tennessee Valley Healthcare System, York Campus (3400 Lebanon Pike, Murfreesboro, TN 37129).

Furnish all labor, material, equipment and supervision necessary for a Design/Build project to accomplish project 626A4-12-301 for “GI Clinic Pressurization & Monitoring” Building 3, 1st floor at the VA Tennessee Valley Healthcare System, York Campus, 3400 Lebanon Pike, Murfreesboro, TN. Work at the York campus includes but not limited to a design/build project to install 2 negative pressure systems, install pressure monitor systems for 4 rooms, install a VAV box, install new gasketed ceiling grids in 4 rooms and provide utilities for a medical equipment install in the existing GI Clinic. The first negative pressure system will provide a flow of 100% exhaust to 3 rooms, (2) ea GI exam rooms and (1) Bronchoscopy Room. The second negative pressure system will provide a flow of 100% exhaust to 1 room, (1) Scope Cleaning Room. The systems will include new fans and HEPA filters, placed in the attic on concrete pads, two floors higher than the GI clinic, where the exhaust will be ducted out the building’s dormers. This project will provide and install all ductwork, dampers and grills required to provide new exhaust and demo and cap existing exhaust in the affected rooms. All ductwork, fans, dampers and other system equipment must be corrosion resistant for the scope cleaning room. Controls for the fans will be fully compatible with the existing Siemens control system on the campus and provide but not limited to two features of functionality, on/off and current readings. The three rooms placed on the new exhaust systems will have pressurization monitors installed that will be fully compatible with the existing Siemens control system on the campus. The three pressurization monitors installed for the three exam rooms associated with the first exhaust system shall have an override to bypass the alarm when the space is not functioning in its primary mode. An office located in the GI clinic will have a VAV box installed to control air flow and temperature of the space. The VAV box will be controlled with a thermostat located in the office. The ceilings of the four rooms related with the new exhaust systems will be replaced with a 2x2 Unitee CR ceiling system or approved equal with hold down clips and gasketed T-Bar suspension system will also be required to meet VA and Industry standards as will new Light fixtures in the affected rooms. Also included, all electrical devices will be gasketed as will all perimeter ceilings, floors and all additional penetrations. Automatic door bottoms will be required on all doors for negative pressure rooms. Finally the project will also install utility requirements to support the install of a VA supplied Medivators Advantage Plus Automated Endoscope Reprocessor in the Scope Cleaning Room. The utilities required are domestic water (mixed at 95oF, 1.32 GPM, pressure 35 to 87 PSI), drain (to handle 5 GPM), compressed air (87 to 125 PSI, 1.5 CFM), and electrical service (120 VAC, single phase, 15A, GFI protected).

A certified test and balance report will be furnished upon completion of the project. All design criteria must meet or exceed values stipulated in the VA HVAC design manual. All work will need to take place after clinic hours and on weekends.

Contractor shall develop a design with a VA design team to insure all needs and project objectives are met. The design consultant shall provide necessary architectural and engineering services required to prepare a complete design (stamped contract drawings, specifications, technical reports, cost estimates, inspections, quality assurance, commissioning, etc.) and construction period services to insure the successful completion of the project. The contractor shall provide the VA with certifications and standard VA warranties. During Design Development (DD), the contractors responsible for independently analyzing conditions, needs, systems, etc., and developing unique, optimal proposals to achieve project objectives. Design and submittals shall be completed within 45 days. All construction work shall be complete within 90 calendar days, including final inspection, punch list corrections, commissioning, and closeout / release of claims. The contractor is responsible for independently analyzing systems/conditions and developing unique, optimal proposals to achieve project objectives.

2. ADDITIONAL PROJECT OBJECTIVES

It is the intent of the Department of Veterans Affairs to have the contractor fully survey, investigate, and document the existing GI Clinic in order to gain the knowledge and insight necessary to prepare quality, accurate documents, and perform the construction in a professional and competent manner.

• The design & construction shall minimize disruptions to medical center operations. Project shall be phased to maintain service and accessibility to the Building(s) during construction. Suitable space/access for equipment maintenance & service must be maintained throughout construction. The contractor will provide temporary measures to provide safety at all building entrances and exits where construction is on-going.

• Schedule work to accommodate the seasons and minimize weather related disruptions.

• The contractor shall design and construct within funding limitations. Contractor shall coordinate cost control measures with COTR to design within funding limits. Cost control alternatives shall be explored in Design Development (DD’s). The contractor shall employ a system of measures, such as timely estimates, cost control meetings, effective product and material selection, efficient management of labor/trades, reduction of overall time/critical path, etc.

• The contractor shall coordinate with leading industry manufacturers/vendors to insure proper selection and installation of materials.

• Product selection shall be done as part of the design and should include and necessary test areas to determine product suitability (stone patch/restoration, gutter restoration, brick cleaning and sealing products/methodology, historical preservation, color matches, durability, etc.).

• Design and construction shall be performed in accordance with all manufacturers’ instructions, recommendations, literature, concept drawings, etc. Coordinate with manufacturers.

• Mobilize and complete all work around one York Building at a time to minimize disruptions.

• The design and construction shall meet VA needs, VA criteria, VA Master Specifications, VA Design Guidelines & comply with VA alerts, recommendations, bulletins, and mandates.

• The design and construction shall comply with all state, local, and federal criteria, guidelines, codes, standards, etc.

• Contractor shall clean spaces hospital clean prior to turning them back over to the VA. Areas shall be ready for occupancy and VA use when turned back ove

DESIGN:

A. All work shall be conducted with the guidance and oversight of the prime contractor.

B. STATEMENT OF A/E TASKS:

1) The Contractor shall furnish all services for a complete design development, construction documents, and construction period services (including final inspection and commissioning).

a. Necessary site survey work such as verifying existing conditions, verifying record drawings, and verifying and analyzing utility systems shall be accomplished in conjunction with preparation of Design Development.

b. The complete site survey work shall include a thorough investigation of existing conditions to accurately document and convey space dimensions for measured drawings, all utilities for affected areas, and utilities that traverse through affected areas, typical construction type and variations, and finishes. This information provides the necessary base drawings for the Schematic Development phase of Design Development.

c. Hazardous material surveys (asbestos, lead, etc.) shall identify approximate quantities, type, and location for complete abatement.

d. Utility system verification shall include, but not be limited to, the necessary testing, adjusting, and/or balancing of affected systems to ascertain the existing operating and performance conditions that will impact the design and construction process. Systems that are insufficient in capacity or are no longer supported by OEM parts shall be considered for expansion and/or replacement.

e. Contractor structural evaluation and design shall include any necessary structural support for equipment and penetrations (wall, floor, etc.).

2) The contractor shall provide cost estimation, project scheduling & phasing, and construction period services.

3) The contractor shall provide construction document preparation.

4) The contractor shall provide competent design and construction period services utilizing individuals who are adequately knowledgeable of the various design disciplines including but not limited to architectural and engineering of structural, mechanical, electrical, and special systems as required by the project. Changes in personnel from those who are proposed in negotiations and at kick-off meeting will not be allowed without VA approval. The services of an industrial hygienist familiar with asbestos abatement will be utilized in the event that asbestos is found. The registered individuals who will stamp the completed documents shall accomplish engineering design work.

C. BASIS FOR DESIGN:

1) Department of Veterans Affairs standards may be obtained from the Internet.

2) Compliance to these standards, National Fire Codes, Uniform Building Codes, and OSHA Standards will be reflected in the completed design and construction documents. Refer to VA Construction Standard CD-30. Conflicting criteria shall be brought to the attention of VA.

3) The A/E shall design within budget.

D. DETAILED DESIGN SUBMISSION REQUIREMENTS: Program Guide PG-18-15, establishes the guidelines for the A/E submissions. These guidelines indicate a minimum level for submission compliance, which may not be adequate for certain types of design. In some instances in which critical decisions need to be made, sufficient and relevant information has to be provided timely in order for the design to progress to meet certain milestone dates. The level of applicability of the program guide depends upon the scope of work and the exclusions, which are herein specified. Questions in regards to the applicability of specific requirements shall be resolved with the medical center staff prior to any submission deadline. Where “days” are stated, this refers to “calendar” not “work” days.

1) At each submission, all drawings and support material shall be dated and appropriately labeled above the title block.

2) Design Team Meetings: Contractor shall meet with design team consisting of key VA staff (engineering, key personnel, etc.) to conduct interviews to insure that design will meet VA needs. Design team meetings are suggested after each submission. The number of meetings may vary based on contractor experience and design progress.

3) VA submission review comments may be verbal, noted directly on review sets, or written list. The contractor shall be responsible for compiling & addressing review comments. Contractor shall produce & distribute meeting notes or minutes for VA review immediately after each submission review meeting.

4) For each submission, furnish 3 copies of all drawings, specifications, and supporting documents (calculations, cut sheets, tests, studies, etc.).

5) Final construction documents shall be reproduced as part of the contract. An initial 5 sets of the Construction Documents shall be delivered to the Contracting Officer.

6) Total time to accomplish design and submittals shall be within 45 calendar days from the issuance of the Notice to Proceed to reproduced Construction Documents. Each submission will require 3 workdays for VA review, depending upon the contractor's responsiveness to previous VA comments and contractor quality control. Contractor shall propose a submission schedule based upon their understanding of the design and their staffing workload. The schedule is negotiable so that the timeframes are mutually agreeable to the contractor and VA. Some construction work shall be complete within 90 calendar days.

D. CONSTRUCTION PERIOD SERVICE REQUIREMENTS:

1) Review of Submittals: contractor shall review and approve all material submittals, shop drawings, test reports, etc. Submittal and submittal review copies shall be provided to VA Contracting Officer upon request.

2) Review Analysis of Requests for Information, Change Orders, and Costs: Contractor shall provide prompt responses when required to review and provide analysis of construction, requests for information, change orders, etc.

3) Site Visits: Contractor shall conduct site visit during the construction. Only qualified individuals with the proper background and experience that are familiar with the project shall make site visits. Prior to each site visit, the contractor shall notify the Contractor and the VA Contracting Officer of the number and disciplines of contractor's representatives participating in the site visit. The Contractor shall invite VA Contracting Officer (and key VA representatives) to attend contractor site visits. The contractor shall observe the construction and advise the Contractor of any deviations or deficiencies, and recommend appropriate corrective actions. The contractor shall promptly provide the VA Contracting Officer with documentation of inspections, deficiencies, recommendations, and evaluation/approval of corrective actions.

4) Contractor shall provide final inspection and commissioning of the new equipment and all building systems. Provide a copy of the final inspection report to the Contracting Officer. In subsequent inspections, update of final inspection list (for the Contracting Officer) to indicate corrective measures (and dates); until all punch list items have been corrected. Signed and stamped commissioning documents shall be provided to the Contracting Officer.

5) "Record Drawings" Requirement: Contractor shall provide marked-up prints to the contractor showing actual construction and any changes which occurred during construction. Contractor shall revise the solicitation documents and provide a complete set of all drawings showing actual, completed construction and reflecting any changes incorporated in the work on reproducible, erasable sepia and on electronic media. Electronic media drawings shall be in PDF and AutoCAD 2010 (.DWG format) or Autodesk Architectural Building Systems. Compatibility means that data can be accessed directly without translation or processing of data files. Problems shall be corrected to satisfaction of VA. VA will not accept electronic drawings with numbered or coded layer names, corrupted information resulting from translation/conversion, non-uniform drawing layers, colors, line-types, etc., and other issues that prevent easy reading and manipulation of the drawing/entities in AutoCAD or AutoDesk 2010. Electronic media specifications shall be in the latest version of Microsoft Word in use at VA.

Guidelines and Standards:

A. Design and construction shall comply with current VA specifications, guidelines, design alerts, manuals, details, criteria, instructions, procedures, standards, etc. Refer to the VA Technical Information Library at . The VA Design Manuals ) are a good place to start. When series standards are not available or are incomplete for a specific function/space, contractor shall apply the standards/guidelines of another series for that specific function or a similar function.

B. Design shall be performed and submitted in accordance with VA Contractor Submission Instructions for Minor and NRM Construction Program. Refer to ().

C. Design and construct for accessibility. Refer to . This includes providing handrails in all corridors.

D. Comply with current applicable state, local, & federal building codes as well as codes from other recognized authoritative bodies; including but not limited to Associated Air Balance Council (AABC), American Concrete Institute (ACI), Acoustical and Insulating Materials Association (AIMA), American National Standards Institute (ANSI), American Society of Heating, Refrigeration, and Air-Conditioning Engineers (ASHRAE), American Society of Mechanical Engineers (ASME), American Society for Testing and Materials (ASTM), The Building Officials and Code Administrators International (BOCA), Gypsum Association (GA), International Building Code (IBC), NEC, NFPA, National Environmental Balancing Bureau (NEBB), National Standard Plumbing Code (NSPC), OSHA, Standard Building Code (SBC), Sheet Metal and Air Conditioning Contractors’ National Association (SMACNA), The Uniform Building Code (UBC), and Uniform Federal Accessibility Standard (UFAS), & American Disabilities Act (ADA).

E. Coordinate with manufacturers and comply with their instructions / recommendations.

F. Insure proper life safety, including (but not limited to) egress, proper fire/smoke compartmentalization, fire alarm, sprinkler system, etc.

G. Design shall comply with VA seismic requirements for the Nashville area.

H. Provide a structural evaluation of the existing structure if necessary.

5. General Requirements:

A. Contractor shall remove and properly dispose of debris and items being demolished.

B. Provide proper layout and support for work, including necessary structural design services and work. During construction, if layout or support is not detailed, noted, or specified, Contractor shall submit a viable layout plan or support detail to AE for approval.

C. Materials, installation methods, routing, and exact locations of new items shall be coordinated with the Contractor’s Superintendent and contractor designer prior to proceeding with work.

D. It is the Contractor’s responsibility to thoroughly investigate the site to verify conditions (both exposed and concealed) prior to pricing the job, during design, & throughout construction.

E. Contractor shall not rely on scaled dimensions and measurements. Contractor shall take actual measurements in the field (for material takeoffs, etc.).

F. Where items are being removed, remove all appurtenances (associated architectural items, utilities, wire, conduit, pipe, etc) related or connected to (in, on, attached, or associated with) items indicated to be removed or demolished.

G. Protect installed items, equipment, finishes, etc. Repair / replace / restore conditions that are damaged, marred, disturbed, etc., as a result of work. This includes (but is not limited to) patching holes in partitions after items are demolished, capping utilities after removing branches, etc.

H. Materials shall resist flame spread and smoke generation.

I. At the conclusion of design, contractor shall provide VA with copies of the final design (construction documents); 2 stamped copies on bond, & 1 electronic media compatible with ACAD 2010. At the conclusion of construction, the contractor shall provide as-builts in the same format and 1 stamped reproducible.

J. Contractor is responsible for coordinating between trades. Coordinate so work is performed in an orderly and uniform manner.

K. VA expects Contractor to match existing facility standard finishes and materials, unless otherwise approved by VA COTR. Variations shall be specified on the cover of the offer.

L. Contractor shall design around, coordinate, and install all VA provided, expendable equipment.

M. Contractor is responsible for designing around, coordinating, and installing all VA provided, non-expendable equipment. All building service equipment shall be provided by Contractor.

N. Coordinate with manufacturers and vendors (VA and contractor) and comply with their instructions / recommendations, insure appropriate utilities and site preparation, etc. It is the Contractor’s responsibility to coordinate with vendors (even VA vendors), incorporate vendor installations into their project schedule to meet construction completion, move, and occupancy dates for the successful/timely completion of the entire project as a whole.

O. Contractor will remove and temporarily store existing equipment and furniture while work is being performed. Contractor is responsible for all temporary measures to enable work, including but not limited to coordination, moves, storage, disassembly / reassembly of modular casework and furniture, detour and construction signs, temporary barriers, negative air, infection control, etc.

P. Design and construction shall minimize disruptions to medical center operations. Project shall be phased to maintain utility service to the medical center during construction. Suitable space/access for mechanical room equipment maintenance must be maintained throughout construction.

Typical Work Conditions / Constraints:

A. Contractor must request (in writing) access to a work area 2 weeks before the date they wish to commence work. If conditions prevent VA from turning over a requested work area, another area will be made available.

B. Work shall be performed so as to minimize the disruptions to medical center operations / functions.

C. A large portion of the work will have to be performed after hours and on weekends or phases to allow the GI Clinic to remain in operation. Unless noted otherwise on the drawings or otherwise coordinated, the contractor shall assume that work will be performed from 5:00pm to 6:00am and on weekends (5:00pm Friday through 6:00am Monday) and federal holidays. VA may grant the contractor permission to work outside these normal project hours (as requested on a case by case basis).

D. The contractor is responsible for dividing the work into areas that can be returned to service after a single after hour shift or weekend shift. Work areas must be divided into manageable sizes that can be actively worked to completion. Once construction starts on an area, there shall be no laps in work or days/shifts taken off until the area is complete. Sections/segments of a work area (including corridors) shall not appear to be setting inactive; there shall be substantial progress made each day/shift. Coordinate areas of work with COTR. Contractor shall clean / polish the space and return it to normal VA operation before the next workday (before 6am Monday through Friday).

E. Contractor shall conform to the VA Medical Center’s Infection Control Policy (see General Requirements Section 01010). Contractor shall cover/bag supply diffusers, returns, and exhaust grilles. Contractor shall provide negative air machine(s) to insure each partitioned work space is negative. Contractor shall take the HEPA filtered negative air to the outside using flex duct. If the contractor cannot acquire outside access, or the flex run is greater than 75 feet, the contractor may flex the clean air to a general (non-medical treatment/suite) corridor.

F. Contractor is responsible for all temporary measures & access (penetrations, patch back, window removal, rigid/sealed blank-offs, temporary barriers, etc.)

G. The interior of the medical center shall not be left open or exposed to outside / exterior weather, elements, bugs, etc.

H. Contractor is responsible for providing and maintaining all temporary, dust proof, partitions to segregate areas of work from surrounding areas, people, and VA property (furniture, supplies, equipment, etc.). Plastic / Poly (minimum 6 mil, black) will be permitted for short term work (not more than an evening or weekend). Hard, drywall & stud barriers will be expected for all other (longer duration) work.

I. The contractor must not close-off or block more than 2 to 4 feet of a corridor, leaving 4 to 6 feet clear for VA traffic, unless closure of the corridor can be coordinated with a suitable detour route that doesn’t jeopardize life safety, cause an unreasonable inconvenience to staff / visitor, cause an unreasonable disruption to medical center operations, or prevent VA employee or patient access.

J. As part of the work in each area, the contractor is responsible for clearing/moving VA items, so that the new finishes can be installed. This includes temporary lifting of items to install new work, dismantling / disassembly, moving, storage, etc. Contractor shall restore each space to its original arrangement as part of the work. VA items includes, but is not limited to, items such as furniture, tables, bookcases, shelves, chairs, filing cabinets, modular / systems furniture, refrigerators, ice machines, microwaves, other non-medical equipment, etc. VA will empty and box the contents of lateral files, bookcases, and open shelves. VA will remove small desktop items, loose paperwork, personal items, phones, and computers/monitors/key boards.

K. VA will allow the contractor to setup an exterior staging area. The contractor can place weather-tight, prefab, metal storage container(s) for the storage of supplies and VA items. Contractor storage for VA property must be climate controlled. If any contractor-owned items are outside the containers, the contractor must erect a minimum 6 foot (non-see-through) fence.

L. Contractor is responsible for all cleaning and housekeeping. Each area shall be hospital clean before it is turned back over to the VA. Sheet vinyl and VCT must be cleaned and waxed (VCT) / polished (sheet vinyl) according to the manufacturer’s recommendations and to match adjacent similar VA areas. Carpet shall be vacuumed. No dust, streaks, marks, or residue shall be left on any surfaces. Final dusting shall be done using a HEPA vacuum.

M. Prior to starting working on a new building or floor area, the contractor shall perform a space final inspection with the COTR and correct all punch list items.

N. The TVHS campuses are full service medical centers offering a variety of inpatient, outpatient, and psychiatric care. Contractor shall plan and execute the work based on this occupancy.

O. Contractor is completely responsible for insuring the safety of the visitors, staff, and patients (including psychiatry patients) during this project. Contractor shall take all necessary steps and provide all temporary measure to eliminate hazards and control risks, including but not limited to barriers, blockades, fences, tape, signs, ramps, railings, covers, etc.

1) Temporary measures shall be coordinated with COTR and in place prior to demo/construction.

2) Contractor shall not leave tools/equipment unattended.

3) In unrestricted areas (i.e. locations accessible to patients, visitors, and non-engineering staff), contractor shall limit site supplies to manageable quantities of working materials that are actively being installed.

4) Contractor is responsible for inspecting and maintaining proper safety measures and temporary provisions.

5) Temporary measures shall prevent patient/staff/visitor access to construction areas, especially when construction sites are unattended.

6) Do not significantly inhibit visitor/staff/patient traffic. Contractor shall maintain easily accessible routes to medical center services. Minimize affects of construction on walkways, pathways, routes, etc.

6. PROJECT CLOSEOUT

6.1 Preparation for Final Inspection and Tests: The following steps shall be taken to assure the project is in a condition to receive inspections and tests.

6.1.1 Record Drawings: Finalize Record Drawings/Manuals, indicating all "As-Built" conditions.

6.1.2 Start-up: Complete start-up testing of systems and equipment/components as recommended by manufacturers and presented within the QCP.

6.2 As-Built Drawings: The Contractor shall provide "As-Built Drawings" based upon actual site installation, as should be reflected within latest Record Drawings/documents within 20 days of substantial completion. Should Record Drawings not represent as-built conditions, the Contractor shall perform field measurements and/or shop drawing takeoffs as required to support accurate As-Built Drawings. Should the Government determine that variations exist between finished construction and the As-Built Drawings; the Contractor shall correct drawings to the satisfaction of the Government within 14 days of Government notification. As-Built documents shall be in AutoCAD or AutoDesk 2010 or version that will open 2010 documents.
