Waiver of Responsibility and Liability for Prescriptions Filled by Online Pharmacies and Catalog Vendors

Did you know that it is illegal for online pharmacies to sell medication? So how do they obtain these medications…we simply don’t know! They are obtained from illegal sources.

By signing this form, I understand that I will no longer be eligible for the clinic rewards program.

I herby request a prescription for medication(s) for my pet so that I can purchase this/these product(s) from an internet pharmacy or catalog vendor. I have been informed of the following risks when I obtain these products from such sources.

What is wrong with using just any pharmacy website to buy cheaper medicine?

This means that if you choose the first pharmacy website that pops up in a search, you will most likely encounter a company that could send you counterfeit medications or steal your personal information. Additionally, 89% of these rogue sites do not require a valid prescription. That’s dangerous, because you could be seriously harmed if you’re not taking the proper medicine for your specific health condition or being monitored by a health care professional.

Can I get cheap prices from pharmacies outside the US?

o You might, but the real cost could be your pets health. Most sites (63%) have no company headquarters listed, which means they don’t want you to know that they likely operate out of a country with substandard drug safety criteria. “Substandard criteria” means that the drugs usually aren’t manufactured with the same safety standards as FDA-inspected facilities, or that the drugs are stored and shipped in conditions that could render them ineffective and dangerous. Also, don’t be fooled by websites that claim to be Canadian. In our review such sites, 74% sourced drugs from outside of Canada.

What are the signs of a suspiciously operating pharmacy?

o Learning the signs that point to suspect pharmacies can help you protect yourself from rogue internet sites. Sites with any of the characteristics listed below may be selling prescription drugs that are counterfeit, contaminated, or otherwise unsafe.

▪ They’ll give you a prescription based solely upon an online questionnaire.

▪ The site doesn’t list a phone number or street address or only allows you to communicate with them via email.

▪ They offer only a limited number of medicines or only a specific type of medicine, such as controlled substances or weight-loss drugs.

▪ They send you spam that could infect your computer with viruses or hack personal information.

• The prescriptions you obtain from these vendors are possibly harmful and dangerous. These products are not regulated and can be falsified by illegal sources.

• There is the possibility that the prescription drugs received from these vendors may be counterfeit and may not have been approved by the FDA; and the pesticides may not have been approved. This has occurred in the past and could recur.

• The manufacturer’s warranties or guarantees for these products may not be valid. This means if your pet’s condition is not effectively treated with the product(s), manufacturers may not stand behind their products or product liability procedures. Additionally, the owners of and doctors at this facility will be unable to assist you in claims against those manufacturers.

• When you purchase your prescriptions elsewhere, we cannot provide instructions for use or risks of adverse effects or send email reminders for follow-up exams and/or lab tests that may be needed to monitor results or adverse reactions.

• You may not have the prescription drug(s) available soon enough to start your pet’s medications at the optimal time to provide relief for his/her condition.

• When prescription products are purchased elsewhere, our staff members are unavailable to teach you how to administer them.

• The number of tablets or capsules, milligram size of the unit, volume and/or concentration of liquid, and number of authorized refills may differ from that prescribed by the attending doctor.

• Manufacturer rebates that would ordinarily be available for products purchased from this facility generally will not be available.

I have read and understand the above information. I accept the risks and I am aware that this facility cannot accept any financial responsibility for paying for or reimbursing me for any treatments required as a result of the use of products purchased from these sources. I also understand that I will no longer be enrolled in the clinic rewards program. In the absence of negligence, I agree to hold this veterinary practice harmless for any deleterious/harmful effects or lack of effectiveness of drugs/products purchased from any other source.
