JOB DESCRIPTION

Job Title: Service Coordinator

I. Job Summary

Under the supervision of the Program Manager, determines program eligibility, maintains client caseload, monitors client’s progress, coordinates referral information, and provides client advocacy.

II. Key Responsibilities

← Intake and assessment: Obtains information from family/collaterals and uses appropriate tools to make assessment, prepares summary and makes recommendations regarding linkage to direct service providers, and communicates client information to team.

← Public relations: Interacts with other agencies, professionals, and referral sources and presents a positive image of him/herself, the division, and the agency.

← Communication: Effectively uses written, verbal, and non-verbal communication to communicate ideas and case management to other employees and clients.

← Continuous quality improvement: Identifies opportunities for program improvement, uses appropriate improvement tools and techniques to address potential quality issues within the division, and participates in quality improvement initiatives/activities.

← Service planning: Works with team to develop individual service plans and provides case management for referral and life-care services.

← Advocacy: Acts as an advocate on behalf of the client, attend required staffings and home visits.

← Record keeping: Maintain client files per DHS guidelines; submit billing on an average of 120 hours/month to client activity and an average of 22 hours/client per fiscal year.

← Documentation and writing: Uses clinically appropriate professional terminology to complete forms and records, accurately and timely documenting client assessment, planning, and progress.

← Ethics and boundaries: Develops and maintains appropriate professional boundaries between self, staff, clients, and family/collaterals and identifies issues related to ethics and boundaries.

← Time management: Identifies tasks to be completed in order of highest to lowest priority and identifies behaviors that will not interfere with the ability to complete assignments.

← Team orientation and cooperative working relationships: Promotes teamwork through collective effort and works cooperatively with team members. Participates in staff meetings and in the making of recommendations regarding evaluations and progress for specific clients.

← Professional development: Maintains appropriate credentials and pursues internal and/or external training and development opportunities.

← Performs related duties as required.

Qualifications

← Bachelor of Arts/Science Degree from an accredited college in a human services field, including, but not limited to sociology, special education, rehabilitation counseling, or psychology, a minimum of one year experience working directly with individuals having developmental disabilities, and credentialed as a Qualified Intellectual Disabilities Professional (QIDP).

← Minimum BA/BS in human service or related field of study, including but not limited to psychology, sociology, education, child development, social work, etc.

← Strong communication and interpersonal skills including listening, problem solving, negotiating, writing, verbalizing, etc.

← Commitment to enriching the lives of clients and families.

← Ability to travel as appropriate.

← Must have a current Illinois driver’s license in good standing and current auto insurance which meets the standards as set forth by PACT policy.
