This post will be ideally suited to a clinician from a therapy (Physiotherapy?Occupational Therapy or Speech Therapy) Clinical Psychology or Nursing background.The post holder will provide clinical leadership and will be responsible for the clinical activity delivered in Headway Place for adults recovering from traumatic or acquired brain injury.Your key tasks are summarised here:Acts as a professional and clinical role model to all staff, client’s/services users, carers and the public at all times. Use advanced skills to ensure the delivery of a clinically effective service which promotes optimum health, self-management of individuals, families and communities in line with Headway Basingstoke development plans, clinical pathways, policies and procedures. Use advanced clinical skills and expert knowledge to clinically lead and enable others to deliver best practice across the service whilst leading and monitoring the development of services, new ways of working and the implementation of change. Communicate highly complex clinical information about client’s/services users’ conditions to multi-disciplinary team colleagues, to ensure effective delivery of treatments. Lead on the assessment of physical, mental health and social needs of patients and authorise therapeutic rehabilitation care plans and treatments based on the best available evidence in order to improve health outcomes. Develop and monitor comprehensive clinical assessments of client’s/services users’ needs. Plan, implement and evaluate rehabilitation care delivery according to changing health care needs. Ensure patient information is collated, monitored and fully accurate, maintaining complete and contemporaneous records in line with GDPR and organisational standards.Ensure staff are trained appropriately to competently deliver the rehabilitation care required and ensure supervision and mentorship is in place. Foster a culture of networking in an open and transparent approach with internal and external MDT’s and to work collaboratively with stakeholders to ensure timely and appropriate response to referrals.Responsible to the Service Manager in monitoring the delivery of performance outcomes and targets ensuring that NHS Standard Contract requirements are adhered to. Work directly with Clinical Commissioning groups outside our NHS contract area to provide appropriate clinical evidence and programmes to secure individual funding requests for patients.
