Provides primary triage response to all referrals made to the Neurosciences Unit Adult/Paediatric General Diagnostic Programs. Identifies the nature and appropriateness of referrals and sets priorities for management. Collates information for presentation to Neurosciences Adult & Paediatric General Diagnostic Program teams. Conducts risks and initial assessments and brief intervention as required.

Brief Summary of Duties (in order of importance)

1. Clinical (70%) 1.1 Responsible for all referrals, inquiries and requests for advice and information, including

giving advice on queries regarding medication, effects and side effects and screening of referrals to determine suitability for service, seeking further information regarding a referral as required. 1.2 Responsible for the coordination and gathering of collateral information of all referrals for new and known clients' current management plan including medication regimes, effectiveness of same and assessing the urgency of response required. 1.3 In consultation with multidisciplinary team acts as assisting decision maker for urgent referrals, contributes to complex mental state examinations, risk assessments and develops action plans. 1.4 Acts as an information resource for clients / families / carers / community agencies seeking information and support regarding neuropsychiatric conditions in adults/children including giving advice and relevant information regarding medication, effects and side effects. 1.5 Acts as a consultant to the Neurosciences adult and paediatric teams and other health professionals regarding assessment of risk and strategies for crisis / brief intervention. 1.6 Ensures information systems e.g. PSOLIS are used effectively and efficiently. 1.7 Ensures an individualised treatment plan is formulated for each client under care, maintaining appropriate records. 1.8 Drives and participates in ongoing quality improvement activities. 1.9 Ensures nursing practice is upheld to ensure adherence to the Mental Health Act and other relevant legislation. 1.10 Actively participates in all relevant consumer and service directed meetings.

2. Management and Leadership (5%) 2.1 Demonstrates effective leadership and decision making qualities, supports change and acts

as a role model for other staff within the designated setting. 2.2 Formulates, updates, reviews, implements policies, processes and other documentation

related to Nursing Practice. 2.3 Conducts activities in compliance with the Public Sector Management Act, Public Sector

Code of Ethics, NMHS Code of Conduct and health service policies and procedures. 2.4 Ensures principles and practices of relevant legislation, including EEO, Disability Services,

OS&H are maintained. Participates in Risk Management and Clinical Governance activities. 2.5 Contributes to the effective functioning of the mental health service through the efficient use

of resources and active participation in relevant administration procedures. 2.6 Ensures appropriate risk management strategies are employed including the process of

identification assessment, investigation, controls and monitoring. 2.7 Participates in clinical research projects. 2.8 Enters data and provides statistical data and reports as required by service unit. 2.8 Assists with staff performance appraisals and supervision requirements in liaison with

Manager where appropriate.

3. Education (10%) 3.1 Prepares and participates in educational activities and programs for staff, students and

community agencies. 3.2 Participates in developmental training of all staff in nominated program/practice, including

mandatory skills training. 4. Professional (15%) 4.1 Makes decisions to the level of expertise and competence and accepts responsibility for

decisions. 4.2 Takes part in Clinical Supervision.

5. NMHS Governance, Safety and Quality Requirements 5.1 Participates in the maintenance of a safe work environment. 5.2 Participates in an annual performance development review where required. 5.3 Supports the delivery of safe patient care and the consumers' experience including

participation in continuous quality improvement activities in accordance with the requirements of the National Safety and Quality Health Service Standards and other recognised health standards. 5.4 Completes mandatory training (including safety and quality training) as relevant to role. 5.5 Performs duties in accordance with Government, WA Health, North Metropolitan Health Service and Departmental / Program specific policies and procedures. 5.6 Abides by the WA Health Code of Conduct, Occupational Safety and Health legislation, the Disability Services Act and the Equal Opportunity Act.

Work Related Requirements

Essential Selection Criteria 1. Eligible for registration in the category of Registered Nurse by the Nursing and Midwifery

Board of Australia. 2. Demonstrated advanced knowledge and skills in assessment and crisis intervention for

children and adolescents &/or adult consumers with complex needs. 3. Knowledge of community resources relevant to adults &/or children and adolescents with

disabilities such as neurological/cognitive, neurodevelopmental disorders and related emotional and behavioural conditions. And ability to liaise effectively and maintain relevant networks. 4. Experience in prioritising and management of referrals based on clinical acuity and risk. 5. Interpersonal and leadership skills whilst working within a multidisciplinary team. 6. Demonstrated knowledge of the Mental Health Act 2014 and other relevant legislation.

Desirable Selection Criteria 1. Holds or working towards a relevant tertiary qualification or a related post registration

qualification. 2. Demonstrated commitment to the maintenance of a high standard of nursing care and the

principles of continuous quality improvement. 3. Computer literacy and relevant software skills. 4. Current knowledge and commitment to Equal Opportunity in all aspects of employment and
