SCHOFIELD RESIDENCE COVID VISITATION POLICY- *UPDATED*

*The COVID Visitation Policy has been updated to reflect the changes implemented by the Department of Health 09/15/2020*

PROCEDURE:

• All responsible parties will be notified of the revised visitation plan which must follow all guidance provided by the Department of Health.

• Visitation plans will need to be structured with regulations that must be adhered to.

• The structure for visits will allow for each hall to have a designated day for visitation:

Monday- 2nd Floor South Hall

Tuesday- 2nd Floor East Hall

Wednesday- 1st Floor South Hall

Thursday- 1st Floor East Hall

Friday- 1st Floor North Hall

• Visitation will be limited to 30 minute blocks of time

10:00am- 10:30am

11:00am- 11:30am

01:30pm- 02:00pm

04:00pm- 04:30pm

05:00pm- 05:30pm

• Reception will coordinate scheduling of all visits in adherence of the day and time

schedules outlined above. The receptionist can be reached at our main number

(716) 874-1566.

• Facility will not schedule more than 5 residents during the designated times.

• Only two visitors will be allowed per visit at any one time.

• Due to the changes in the weather the facility will determine if visits will be held outside on the patio or inside in our Main Dining Room.

• In the event any resident or employee tests positive for COVID-19 visitation will immediately be suspended until no new cases have been identified for 14 days.

• New admissions to the facility will not be able to schedule a visit until after the 14 day monitoring period is over.

• Residents who go out on medical appointments will be monitored for 14 days following their appointment and will not be able to schedule a visit until after the 14 day monitoring period is over.

Outside Visitation Process:

Visitor(s) (no more than 2 per visit) arrive at facility are greeted outside of the Main Door by designated staff member. At that time, the visitors will have their temperature taken, asked the required COVID questions, review the visiting process, and complete the sign in log (see attached) and present proof that they have had a negative COVID-19 test within the last seven (7) days. Once the visitor is cleared, they will be allowed to visit as the designated area that is 6 feet away from the resident to ensure proper social distancing. Both the resident and the visitor will be required to wear a mask. Unfortunately, close contact such as hand holding, hugging, kissing, or feeding are prohibited to ensure the spread of COVID is minimal. If at any time these rules are not followed the visitation will be ended. The visitors will be asked to be seated and remain so at the designated marked area. Once all visitors have arrived, residents will be brought out to the patio to begin the visit. Visits will only be 30 minutes, during this time a designated staff member will be outside on the patio monitoring the visitation.

The Visitation will resume at the facility once the guidelines are met:

• The NH has no staffing shortages as evidenced by the NH’s individual staffing plan and as reported by the NH through submissions to the National Health Safety Network (NHSN).

• The absence of any new onset of COVID-19 among staff or residents as reported to the Department on the HERDS and staff testing surveys and as reported to the NHSN for a period of no less than fourteen (14) days

• Visitation should be limited to outdoor areas, weather permitting. Visitor and residents must be appropriately socially distanced and wearing a facemask or face covering while in the presence of others.

Limited visitation, including, but not limited to, family members, loved ones, representatives from the long-term care ombudsman program (LTCOP), and resident advocacy organizations, will be permitted under the following conditions:

Visitors are screened for signs and symptoms of COVID-19 prior to resident access and visitation will be refused if the individual(s) exhibits any COVID-19 symptoms, do not pass the screening questions, or have a negative COVID-19 test within the last seven (7) days. Screening shall consist of both temperature checks and asking screening questions to assess potential exposure to COVID-19 which shall include questions regarding international travel or travel to other states designated under the Commissioner’s travel advisory. The facility must maintain screening questions asked onsite and make it available upon the Department’s request.

Documentation of screening must be maintained onsite in an electronic format and available upon the Department’s request for purposes of inspection and potential contact tracing. Documentation must include the following for each visitor to the nursing home:

• First and last name of the visitor;

• Physical (street) address of the visitor;

• Daytime and evening telephone number;

• Date and time of visit;

• Email address, if available; and

• A notation indicating the individual cleared the screening (both temperature and questions) that does not include any individual temperatures or other individual specific information.

There is adequate PPE made available by the nursing home to ensure residents wear a facemask or face covering which covers both the nose and mouth during visitation, if medically tolerated.

Visitors must wear a face mask or face covering which always covers both the nose and mouth when on the premises.

Visitors must maintain social distancing, except when assisting with wheelchair mobility. The nursing home must have adequate supply of masks on hand for visitors and must make them available to visitors who lack an acceptable face covering.

Facilities provide alcohol-based hand rub, consisting of at least 60 percent (60%) alcohol, to residents, visitors, and representatives of the long-term care ombudsman visiting residents and those individuals are able demonstrate to appropriate use.

No more than 10 percent (10%) of the residents shall have visitors at any one time and only two visitors will be allowed per resident at any one time.

Visitors under the age of 18 years old are prohibited from visiting residents at this time

Current COVID-19 positive residents, residents with COVID-19 signs or symptoms, and residents in a 14-day quarantine or observation period are not eligible for visits.
