SNAP is seeking a qualified individual for a Community Health Worker, to work in SNAP’s Community Action Core. This position is half-time, regular and open to all qualified individuals. Job Description is attached.

JOB DESCRIPTION

Position Title: Community Health Worker

Core Area: Community Action

Department: Spokane Ride to Care

Reports to: Transportation Coordinator

FLSA Status: Non-Exempt

Grade: 7

EEO Class: 1.2 (Mid-Management)

Time Classification: Half Time

Position Summary: This Community Health Worker position will be focused on providing culturally appropriate health education and information, conducting outreach, building relationships with referral sources, coordinating care and helping clients navigate the health and social determinants of health systems.

Principle Duties and Responsibilities:

1. Responsible for establishing trusting relationships with clients and their families while providing general support and encouragement.

2. Conduct social health determinants assessment to assure client and family members are living in safe environment, children are cared for and attending school, immunizations are up-to-date, family has access to healthy foods, transportation to appointments, etc.

3. Provide referrals for services to community agencies as appropriate. Help clients connect with transportation resources and give appointment reminders in special circumstances. Transporting clients is strictly prohibited.

4. Work closely with medical provider to help ensure that clients have comprehensive and coordinated care (including medication management, re-ordering prescriptions,)

5. Follow-up with clients from initial identification through closure. Work cooperatively with other clinical personnel assigned to the same client.

6. Provide ongoing follow-up, basic motivational interviewing and goal setting with clients/families. Conduct intake interviews with clients, including enrolling and/or referring clients into other community services programs.

7. Follow-up with clients via phone calls, home visits and visits to other settings where clients can be found.

8. Assist clients with completing applications and registration forms. Conduct eligibility determination, enrollment and follow-up with uninsured clients. Help clients set personal goals, and attend appointments.

9. Be knowledgeable about community resources appropriate to needs of clients/families.

10. Be responsible for providing consistent communication to the Care Management Coordinator to evaluate client/family status, ensuring that provided information, and reports clearly describe progress.

11. Act as a client advocate and liaison between the client/family and community service agencies (i.e. schools, Department Human Services, Heath Care for Homeless, hospitals, support groups, etc.).

12. Record client care management information in the agreed upon client database (training provided) and other software no later than 24 hours after client contact.

13. Manage assigned caseload of clients.

14. Transport clients to and from appointments timely. Operate wheelchair lift as needed.

15. Assist transportation clients in and out of vehicle and to and from the vehicle when required.

16. Attend regular staff meetings, trainings and other meetings as requested.

17. Work with coworkers and volunteers to promote a positive, respectful and productive working environment, while protecting client confidentiality and dignity.

18. Within the stated context of the stated principle job duties, serve as an ambassador of SNAP’s mission and values.

19. Completes assigned projects and performs related duties as needed or as situation dictates and looks for opportunities to keep busy.

20. Provides excellent customer service at all times as well as helps to grow the customer service culture within the agency.

Requirements:

Education and Experience

• Required: HS/GED and at least four years of experience of Community Health or related work. Must have clean driving record. Must be able to pass Department of Transportation drug testing. Must be age 21 or over.

• Preferred: Some combination of education and experience totaling six years including Community Health work and client transportation. CPR Certification preferred.

Knowledge, Skills, and Abilities

• Ability to empathize with and advocate for low-income households, and to maintain confidentiality with personal information

• Must possess strong professional boundaries

• Must have a strong sense of safety and adhere to safety protocols

• Strong communication, interpersonal and customer service skills with a diverse population

• Computer proficiency with MS Office required, with aptitude and ability to learn related technical skills

• Ability to follow directions and work effectively in an open, fast-paced, high-stress setting with frequent interruptions

• Good planning, organization and critical thinking skills, including ability to prioritize and communicate questions and concerns

• Ability to work independently, using initiative and ethical judgment to resolve problems and ensure workflow is appropriately addressed in a timely manner

Training and Certifications

Trained Community Health Worker within 90 days of hire

CPR/First Aid Certification within 60 days of hire

Valid Driver’s License Required

Work Environment/Physical Activities:

Work is conducted in vehicles and client homes. Work requires ability to walk clients to and from vehicle. Some lifting may be required. May deal with difficult people, sometimes in crisis situations.
