JOB DESCRIPTIONJob Title: Support Worker Responsible to: Manager Supported Accommodation Responsible for: A Broad range of support duties within supported Accommodation, tenancy support and some domicillary Services. Job Purpose: To provide assessment and develop support plans for service users. To co-ordinate and provide support to people living in supported accommodation and their own homes in line with the plans. To work with service users to achieve positive outcomes that will enable them to take greater control of their own lives.Core Duties and Responsibilities1. Assessment and review of Service Users needsTo participate in the assessment of referrals into the Supported Accommodation project/ Floating Support Scheme and work with applicants to identify the goals they wish to achieve in line with the objectives of the scheme To work in partnership with the Service User to plan ways in which these goals will be met and develop and regularly review the progress made in delivering the outcomes identified within the plans.To work with the service user alongside staff from Cardiff Mind, Volunteers and other agencies to enable the tenants to achieve the goals they have identified in their Individual Support Plans2. Access to informationTo ensure that the Service user is able to access information not only on services available in Cardiff and issues relating to the needs of people with mental health problems, but also to general information that will enable them to make informed decisions and fully participate as citizens.3. Service User InvolvementTo facilitate the development of the ability and potential of each service user to become involved in the development and review of their Support Plan. To encourage active service user participation in influencing services provided by Cardiff Mind and in the wider sphere of mental health service provision in Cardiff.4. Organisational IssuesTo work in accordance with the organisational policies and procedures.To actively participate in supervision, performance review and training provided by Cardiff Mind and identified as adding value to the service.5. Housing ManagementTo work in line with the requirements set by the Housing Associations to ensure a good standard of housing is provided to service users. 1. Assessment and review of Service Users’ needsTo carry out a full assessment of all persons referred to the scheme as directed by the Manager of the Service.To prepare individual support plans for persons joining the scheme.To carry out risk assessments of supported persons and the environment in which the support will be delivered as required.To plan and carry out reviews of all support plans in compliance with the requirements of the standards and timescales for the service.To be the key worker responsible for assessment and review for named individuals.2. Co-ordination and implementation of support plans.To co-ordinate work with the service user and other staff on the goals identified by the in support plans, including personally delivering those elements for which you are responsible or parts of that work as appropriate.To take on responsibility for areas of work detailed in support plans especially work which includes advocacy, benefit applications and liaison with other agencies.To record the work carried out with the service user to evidence progress and facilitate effective evaluation of the support plan.To provide appropriate emotional support to enable the tenant to develop new skills and achieve identified goals. To develop with the service user a range of options which will open up the opportunities available to increase their expectations and make an informed choice, including expanding social networks.To work as part of a team to offer consistency in the quality and approach of the support offered3. Provision of InformationTo ensure that Service Users are able to access accurate and quality information on issues relating to their general well-being. This will include being able to sign-post to other agencies with specialist expertise as well as having a broad personal knowledge base.To have information available to tenants about other services and facilities available in Cardiff.4. Service User InvolvementTo encourage service users to make their points of view known to other agencies and to Cardiff Mind as appropriate.To encourage service users to take part in monitoring or quality reviews of the project.To encourage the service users to take part in Cardiff Mind as an organisation, in the wider community, including the geographic community, the mental health community or any other group which the service user feels is relevant to them.5. Organisational IssuesTo carry out administrative duties as required to ensure the efficient running of the project and of Cardiff Mind, including the maintenance of activity data.To take an active part including preparation for supervision, performance review, team meetings and other organisational meetings. To develop own professional skills and work to current best practice.To take part in the development of policy and procedure and other organisational concernsTo challenge discrimination and work to Cardiff Minds equal opportunities policy and its values.To maintain good working relationships and a professional approach with external workers and agencies. To maintain the confidentiality of information and respect the rights of service users, staff and the organisation.To work to within the policies, guidelines and procedures of the organisation and to assist the organisation to achieve it's overall aims and objectives.To undertake such other duties as may be required to provide a quality service commensurate with expertise and experience.To actively participate in the on-call service as required ................
