Housing Community Support Specialist

Queen of Peace Center has an opportunity for a Full-Time Community Support Specialist. Queen of Peace Center is a family-centered behavioral healthcare provider for women with addiction, their children and families. The gender focused female recovery center provides a variety of treatment options to women with co-occurring disorders and those affected by trauma.

General Description: Community Support Specialist/Housing Coordinator responsibilities consist of specific activities with or on behalf of a particular client in accordance with an individual recovery plan to maximize the client’s adjustment and functioning within the community while achieving sobriety and sustaining recovery, maximizing the involvement of natural support systems, and promoting client independence and responsibility. The CSS will be responsible for the development and delivery of housing services and programs for Queen of Peace Center’s (QOPC) permanent supportive housing program, Shelter Plus Care. The CSS will also provide support and communication between Housing, Clinical, Programs and client services.

Specific Duties:

1. Participate in the interdisciplinary team meeting in order to identify strengths and needs related to development of the individual’s rehabilitation plan.

2. Attend periodic meetings with designated team members and the client, whenever feasible, in order to review and update the rehabilitation plan.

3. Contact clients who have unexcused absence from the program in order to re-engage the person and promote recovery efforts.

4. Arrange and refer for services and resources and, when necessary advocating obtaining the services and quality of services to which the person is entitled.

5. Monitor service delivery by providers external to the program and ensuring communication and coordination of services.

6. Locate and coordinate services and resources to resolve a crisis.

7. Provide experiential training in life skills and resource acquisition.

8. Provide information and education to an individual in accordance with the person’s recovery plan including relapse prevention and discharge/transition planning.

9. Provide Group and Individual Education

10. Assist client in establishing treatment goals.

11. Provide Care coordination between the Shelter Plus Care program and the Queen of Peace Center (QOPC) Clinical Team.

12. Provide additional programing, safety checks and family engagement for QOPC Housing Clients.

13. Collaborate with the Community Connections program to increase employment opportunities, income, and community engagement for QOPC Housing Clients.

14. Work with the Housing and Clinical team to develop Shelter Plus Care Housing orientation, implement compliance standards, re-engage and outreach when needed.

15. Transport clients in employee’s vehicle - Must have Class E Missouri driver’s license.

16. Other duties as assigned.

Qualifications: Bachelor’s degree in social work, counseling, psychology, or closely related field or four (4) years relevant experience in human service delivery field. Valid Missouri driver’s license, vehicle, and current automobile liability insurance are required. CSS must own or lease their own vehicle for use in transporting clients. (Automobile insurance coverage to include transporting clients in employee’s vehicle.) Good communication skills both oral and written; skills involving proper grammar, punctuation and clarity. Ability to use computer for charting and familiar with Microsoft Windows, Excel and Outlook Calendar. Ability to lift 20 pounds.
