This is a contract between K9 Country Club and the pet owner whose signature appears at the bottom of this document (hereinafter called Owner).

1. Owner shall pay the rate in advance for daycare and boarding that is in effect on the date the pet is checked in to K9 Country Club. If Owner has not picked up his/her dog by closing time, the pet will be boarded overnight and a boarding fee will be applied.

2. Owner certifies to the accuracy of all information given about the pet on the pet's permanent record.

3. In order to provide a safe and healthy environment for all pets under K9 Country Club's care, the following is required:

A. Vaccinations -- The following up-to-date immunizations are required unless Owner has a letter from a veterinarian stating otherwise. For puppies less than one year: Full series of DHLP/P and Corona (usually three), Bordetella, and Rabies. For dogs over one year: Vaccines boosted yearly. Bordetella boosted twice yearly.

B. Flea and tick protection -- All dogs must be wearing monthly topical flea/tick protection such as Frontline?, Revolution? or Advantage?. Frontline? is available at K9 Country Club for $17 per application.

C. Sociability -- Dogs enjoying Doggie Daycare who are over six months old must be spayed or neutered. Owner's dog will be checked for socialization skills on his/her first visit.

D. Parasite screenings -- All dogs coming to K9 Country Club are required to have a fecal examination to check for both giardia and other parasites common in our area. Fecal examinations are required yearly thereafter.

4. Owner recognizes that dogs can be unpredictable and difficult to control at times and therefore it is possible that Owner's dog may harm or be harmed by another dog and that this is an unavoidable risk when dogs participate in Doggie Daycare on K9 Country Club's premises. Owner accepts this risk of harm to self and to the dog and shall not seek to recover any damages against K9 Country Club, its owners or employees for any damage so suffered.

5. Owner also agrees to indemnify and hold harmless K9 Country Club for any and all damages to the premises, equipment, employees, other clients and other dogs, which is caused by Owner or Owner's dog. Owner further agrees to pay all costs and expenses incurred for such damage and injury.

6. K9 Country Club will do its best to assure that any dog under its care is reasonably compatible with other dogs and meets its standard of an acceptable participant in its Doggie Daycare program. K9 Country Club further agrees to exercise all due and reasonable care to prevent injury or illness to all pets under its care.

7. Owner grants permission to K9 Country Club to act in its behalf and its dog's best interest, in the case of injury or illness, by obtaining veterinary and other necessary care for Owner's pet while under K9 Country Club's care. If Owner's pet becomes ill or injured, K9 Country Club will try to contact Owner immediately. If that is not possible, and K9 Country Club deems it necessary, K9 Country Club will seek immediate medical attention for Owner's pet at Owner's sole expense. By signing below, Owner authorizes K9 Country Club and its representatives to act "as owner" at Owner's sole expense, in the case of a medical emergency. Owner agrees to reimburse K9 Country Club for all costs and expenses incurred for such services.

8. Owner understands that no vaccine is 100% effective and that there may be various strains of viruses that could still cause disease in pets, despite vaccination. Owner accepts this risk and holds K9 Country Club harmless in the event of illness of his/her pet.

9. Owner will inform K9 Country Club if his/her pet can climb fences or digs excessively. Owner agrees to hold K9 Country Club harmless in the event of the escape of his/her dog.

10. As there will be "dog safe" toys available for play in the Doggie Daycare areas, plush pet beds in the kennels and pea gravel used as a footing substrate in various areas, Owner agrees to hold K9 Country Club harmless in the event of ingestion and impaction of such items.

11. Owner will inform K9 Country Club if his/her pet destroys, chews, or ingests bedding. Plush pet beds will be used in dog's individual kennels unless Owner informs K9 Country Club of the chewing behavior. Owner will be charged for damaged bedding in the event of destruction of the bed.

12. It is understood by all parties that this agreement is intended to be as broad as possible under Washington state law.

13. Should there be a dispute or litigation over this document, or any other documents signed with K9 Country Club, K9 Country Club shall be entitled to reasonable attorney's fees and costs. Any litigation will take place in Yakima County in the state of Washington.
