The updates and guidance come to us daily and changes are being made continually to maintain the wellbeing and safety of our patients and staff alike. We have been overwhelmed by the understanding and thanks we have received from our patients.Adjustments have been made to almost every aspect of our lives and the vast majority of our patients have been particularly flexible and understanding. Thank you!You may have noticed that our doors have been closed! We are, of course, still open and we remain committed to providing excellent care to our patients, which includes making face to face appointments when necessary. We have encouraged as many patients as possible to use our online services to make prescription requests, as well as supporting those patients who find this slightly more challenging. We have moved almost all our appointments over to telephone and video consultations (at times a challenge for clinicians and patients’ alike).Our Frailty Team have set up routine telephone calls to our care homes to help support the staff wherever we can, including visiting patients who continue to need clinical care.A team of staff have been calling our vulnerable patients or those that just need a little extra support and reassurance. We have managed to maintain a sufficient level of PPE (so far) for our clinical staff, to ensure safety at all times. We’ve also been lucky enough to have scrubs made for us. Our Care Navigators have continued to answer your calls and endeavoured to direct you towards the most appropriate care and services. This is new, sometimes daunting for all of us. We have laughed and cried and eaten lots of cakes, supported each other and talked things over A LOT. This could include treatment for the following:Minor aches and painsMinor sprains, sports injuries, grazed knees and scars Coughs, colds, blocked noses, fevers and sore throatsBelow are some other top tips to take care of yourself:Keep a well-stocked medicine cabinet at home. Most commonly this is done by a link worker, that’s where I come in.A referral is generally made via the surgery’s GP’s, nurses, clinicians and staff but you can also self-refer, we then meet, (that’s the best part) have a conversation sometimes this can last up to 45 minutes and we work out a plan of action, building on the assets you already have but sometimes you don’t know it. I might connect you to a community group, a new activity, a local club or 1-1 counselling. Or perhaps it’s more practical support that’s needed, a referral into local resources such as peer support groups, benefit advice, debt support, housing advice or you just need a form to be filled out. Information and guidance will always be offered and if I don’t know it, I will try to find it out for you.As a Social Prescriber, my hope is to enable you to have more control over your own health and find ways to improve how you feel about yourself and in a way that suits you. So a little bit about me….I’m a talker but also a listener, I’m a half full kinda girl, positive, outspoken but with a fun and curious outlook on life. I’m a mum of three, also a new nannie to a beautiful granddaughter. Love to read, especially psychological thrillers, enjoy a good bike ride, traveling, a good G&T, passionate about fashion and enjoy a hot curry and any kind of crumble, but not at the same time! I truly believe my previous job roles (various retail jobs, Graphic Designer, Integrative Mental Health Counsellor, Outreach officer and a Mental Health Employment Navigator) have been a pathway to my current position.
