Geographical Background and General Hospital Facilities

Stockport constitutes one of ten boroughs in Greater Manchester and its proximity to the Manchester Medical School has enabled the development of close working relationships with various University Departments. The general environment in Stockport and the surrounding countryside is a pleasant one, with the Cheshire Plain to the South West and the High Peak of Derbyshire to the South East. There are numerous pleasant residential areas, and facilities for all types of education are good.

Stockport NHS Foundation Trust, which was formed on 1 April 2004, serves a population of approximately 300,000 and its main site, Stepping Hill Hospital, is the District General Hospital for the area. The Trust has 858 beds. The Trust also manages 1 other hospital. The Trust incorporates all of the major specialties and these services are provided via a Clinical Directorate/Divisional structure.

FOUNDATION YEAR 1 JOB DESCRIPTION

Foundation Rotations

All the rotations last for 1 year and are recognised by the dept of postgraduate medicine as being suitable for training in order to gain full registration with the GMC.

Each rotation comprises 4 months in a medical specialty with general medical on-call, 4 months in a surgical specialty with general surgical on-call and 4 months in a further specialty, surgical or medical.

Integrated Care Business Group

General Medicine

The Division of Medicine provides General Medical, Diabetology, Chest Medicine, Cardiology, Gastroenterology, Dermatology, Clinical Haematology, ECG / EEG, Rheumatology and comprehensive health services for older people, delivering Acute Care of the Elderly, Rehabilitation, Outpatient Services, Continuing Care and Respite Care across Stepping Hill Hospital and Cherry Tree Hospitals.

Duties will include clerking of routine and emergency admissions, investigations, follow-ups and ward rounds. The on-call is covered by 1 ST3+, 2 ST1/2 and 1 F1 and the hospital operates a policy of “acting down”. Work will be shared by all the on-call team when “busy”.

Admissions are assessed on the Medical Admissions Unit, which during the day is covered by a Consultant and a Staff Grade Practitioner. Competencies gained would include assessment and initial management of acutely ill medical patients, skills in communication, time management, prioritisation, multidisciplinary team working and experience in the subspecialty where they are based. Trainees are expected to participate in department educational activities and have protected group teaching

The Division of Medicine for Older People

The Unit serves a population of approximately 50,000 over 65's. There are 109 acute beds in Medicine for Older People at Stepping Hill Hospital, plus 51 rehabilitation and continuing care beds at Cherry Tree Hospital. In addition, there are intensive rehabilitation and continuing care beds.

The Cavendish Hospital in Buxton has 40 Medicine for Older People beds and 16 ESMI beds are managed by North Derbyshire Health Authority but Consultant input for Medicine for Older People is provided by Stockport.

This post will give experience in dealing with a wide range of acute and chronic medical conditions. Supervision is provided by a team led by the Consultant and includes an ST3+, ST1/2 and Nurse Specialist. Competencies gained would include assessment and initial management of acutely ill medical patients, skills in communication, time management, prioritisation, multidisciplinary team working and also an understanding of the rehabilitation problems faced in dealing with older people, and how to support patients in their own home. Trainees are expected to participate in department educational activities and have protected group teaching

Acute Medicine (MAU)

This new post is based in the Medical Admissions Unit as part of the recent Hospital expansion and redevelopment. The MAU will take all acute medical referrals from GPs into a specialist unit, where they can be assessed, investigated and initial treatment commenced prior to referral to specialist medical teams.

It is envisaged that the trainee will be based on a unit and will deal with the assessment, investigation and initial management of a whole range of acute and acute on chronic medical emergencies. They will be able to develop competencies in the acute care of medical problems in line with the curriculum, but also acquire skills in the areas of communication, time management and team working. Supervision will be by a Consultant based on the unit and the on call medical team. Close liaison with Specialist teams and patient follow up will be encouraged. Trainees are expected to participate in department of Medicine educational activities and have protected group teaching.

Accident & Emergency/MAU

This post will be based in the Medical Admissions Unit and the Emergency Department under the clinical supervision of the Emergency Department Consultants. The F1 doctor will be involved in the clinical care principally of the patients in the Emergency Admissions Unit. Duties will include the clerking of patients, participation in Consultant ward rounds and general patient management. If the MAU is quiet the F1 doctor will work in the Emergency Department assisting in the selection and management of Emergency Department patients likely to be referred through MAU.

During this placement the trainee should have the opportunity to acquire competencies in the assessment, resuscitation and initial management of a range of patients presenting as emergencies through the A&E department. They should become skilled in history taking, examination, ordering investigations, prescribing fluids and drugs in the context of a highly supervised unit. They will also be expected to learn to work in a multidisciplinary environment and communicate effectively with patients, relatives, carers and other health professionals. There will be excellent opportunities for developing practical skills. The department is an ideal environment to appreciate the importance of using evidence-based protocols in medical practice.

They will be released for F1 teaching but will also have the opportunity to participate in some of highly developed Virtual Learning Environment in the department. The F1 doctor will participate in the General Medical On-Call

The Medical Division works a full shift system for both General Medicine and Geriatric Medicine, which includes prospective cover for annual leave. The rota is a full shift and is EWTD compliant.

Surgical & Critical Care Business Group

The Surgical Division incorporates the Directorates of Trauma and Orthopaedics, Urology and Ophthalmology. The Division also provides a comprehensive general surgical service as detailed below.

General Surgery

An elective and emergency general surgery service is offered by the Division. The department has the benefit of four in-patient wards; these are complemented by the expansion of the Day Case Unit and the short stay Surgical Unit. The outpatients department for general surgery is based in the new hospital wing.

F1 duties will include day to day care of ward patients, under supervision. Clerking of admissions and administration of IVP injections. Urology

The Urology service provides Urological services at both Stepping Hill Hospital and Tameside General Hospital. The service operates from the ‘hub’ unit at Stepping Hill with the ‘spoke’ unit at Tameside General Hospital.

Trauma and Orthopaedics

The expanding Orthopaedic Service is housed in a new purpose-built unit. The work consists of both Trauma and elective surgery.

Duties include day to day care of ward patients, clerking of admissions, arranging theatre lists, assisting at operations and performing a few minor procedures and the opportunity to attend outpatients. There are trauma meetings daily at 08.00 hours. Supervision is provided by a team led by the Consultant and an ST3+ or an ST1-3 with Nurse Specialist support. Additional supervision and support is also provided by a Consultant and middle grade Orthogeriatric presence on the unit.

Competencies gained would include assessment and initial management of acute and elective orthopaedic and acute surgical emergencies. It would also allow an understanding of the rehabilitation problems faced in dealing with older people. They would acquire skills in communication, time management, prioritisation, multidisciplinary team working. Anaesthetics

This post is based in the ICU, HDU and theatres and allows the trainee to develop skills related to recognition and treatment of acutely ill patients across a wide range of acute conditions and recognise the impact that co-morbidity may have on surgical and anaesthetic management. It will also facilitate the development of airway management and vascular access skills.

Duties:

• Assessment, resuscitation and care of critically ill patients under Consultant supervision

• Planning to ensure a safe step-down to ward environment

Child & Family Business Group

Paediatrics

Paediatric and Children’s’ Health Services are based in a purpose built integrated building - ‘The Tree House’. The unit houses a Child Development Unit which has a multidisciplinary, multi agency team, which provides medical, and therapy services for children with development problems together with a 23 bedded surgical ward catering for general surgical problems, paediatric orthopaedics and ENT and the outpatients department. Last year there were 5147 inpatients. The Maternity Unit (3,700 deliveries per year) has 16 neonatal cots, 3 of which are fully equipped for intensive care.

Duties include the day to day care of in-patients, clerking of admissions and supervised assessment of emergency presentations and attending Paediatric ward rounds. There will also be opportunities to attend outpatient clinics. Competencies gained would include assessment and initial management of paediatric emergencies. It would also allow an understanding of the challenges faced in communicating with children and their carers. They would acquire skills in communication, time management, prioritisation, multidisciplinary team working. A team led by the Consultant, ST3+ and ST1/2 with Nurse Specialist support provides supervision.

Trainees are expected to participate in department educational activities and have protected group teaching. You are required to attend Paediatric ward rounds; there will also be opportunities for you to attend outpatient clinics.

FOUNDATION YEAR 2 JOB DESCRIPTION

Emergency Medicine

This is a busy department that has long been recognised for the excellence of its teaching and training. Trainees participate in shift working under senior supervision as part of a team of ST3+, Staff Grade and Consultants would expose trainees to a wide range of acute clinical problems. It would foster rapid clinical assessment, diagnostic skills, appropriate pathways of investigation and treatment and decision making skills. Generic skills including time management, communication, team-working and multidisciplinary working would be developed. Teaching within the department and release to the F2 teaching programme would ensure the full programme could be delivered.

This would also be an effective placement to deliver a wide range of practical competencies including wound management, chest drain placement, complex vascular access and a range of specialty specific competencies that could be developed by the trainee. It has long been a popular placement and would be an invaluable component to a balanced programme. All tracks would include a placement in this department.

Educational / Medical Opportunities

Wide experience will be gained in the reception and management of patients with acute medical and surgical emergencies; paediatric illness and injury; trauma and soft tissue injury; primary eye problems.

Informal teaching is provided by the senior and middle grades within the department. There is a weekly teaching session lasting approximately three hours, which covers a wide range of Accident and Emergency presentations. The teaching is more intensive over the first two weeks. The Senior House Officers are themselves taught teaching skills and given a topic to teach to their colleagues. This weekly teaching is mandatory for all Senior House Officers.

Opportunities for research exist and are encouraged whenever possible.

The Foundation Year 2 doctors encouraged to attended regional courses which include, a one day Paediatric Life Support and Advanced Life Support, soft tissue injury; x-ray interpretation. There may be opportunities to spend a day with the Ambulance Service as an observer.

Career guidance is available from the Accident and Emergency Consultants who will discuss careers in other specialties with Consultant colleagues where necessary.

Paediatrics

These posts, which have been very successful and popular, are based in the purpose built paediatric department.

Duties include ward work, outpatient clinics and special care baby unit.

They are part of a team including Consultants, ST3+, ST1/2, F2 and F1 trainees.

Out of hours duties will be part of the paediatric on call team.

Attendance at specialty teaching and the F2 teaching programme will be mandatory.

Objectives of the Post:

• Familiarity and confidence in dealing with a wide range of common paediatric problems.

• Understanding the special issues surrounding the care of children rather than adults

• Gain experience in dealing with the communication difficulties that may be associated with children and their carers

• Become confident at the practical procedures that are required to treat children including amongst others cannulation, lumbar puncture, assessment of pain and managing breathing difficulties.

• Out of hours duties will include the management of those patients presenting with a wide range of emergency medical conditions

Obstetrics & Gynaecology

Duties of the post will include care of in patients on the gynaecology wards, the labour ward, ante-natal and post-natal wards. Gynaecology outpatient clinics will be an opportunity to see new patients and review follow up patients. The ante-natal clinic is a multidisciplinary clinic with care of the pregnant patient being shared between midwives and doctors in hospital and the community. There is also the opportunity to participate in fertility clinics. Theatre work will include both elective and emergency work both in theatre and on the delivery suite. All work will be supervised by Consultants and STs. The out of hours work will be shared with specialty and GP trainees.

Objectives of the post will be to familiarise trainees with the clinical issues surrounding common gynaecological problems, pregnancy and delivery. It is also an excellent forum to develop generic skills including communication, team working, breaking bad news, dealing with bereavement and the conflicts that may arise between the needs of the mother and that of the foetus. Organisational and time management skills will also be highlighted. This is an area where many practical skills can be acquired by interested individuals and resuscitation and diagnostic skills can be assessed.

Teaching within the department and attendance at the F2 teaching programme plus participation in the audit and clinical governance activities of the department will ensure that these areas of the curriculum can be addressed

Laboratory Medicine

Three week attachments to each of Haematology, Biochemistry, Histopathology and Microbiology with a supervised programme of activities and one day a week to be spent in the Clinical Effectiveness Unit supervised by Consultant Epidemiologist.

Participation in acute medical on call at nights and weekends on MAU.

Aims and Objectives

• Develop clearer understanding of the role of Laboratory Medicine and

• how to use it most effectively in the clinical management of patients

• Provide training in history, diagnosis and management of patients with haematological disease

• Develop further understanding of infection control and antibiotic therapy within the hospital

• Underline the importance of a multi-disciplinary approach to patient care

• Opportunity to support an increased academic approach to medicine and the value of evidence based practice

• To develop a significant audit project under the auspices of the Clinical Effectiveness Unit

• Opportunity to consider career opportunities in specialties where there may have been less exposure in Medical School such as Histopathology, Biochemistry, Microbiology, Haematology, Epidemiology and Public Health

This post provides an excellent environment to improve and develop a wide range of clinical and generic skills whilst having the opportunity to consider a career in a field that the trainees have had little previous experience of.

The trainees who have previously experienced this placement have completed audits and projects that have strengthened their CVs and, in some cases, have been published.

Psychiatry

Duties. Based with the Mental Health Unit, Pennine Care NHS Trust, Stepping Hill Hospital. Duties will include clerking of new patients, investigation and management of inpatients and outpatients, presentation of cases on ward rounds, liaising and coordinating treatment programmes with community based teams and the rehabilitation team and assessing referrals from other hospital areas. There will also be an opportunity for outpatient and community experience with the Community Mental Health Team and to do home visits with the Consultant and witness Mental Health Act assessments.

On Call duties on a 1:9 rota will cover Acute psychiatric and old age wards, A&E, Rehabilitation unit, the psychiatric intensive care unit and referrals from GPs via A&E, the Crisis Resolution team and A&E liaison team.

The post is designed to give an insight into a range of acute and chronic mental health problems. Trainees would also be expected to attend Specialty and F2 teaching sessions and to be involved in audit. There would be opportunities for administering under supervision, basic behavioural and cognitive treatments in selected cases.

Learning objectives are negotiated at the beginning of the post and consultant supervision will be by the clinical supervisor, the on call consultant and will include an hour a week individual teaching and reflection of learning by the Clinical Supervisor

Intensive Care Medicine & Anaesthetics

This post would be based in the ICU and HDU. It would be directed predominantly at the care of patients undergoing complex surgery or surgery in the presence of co-morbidity supervised by Critical Care Consultants and part of a team of STs.

Duties:

• Pre-operative assessment planning for at risk patients

• Post-operative care on the ICU and HDU

• Planning to ensure a safe step-down to ward environment

General Practice

Doctors will be attached to training practices and apprenticed to an educational supervisor and GP trainer.

The post provides experience and exposure to a range of generic skills and competencies, based in general practice. F2 trainees will be exposed to a range of acute illness and chronic disease management, be involved in community Primary Health Care Team (PHCT) liaison and practice management.

The hours of duty will be no more than 48 hours per week and F2 trainees in general practice will be expected to attend the practice for duties of nine half day sessions during normal surgery hours.

The weekly programme will include surgery consultations and home visits, the F2 trainees will be exposed to a range of acute illnesses and chronic disease. They may be released/seconded for areas of special interest and will work within the wider PHCT.

F2 trainees will be expected to attend and participate in practice and PHCT meetings and will engage with the practice management team and administrative staff to develop their management skills.

Education and Training

Each directorate takes an active interest in postgraduate education and medical staff are actively encouraged to attend regular teaching sessions held in the Medical Education Centre.

Stockport NHS Foundation Trust has an active Medical Education Centre. The Medical Library is well stocked with textbooks and journals, and has a Computer Suite with adequate resources to enable doctors to complete the Deanery VLE programme. The Trust has a very active Audit Department, and departmental/divisional audit meetings are held on a quarterly basis. Support for Junior Doctors carrying out audit work is also readily available.

Each Foundation doctor has an Educational and Clinical Supervisor and regular appraisals are carried out throughout the year. There are many other opportunities for feedback, both formal and informal.

Regular teaching will be available, again both formal and informal, on the wards as well as the opportunity to enhance good medical care through supervised clinical practice.

There is an induction programme held over the two days immediately prior to start of post. There is also the opportunity of a 2 week shadowing period prior to start of post. You will also receive a ‘Survival Guide’ which is produced by the current F1’s and is an essential guide to Ward jobs.

The weekly (bleep free) educational programme for FY1 and FY2 spans the entire year and is mapped to the curriculum. Career guidance will be available as part of this programme. We have a fully functioning Simulation Suit plus Meti Man. Simulation teaching is a regular part of the teaching programme.

The Trust is keen to ensure that each Doctor develops competencies such as working together, communicating confidently and planning and reviewing as well as developing new knowledge and skills. Training in these competencies will be tailored to the individual needs of each Doctor.

Main Conditions of Service

The post is offered on a full time basis and covered by the Terms and Conditions of

Service for Hospital Medical and Dental Staff (England and Wales) as amended from time to time.

Pre employment checks will be undertaken prior to commencing in post, as follows:

- Written evidence of current registration with the General Medical Council.

- Health Clearance

The successful candidate will be required to complete a health questionnaire and undergo a pre-employment medical examination before commencing employment with the Trust.

The post that you have applied for is exempt from the Rehabilitation of Offenders Act 1974. This is because the job is concerned with the provision of health services which involves access to patients. You must therefore declare any previous convictions, ‘spent’ or otherwise. The posts meet all New Deal requirements and accreditation has been awarded to the Trust.

The junior doctor accepts that he / she will also perform duties in occasional emergencies and unforeseen circumstances at the request of the appropriate Consultant, in consultation where practicable with his / her colleagues both senior and junior. It has been agreed between the professions and the Department of Health that while juniors accept that they will perform such duties, the Secretary of State stresses that additional commitments arising under this sub-section are exceptional and in particular, that juniors should not be required to undertake work of this kind for prolonged periods or on a regular basis.

Transport

• Car

Car Parking is available on site. Although there is a small fee to use the car park, the money is used to provide security guards, who regularly patrol the site. Car sharing is encouraged and those who car share at least twice a week are entitled to free parking.

• Cycling

We also support a cycle leasing scheme, with cycles and equipment available for hire at a minimum charge. Cycle racks and sheds and changing facilities are also provided.

• Buses

• Trains

The nearest train line runs from Buxton to Stockport, with trains running every 20 minutes in both directions. Trains also run direct to and from Manchester and beyond at least once an hour. From Stockport Station, connecting trains can be caught to most areas within the North West.

Security

Trust premises are by their nature places with public access. This means that extra vigilance is required with regard to the security of both Trust and personal property. The Trust advises that patients, visitors and staff do not leave any items unattended or unsecured and accepts no liability for loss or damage to personal property. Lockers are available for staff valuables right across the Trust.
