Designed and successfully implemented Shared Governance model and Primary Nursing care delivery model in the ACN leading to increased employee engagement with a documented employee satisfaction score from 70%-82% and patient satisfaction from 82.8 to 84.5. Designed and implemented Telehealth Nursing Center at NYP/ACN designed to increase access to care, reduce walk in admission, reduce avoidable ED visit/admission and increase over all patient experience. Designed and implemented patient cooking demo which increased WHI community’s understanding of affordable healthy food choices. Hosted in Univision national TV and recognized as a best practice. Implemented interdisciplinary team based care (ITD) resulting in increased collaboration among team members and improved thru put as well as reduced turnaround time by 30 minutes/visit.Successfully lead 6 TJC facilitating accreditation.
