See General Conditions.A.1 SCOPE 0F WORK: The quotation shall include all work necessary for improvements to the rear property of Team Inc. including but not limited to the expansion of the dumpster enclosure and all ancillary related work and the reconstruction of a section of the existing driveway. It shall include lowering an existing manhole frame and cover the restoration of all disturbed areas and repairs to any damaged elements of the existing site. It shall include all equipment, material, labor and items of work necessary to complete the improvements and provide the Owner a complete product ready for use.A.1.1 Preparatory Work: A meeting with the staff of Team Inc. must precede any work to address security and coordination necessary to minimize the impact to the operation of the facility and review the schedule. A.1.2 Repairs to Driveway Entrance: The existing concrete around the catch basin is to be removed from the site. Excavation will be required to provide a base of 6 inch depth and a concrete surface of 6 inch depth. The sub-base is to be thoroughly compacted using mechanical tamping equipment and reinforcing steel installed. The concrete is to be vibrated in place and broom finished. It shall be covered in plastic and protected from vehicle use for a minimum of seven days. Expansion joints are to be provided at both ends of the driveway section and all sides of the catch basin.A.1.3 Patio Block Area: Patio block are to be installed on a 5 inch depth of compacted sand. One of the existing manholes frame and cover has to be lowered. The patio block will have to be cut and shaped to encircle the manhole covers. The block must be set tight with virtually no separation at the seams since a wheeled container will have to be able to roll across the surface. Patio block must be bedded fully on the sand base to prevent cracking.A.1.4 Enclosure: The addition to the enclosure is to be 6 feet in height. It shall utilize one side of the existing enclosure. A double leaf gate is to be provided and installed. Diagonal bracing is to be incorporated in each leaf. Gate latches are to be commercial grade with provisions for the use of a padlock. Slats that match the existing enclosure slats are to be installed. The base slab is to be 5 inch depth of concrete on a base of processed gravel also 5 inches in depth. A.1.5 Repairs to Existing Gate: The missing bottom rail on one of the gate leaves is to be replaced and diagonal braces added for both top and bottom sections of both leaves.A.1.6 Retaining Wall: The retaining wall along the east side of the enclosure addition is to be constructed of 6 inch square pressure treated landscaped ties. The minimum length of the ties shall be eight feet. Joints shall be staggered. They shall be fastened with 1/2 inch diameter steel pins spaced 24 inches on center. Pins must extend at least ? the thickness of the bottom tie. A.1.7 Landscaping: The area to be sloped and all disturbed areas resulting from the construction that are not surfaced with bituminous concrete or other manufactured materials, shall be loamed and seeded. Existing topsoil maybe stripped, stockpiled, and used if it meets the requirements for new topsoil. Topsoil shall be a minimum of four (4) inches thick after Installation. Seed shall be spread in accordance with the Manufacturer’s directions but not less than one lb. per one thousand square feet. Hay shall be spread on the newly seeded areas. Evergreen shrubs shall be installed in accordance with the instructions of the nursery from which they were purchased.A.1.8 Cleanup: All construction materials shall be removed from the site at the completion of the work. Newly seeded areas shall have temporary stakes and ribbon to alert people to avoid crossing the area until growth is established. The existing walk to the rear entrance shall be swept clean.A.1.9 Documentation: The Owner shall be provided with a list of the products used to allow the purchase of replacement items if necessary. Also the names of any subcontractors used on the construction shall be furnished. B.1 Materials: All materials must be manufactured for the specific purpose used. They must have been in use for at least five years and readily available. The manufacturer’s recommendations are to be followed. Not all materials required are listed. It is to be understood that all materials are to be of a professional standard accepted in the industry as being of high quality and intended for the purpose used. The Owner may request certifications, test results or other documents to insure materials are in compliance.B.1.1 Reinforcing steel: Number 4 bars shall be used. Reinforcing steel shall conform to ASTM A615 Grade 60 and shall be bent cold. Tie wires shall be a minimum of NO. 18 gage and be galvanized or stainless steel. B.1.2 Concrete: Concrete shall be 4000 psi air entrained. The air-entraining agent shall conform to ASTM C 260. Cement shall be American-made Portland cement. It shall be free of water soluble salts. Fine aggregate shall be sand: inert, washed, natural and conforming to ASTM C33. Coarse aggregate shall be crushed stone or washed gravel conforming to ASTM C33. Silt shall not exceed 1.0 percent. Owner reserves the right to have a testing laboratory take cylinders when concrete is being poured and have tests done at 7 days and 28 days to insure compliance with the requirements. B.1.3 Processed Aggregate: Processed aggregate base shall conform to DOT Form 816 – Articles M.0501-1 through M.05.01-3 and shall have a compacted thickness of 5 inches. Coarse aggregate shall be broken stone. It shall be uniformly compacted with a mechanical compactor. B.1.4 Fence and Gate: Six feet in height this woven wire fence usually referred to as chain link is to be covered with vinyl. Corner post and posts at any change in direction shall be 2.37 inches in diameter. Intermediate post shall be 1.90 inches in diameter. Diagonal braces shall be 1.62 inches in diameter. Stretcher bars shall be ? inch by ? inch. Bands for stretcher bars shall be spaced 14 inches apart. Fabric is to be at 6 gauge, 2 inch mesh. Polymer coating shall be in compliance with ASTMF668. Color shall match the existing enclosure and shall be in compliance with ASTMF934. All post and pipe shall be coated to match the fabric. Gate fabric shall match the fence fabric. Gate construction shall be galvanized steel pipe welded fabrication in compliance with ASTM F900. Frame members shall 1.90 inches in diameter. Polymer coating of all components shall match the fence. Minimum height of gate shall be six feet.B1.5 Landscape Ties: Landscape ties shall be pressure treated and from heartwood with no more than 20% sapwood. Timbers are to be treated with ACQ (Alkali Copper Quat) or equivalent. Timber shall be pressure impregnated and dried to no more than 19% moisture content after treatment. Net retention of ACQ shall be a minimum of 0.60 pounds per cubic foot of ACQ. Each piece shall a AWPA (American Wood Preservers Association) mark evidencing compliance with LP-22 (Ground Contact Use).B.1.6 Sand: Sand shall be sharp, clean and free from silt, stone or any deleterious products. Size shall be between 0.02 inches and 0.04 inches.B.1.7 Topsoil: Topsoil shall be natural loam, friable fertile, free of clay, lumps, roots, brush or other litter, and stones larger than one inch in diameter. It shall be obtained from well drained sites not bogs or marshes. B.1.8 Grass Seed: Grass seed shall be new-crop fresh seed as follows: Chewing Fescue - 50 % Kentucky Blue grass - 35% Perennial Rye grass - 15%C.1 Site Limitations: There is limited room available on the back yard of the parcel. The sidewalk on Olivia Street should not be blocked except for the concrete pour. At that time personnel must be available to escort pedestrians around the truck. The walk to the rear door of the building may not be blocked.D.1 Site Safety: In addition to conventional concerns regarding safety at any site the Bidder is cautioned that care must be taken in the location of materials, equipment, tools and debris of all types to avoid encroachment of the sidewalk. E.1 Time Limitation: All work is to be completed within 45 (45) consecutive calendar days of the date established as the start of work. F.1 Sales and Use Tax Exemption: The Owner is a tax exempt organization of the State of Connecticut. Accordingly sales of equipment, materials and supplies to the owner are exempt from the sales and compensating use tax of the State of Connecticut.G.1 State and Federal Wage Rates: State and Federal wage rates apply to projects exceeding $2,000.00 in cost. Projects exceeding $2,000.00 in cost require compliance with the wage rate schedule of the Connecticut Department of Labor at the time of bidding for the City of Waterbury area. The Contractor will be required to furnish the applicable documents including certified payroll forms. The Regulations made pursuant to the Davis Bacon Act (40 U.S.C. 276A) and all Supplements including Supplements by the Department of Labor regulations (29 CFR) and modifications and amendments apply.H.1 Nondiscrimination in Employment: Contractors and sub-contractors performing work under this contract will be obligated not to discriminate in employment practices.I.1 OSHA Requirements: This project is subject to all the Safety and Health Regulations (CFR 29 Part 1926 and all subsequent amendments) as promulgated by the U.S. Department of Labor.J.1 Owners Right to Prosecute Work: If the Contractor fails to correct work found to be unacceptable the Owner may suspend any additional work until the corrections are made. If the Contractor defaults or fails to prosecute the work the Owner may have the work corrected independently of the Contractor’s operations and the cost of any corrections to work previously done deducted from any payment due the Contractor.K.1 Supervision: When work is being prosecuted the Contractor shall always have an individual on-site that has the authority to direct the activities of all his personnel and sub-contractors and to immediately respond to any emergencies or issues raised by the Owner or Owner’s representatives. Emergency telephone numbers shall be provided.L.1 Certifications: The Contractor shall provide certifications for all major components of the work attesting to their compliance with the Specifications.M.1 Completion of Work: The work will be considered complete when all paperwork has been submitted and approved, all construction materials, equipment and debris removed, any damage resulting from the work repaired and the area cleaned. N.1 Warranty: Prior to release of the final payment to the Contractor a one year, written warranty including both material and labor shall be provided. O.1 Proposal: The proposal is to include the price of all work necessary to complete the project. The costs of items not specifically mentioned are to be included in the cost of items listed. An entry is required for each item. Proposals that list only the total will not be accepted. The Owner reserves the right to delete items in the proposal from the scope of work.P.1 Agreement: The Agreement Form is included to illustrate the agreement that will be used after a contractor is selected.
