High school graduates should complete the Secondary Education Transcript Request form included in this application booklet and send it to their graduating high school. GED recipients must complete the Secondary Education GED Test Score Request form and send it to the PA Department of Education. _____ Attend pre-entrance assessment testing and attain a minimum score as prescribed by the program. The Lancaster County CTC uses the College Board's online Accuplacer exam and tests in two basic areas, Arithmetic, and Reading Comprehension. Lancaster County CTC may consider postsecondary credits in English and Math with a grade of C or better or SAT scores of 480 or higher in Critical Reading and Math in lieu of placement testing. (Accuplacer, TEAS, SAT)_____ Transfer of credit: For college credits to transfer, official transcripts must be received prior to the first day of class. Determination of credits transferred will be provided to the prospective student. Submit any college transcripts. All of these transcripts should be sent from the institutions._____ TOEFL scores (If you are a non-native speaker of English)_____ High school or College Level Chemistry and Biology or high school AP Chemistry and Biology are required prior to admissions directly into the hygiene program. By signing this waiver, you are permitting the results of the clearance to be forwarded directly to the school._____ Request for PA State Police Criminal Record Check form. Individuals who have a positive criminal history may be denied admission to the program. The program director, Director of Adult Education, and the CTE coach will interview and score applicants using a 5- category rubric. Applicants must interview to be eligible for the program. The five categories include: Career Goals, Commitment to Educational Program, Risk Factors, Punctuality, and Presentation.Review and sign Enrollment Agreement.Attend a student orientation program prior to the start of training. Students who cannot easily travel to PA may be considered for skype or phone interview. Medical exemptions are also allowed.Students must submit an exception request in writing and must sign an informed consent regarding lack of vaccinations. Tuberculosis testing is done on a biennial basis for all LCCTC employees that are involved with patient care activities. Employees must have a TB skin test. They do not need to have a two-step skin test unless there is an outbreak or exposure. TB test is done at the STUDENTS’s expense.Immunization and testing records for all employees are maintained in the school nurse and director’s files.Immunization requirements for all students enrolled in the Dental programs:Are regulated by director of dental hygiene and school nurse.CPREach student is required to have a current CPR for Health Providers prior to beginning the dental hygiene program. Students will not be able to treat clients without a current CPRcertification.
