JOB DESCRIPTION

JOB TITLE: In-Patient Licensed Practical Nurse

FUNCTION

Provides nursing care to Hospice of the Upstate patients in accordance with Medicare Conditions of Participation and organizational policies following medical directives and Hospice plan of care.

SUPERVISION RECEIVED

Works under the supervision of the Registered Nurse while ensuring quality health care. Reports to the Rainey Hospice House Patient Care Coordinator.

QUALIFICATIONS

1. Must have a current South Carolina license as a Licensed Practical Nurse.

2. Demonstrate the ability to be flexible and work independently.

3. Must be empathetic and have the ability to relate well to patients families, and members of the Hospice of the Upstate care team.

4. Understands and embraces the hospice philosophy.

RESPONSIBILITIES

1. Completes assessment and running notes for assigned patients prior to the end of the scheduled shift.

2. Maintains patient records, reflecting plans, problems, actions and goals and current medication profile.

3. Provides patient care based on assigned patient’s care plan that reflect patient/family wishes and appropriate level of care.

4. Ensures medication profile is current.

5. Communicates patient needs and changes with Hospice Medical Director, attending physician, and contracting agencies who provide services.

6. Provides education, training, and support to the hospice patient/family as directed by the patient’s plan of care.

7. Communicates and collaborates with home care staff regarding patient transfers, changes in patient status, and deaths.

8. Practices within the guidelines of the appropriate Boards of Nursing.

9. Attends agency staff meetings, department staff meetings, educational offerings, and other offered hospice events.

10. Assists other departments with a spirit of team work and cooperativeness.

11. Ensure the protection of patient and employee information by following the HIPAA-HITECH guidelines.

12. Maintain the integrity of all forms of patient information, including but not limited to, electronic personal health information and assure that there are no attempts to access more than the minimum necessary information to complete his/her job.

13. Be flexible, organized and work effectively and professionally under stress.

14. Professionally represent the organization with credibility and dignity and in the spirit of excellence and trust.

15. Demonstrate and communicate the mission, ethics, and goals of the facility as the face of Hospice.

16. Assumes other duties as necessary.

TOOLS AND EQUIPMENT USED

Standard medical equipment.

PHYSICAL DEMANDS

May be required to climb steps, stoop, bend or lift up to 35 pounds. Must be able to communicate compassionately with patients, families, and the general public. Must be able to stand for long periods of time.
