11755: Nail Unit Biopsy
Biopsy of the nail unit (e.g., plate, bed, matrix, hyponychium, proximal and lateral nail folds
(separate procedure)
11100 & 11101: Cutaneous Biopsies
11100: Biopsy of skin, subcutaneous tissue and/or mucous membrane (including simple closure),
unless otherwise listed; single lesion.
11101: Each separate/additional lesion (List separately in addition to code for primary procedure)
(Use 11101 in conjunction with 11100)
11300 Series: Shaving of Epidermal or Dermal Lesions
Shaving is the sharp removal by transverse incision or horizontal slicing to remove epidermal and
dermal lesions without a full-thickness dermal excision. This includes local anesthesia, chemical or
electrocauterization of the wound. The wound does not require suture closure.

Excisional CPT Codes
To use the CPT 11400 and CPT 11600 series of codes, local anesthesia must be applied. In addition,
subcutaneous fat should be present upon visual inspection or histopathologic examination.
