Your first appointment in our office is an evaluation. During this visit, we want to learn about your past dental experiences, medical history, and expectations. We also will examine you and gather any relevant radiographs or images. We then need a little bit of time to look through all of this information, to discuss your case with you other doctors, and to formulate 2-3 options we think might be best for you.We then have you back for your second appointment. This is a treatment planning discussion, where we will show you what we found and present the options we have created. We will be sure to cover the pros and cons of each option, as well as the treatment timeline and financial costs. Our hope is that by the end of this 2-step process, you will understand the current state of your dental health, the problems that exist, the treatment options that are possible, and the differences between them. Before Your First AppointmentNew Patient FormsWe will need you to fill out several forms prior to your first visit. Please download and complete our new patient forms by clicking the icon below. Upon request, we can also send the forms to you via mail, email, or fax. Please complete the forms and bring them with you to your first appointment. Please arrive 15-20 minutes prior to your scheduled appointment. X-RaysIf you have had recent x-rays at another dental office, please contact that office prior to your visit with us and request copies of your x-rays. You can usually have them sent directly to our office, but you may also obtain copies and bring them with you. If your past x-rays are not sent or if they are of poor quality, we will acquire these in our office, at no additional charge.Medical InformationPlease bring the name and phone number of primary care physician as well as a current list of medications to your first appointment. Financial PoliciesAll charges incurred are your responsibility regardless of your insurance coverage. As your dental care provider, our relationship and responsibility is with you, the patient. Your insurance policy is a contract between you, your employer, and an insurance company. Our office is not a party to that contract. However, as a courtesy to our insured patients, we can submit claims to your insurance company and we will help you receive your maximum allowed benefits. In order to do this, we need your insurance card and all the needed information of the insured subscriber.?Payment for dental treatment is due at the time services are rendered, unless prior arrangements have been made. Payment methods include cash, personal check, MasterCard, Visa, Discover and American Express. We offer a discount to patients who pre-pay for their treatment. Trailhead Dental also offers financing plans through 3 different third-party agencies; Care Credit, The Lending Club Financial Institute, and Green Sky Financial.
