I. Project Summary

On January 12th, 2012, New York State Department of Transportation (NYSDOT) Region 5 staff conducted an asbestos sampling survey of BINs 1022660 (Delavan Avenue over NY Route 33, City of Buffalo), 1022710 (Hospital Drive over NY Route 33, City of Buffalo), 1022730 (Deerfield Avenue over NY Route 33, City of Buffalo) and 1022880 (Union Road over NY Route 33, Town of Cheektowaga). All four BINs are located in Erie County. Based on information obtained using the procedures described in the Inspection Procedures portion of this report (section III), the presence or absence of Asbestos Containing Materials (ACMs) was ascertained.

II. Site Description

The sites are located in the City of Buffalo and the Town of Cheektowaga, New York. For the purpose of this report, the sites consist of BINs 1022660, 1022710, 1022730 and 1022880. (See Site Location Maps 1-4).

III. Inspection Procedures

The following procedures were used to obtain the data for this report:

NYSDOT staff under the appropriate New York State Department of Labor (NYSDOL) issued certificates reviewed the bridge plans and performed a visual inspection of subject structure was conducted to identify potential visible/accessible sources of ACM. Observations and notes were made to provide a description of the structure and an estimate of the approximate amount, length, or area of ACM present.

Physical or operational constraints which might affect the removal of the ACM were identified and reported. All inaccessible areas identified on the structures are listed for each BIN location and sampling/testing will be required once the material is exposed and is accessible during construction operations.

A total of 21 bulk samples of suspected ACMs were collected during the site inspection and submitted to a New York State Department of Health (NYSDOH) Environmental Laboratory Approval Program (ELAP) accredited lab for analysis. Preliminary Polarized Light Microscopy (PLM) and confirmation Tunneling Electron Microscope (TEM) analyses of non-friable materials were performed by Paradigm Environmental Services, Inc. to determine if asbestos is present, and, if so, the percentage in each sample.

Results of the bulk sample analyses are tabulated in the attached Bulk Sample Results Table. Lab results were used to determine the approximate location, type, and amount of any verified ACM.

IV. Laboratory Results of Sampled Suspect Asbestos Containing Material

The table below indicates only the materials that were determined to contain asbestos. Full laboratory test results of all bulk samples taken can be found in Appendix C.
