Subject: New Family Care Safety Registry (FCSR) Worker Registration and Employer Background Screening Request Forms

There are new versions of the FCSR Worker Registration and Employer Background Screening Request forms posted at .  The old forms will continue to be accepted, but Department of Health and Senior Services (DHSS) FCSR encourage transition to the new forms as quickly as possible in anticipation of implementation of the new background screening system that will be taking place over the next 4 to 6 weeks.

If you are using customized form(s), you may continue to do so, but the DHSS FCSR requests that you modify the form(s) to include all the fields shown on the new versions of these forms.  Those that use customized forms won’t need to include all the choices on certain fields, however (e.g., for foster parents and home child care providers, you won’t need to list all the registration types or the long term care sub-categories).   DHSS FCSR staff will be happy to work with you to ensure that any revisions meet both your needs and the Departments.

When DHSS FCSR is ready to fully implement the new database, documentation will be made available to provider internet users so that they’ll be able to successfully navigate the new system.  The new system is 100% web-based, and includes registration, background screening and other FCSR functions in one comprehensive package.  More information will be forthcoming after testing has been completed and the FCSR is ready to roll out the new system.
