Utilising Digital Transformation to Better Manage Population Health

As the healthcare landscape continue to evolve and change, organisations and systems are working much more collaboratively both at a place-level and across bigger footprints such as the 44 STP footprints and integrated care partnerships. To do this, innovative, integrated and accountable models of care are being developed, and in order to realise their potential it is essential that partner organisations are able to utilise the data available to them in order to develop real-world understanding of the needs of the population in addition to the current and future demands on the Health system. With the introduction of the Global Digital Exemplars and in light of the Wachter report it is clear that, in order to move towards an outcomes-based service model, significant transformation and enhancement is required.

This award will celebrate genuinely innovative, results-based digital transformation programmes that have led to measurable improvements in patient care, outcomes and cost-efficiency in service provision across an entire health economy/ system.

Judges will be looking for demonstrable evidence that the entry:

• Has a clear rationale for the innovation

• Has led to measurable improvements in patient care, experience and outcomes

• Characterises strong leadership and staff engagement - How do staff actually use the technology? Who championed the project? Was the change welcomed/challenged?

• Has led to better value and quantifiable financial savings

• Has spread or is potentially replicable and scalable

• Improves ability to utilise data from across the health and care system

• Involves close collaboration across all key partners, commissioners, providers, local government, etc.

• Deploys appropriate technology to improve outcomes and efficiency

• Has cross-organisation and cross-sector impact

Supporting evidence from stakeholders is encouraged and any videos you have to support the entry should also be submitted. If you are submitting supporting documents please include it all on one email and reference links and data within this completed submission.
