NYC DOE Student Verification Consent Form for IDNYC

Dear Parents:

IDNYC is New York City's free municipal identification card program for all New Yorkers age 14 or older. Immigration status does not matter. The IDNYC program offers many wonderful benefits for you and your child, including discounts on tickets to movies, Broadway plays, sporting events, and water parks; discounted YMCA and Parks Dept. Recreation Center memberships; and free one-year memberships at 40 of the city's premier cultural institutions, including the Museum of Natural History, the Brooklyn Children's Museum, botanic gardens, and zoos. The card can display personalized emergency contact information for your child, and the NYPD recognizes the card as valid government ID. IDNYC is also a prescription and grocery discount card.

We are happy to announce that your school can now provide a Student Verification Form (SVF) making it easy for your child to apply for an IDNYC card. In addition to the SVF, your child will need one form of photo ID, which could be a school ID, a SYEP card, or any other photo ID that IDNYC accepts. If your child does not have their own photo ID, a parent or guardian can accompany them instead. For more information about IDNYC's application process, visit idnyc.

If your child is younger than age 18, you must complete the consent form below for the DOE to be able to verify and disclose your child's information to IDNYC staff for IDNYC application purposes. Once we receive your consent, your child will be given a Student Verification Form which s/he can present as supporting documentation for his or her IDNYC application.

By signing this form, I consent to the release of the following information from my child's DOE records, by the NYC Department of Education to IDNYC personnel for IDNYC application purposes:

Student name, home address, date of birth, student identification number, and the name and address of the school the child currently attends.
