This post will be based within the Immediate Assessment Unit (IAU) and Clinical Decisions Unit (CDU) in the Queen Elizabeth University Hospital (QEUH). The assessment unit is staffed by a team of passionate, dedicated medical and nursing staff striving to deliver the highest possible standards of care. 12 consultants work an extended shift pattern, with middle grade doctors and junior medical staff providing 24-hour cover. The IAU accepts all medical and general surgery GP referrals with patients being initially managed in the IAU or in the newly opened CDU.

To support the team within the IAU and CDU we are seeking to appoint an enthusiastic Acute Physician who enjoys the fast and ever changing pace of the acute assessment environment.

This post would provide a challenging and rewarding clinical experience for any doctor looking to continue or improve their practice of Acute Medicine in a busy modern Assessment Unit and CDU that is continually developing.

Applicants must have full GMC registration, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to CCT or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from the date of interview. Non-UK applicants must demonstrate equivalent training.

The Acute Operating Division of NHS Greater Glasgow and Clyde is the largest group of adult acute hospitals in Scotland - offering many opportunities to ensure job satisfaction and career development. The Queen Elizabeth University Hospital

The Queen Elizabeth University Hospital is a large teaching hospital with an acute operational bed complement of 1109 beds. The Hospital is sited in the south-west of Glasgow and provides a comprehensive range of acute and related clinical services.

Services include Emergency Medicine, Dermatology, ENT, General Medicine (including sub-specialties), General Surgery (including sub-specialties), Medicine for the Elderly (including Assessment, Rehabilitation and Day Services), Gynaecology, Neonatal Paediatrics, Obstetrics, Ophthalmology, Orthopaedic Surgery, Urology, Physically Disabled Rehabilitation and Continuing Care. The Obstetrics, Gynaecology, Urology and Ophthalmology Departments provide the single in-patient location for the whole population of South Glasgow. In-patient Maxillofacial (trauma and elective surgery and specialist provision for head and neck cancer), Dermatology and the Assessment and Rehabilitation service for the Physically Disabled are also provided for the whole city from the Queen Elizabeth University Hospital.

The Institute of Neurological Sciences is based on the Queen Elizabeth campus and provides Neurosurgical, Neurological, Clinical Neurophysiology, Neuroradiological and Neuropathology facilities for the West of Scotland. The Queen Elizabeth National Spinal Unit for Scotland provides a spinal injuries service to the whole of Scotland. This is housed in a purpose-built facility.

There is also a wide range of therapeutic services including Audiology, Clinical Psychology, Dietetics, Occupational Therapy, ECG, Physiotherapy, Radiology (including MRI and CT provision for the general hospital service) and Speech Therapy.

Living and working in Glasgow

Today Glasgow is a compact, vibrant and modern city. The scale of the City comes as a surprise to many people. It has the largest suburban rail network outside London and is second only to the UK capital as a retail centre.

There are top-ranking schools, excellent leisure facilities, beautiful golf courses and elegant accommodation across all price ranges.

The night life and restaurants are renowned and its high quality opera, theatres, art galleries and museums offer an abundance of cultural stimulation.

From Glasgow, it is only a short journey to many picturesque sites including Loch Lomond (45 minutes), the Argyll peninsula – or over the sea to Arran, Skye, Iona and Mull.

Description of the Department

The Acute Assessment Unit of the Queen Elizabeth University Hospital has 118 beds including an Immediate Assessment Unit (28 beds), Medical and Surgical speciality clusters (90 beds) and a new Clinical Decision Unit which can currently accommodate up to 15 patients at any time.

Immediate Assessment Unit

The Immediate Assessment Unit (IAU) has 28 beds and was planned to see circa 20 000 new GP referrals per annum. A consultant presence is maintained in the Assessment Unit currently between the hours of 0800 - 2000 hours Monday to Friday and 0800 - 1600 hours Saturday and Sunday.

The IAU provides a full 24 hour a day 7 day a week service for all medical and general surgical GP referred patients. All patients requiring admission from the IAU or the Emergency Department for medical or general surgical management are admitted directly to the 90 beds in the speciality clusters in the Assessment Unit.

The IAU is adjacent to the Emergency Department and radiology allowing early swift access to all investigations required for patients on their initial presentation. This process supports early decision making and management of patients for admission to or discharge from hospital. We have close working relationships with the Emergency Department to support patients’ presentations that may require Emergency Department level of support in resus.

Clinical Decisions Unit

This unit is for the management of low acuity patients referred by their GP with medical conditions, with most patients being discharged the same day.  Patients managed in the CDU benefit from timely assessment and diagnostics where necessary with the opportunity to refer patients into emergency ‘hot’ clinic slots and Day Ward services as required.  Many patients seen in CDU will be managed using protocolised pathways for conditions including Chest pain, cellulitis, and headaches, with an expected ongoing expansion to support ambulatory care. The CDU is staffed by 2 GPs with an interest in Ambulatory care, as well as a team of Medical Nurse Practitioners.

Description of post – Queen Elizabeth University Hospital

Title: Acute Physician

Immediate Assessment Unit / Clinical Decision Unit

Duties of the Post

Clinical Commitments:

The post holder will be based at the Queen Elizabeth University Hospital Glasgow and will be managing patients as they present to the IAU and CDU. He/she will provide support and advice to junior doctors, nursing staff and liaise with consultants colleagues and relevant other specialities as and when required.

Administration:

The consultant will undertake the administrative duties associated with the care of his/her patients and the running of the units. In addition, the general administration workload of the service is divided between all consultants under the direction of the Lead Clinician and Clinical Director. This includes such participation in committees and working groups convened by the sector, health board and other NHS bodies as required – within the provision of the time available within the job plan.

Audit/Education:

The successful candidate will be expected to contribute to clinical audit programme and educational activities in the unit including teaching junior medical staff, nurses and other staff groups.

Job Plan

The appointment is full-time contracted as 10 programmed activities comprising 9 Direct Clinical Care sessions and 1 SPA.

This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director.  NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings.  The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.

It is expected that the successful applicant will work their DCC sessions as part of the IAU consultant rota. Currently this involves working shifts from 8am to 8pm Monday to Friday. Moving forward it is anticipated that this shift pattern will change to allow an extended window of time with Consultant presence in IAU and to allow more Consultant presence at the busiest times of the day. This will be part of the regular job plan and will be remunerated at the premium rate in the consultants contract (time and a third) or the PA's affected from 4 hours to 3 hours as per the 2004 Consultant Contract'.

Weekend cover is currently provided by 8 of the IAU consultants, working 1 weekend in 8 in rotation. The successful applicant would be expected to participate in the weekend IAU rota at a frequency of around 1 in 8 weekends, working 0800 to 1600 on both days.

If the successful applicant has any sub- speciality interests or training then this can be discussed to see how this may be incorporated into their job plan.
