For a State ID:

• Proof of Name and Date of Birth, such as a Certified birth certificate, Passport (not expired), or old Driver’s License or State ID that expired no more than 4 years ago.

• Proof of Identity, such as a Social Security Card, Forward (Medical Assistance) Card, Quest (FoodShare) Card, etc.

• Proof of Wisconsin Residency, like a Paycheck stub, Forward or Quest Card, Bank Statement that is at least 30 days old. It must show your current address.

• Your Social Security Card, to prove you’re a US Citizen.

• Money: State IDs: Original or Renewal State ID Cards are $9.00; Replacement State ID Cards are $6.00. Driver’s Licenses: Original or Probationary Cards are $18.00; Renewal Cards are $24.00; Replacement Cards are $4.00. Prices subject to change.

What to bring with you:

Fill out an application for a new, replacement, or corrected Social Security Card (available online and at the local SSA buildings). New cards are always free; replacement cards are free up to 3 per year, and 10 per lifetime. After that you have to pay a fee.

If you are getting your own card:

• Proof of citizenship: Your birth certificate or current U.S. Passport.

• Proof of identity: Your driver’s license, State ID or U.S. Passport.

If you are getting a card for your child:

• Proof of your own identity: See above.

• Proof of your child’s identity: A doctor’s medical record, baptismal, child care center or school record, or a school ID card.

• Bring proof of your child’s citizenship: You will need a copy of your child’s birth certificate.

If you cannot get one or more of these documents, call the SSA at the number above and speak with a customer service representative. They may accept alternative documentation.

|To apply for Child Care benefits, see pages 70-71. If you or your parent is receiving |

|W-2 services, you can apply at your W-2 agency. |

|If you are a teen parent under 18 years old, your parent must sign for you and provide |

|proof of income and employment. Your family must be financially eligible and your parent|

|must be working or participating in an approved W-2 activity. |

|If you are under 18 and have not already graduated from high school, you must be enrolled|

|in school to qualify. Please contact the Teen Parent Liaison at your local W-2 agency |

|for assistance and more information. |

|If you are over 18, you do not need a parent’s consent. You must meet financial |

|guidelines and be working or participating in job search through W-2 or other programs. |

|You may also be able to receive child care while attending a post-secondary educational |

|program such as college. |

|If a person in the household has a disability or health condition that makes them unable |

|to work and take care of the child(ren), the family may still be eligible for Wisconsin |

|Shares. Verification from a physician is needed. |

|If you do not live with your parents, you have a child with special needs, or feel you |

|have other special circumstances, please contact your local W-2 agency for assistance and|

|more information. |
