HOW TO CORRECTLY ENTER COMPOUND DRUG INFORMATION

Entering drug information for controlled substances for compound prescriptions depends on the way the software vendor updates the computer POS program. Please follow the instructions below:

At the normal NDC selection, enter 999999999

This should prompt additional screens: o Compound Drug Ingredient Sequence # - enter 1 for the first ingredient or drug being used in the compound. If the compound contains more than one ingredient, you would use 2 for the second ingredient, 3 for the third ingredient, and so on until you've entered all of the ingredients in the compound. o Product ID Qualifier ? This should always be 01 for NDC o Product ID ? This is the NDC number ? all 11 digits (no hyphens or spaces) o Compound Ingredient Quantity ? Enter the numerical amount of the ingredient ? i.e. 100 if you were using 100 units of a product o Compound Drug Dosage Units Code ? This identifies the unit of measure 01 = each 02 = milliliters, ml (for liters adjust to the decimal ml equivalent) 03 = gram, gm (for milligrams, adjust to the decimal gm equivalent ? i.e. 100 for 100 mg)

CDI: Compound Drug Ingredient Detail

Required when medication dispensed is a compound and one of the ingredients is a PMP reporting

drug. If more than one ingredient is for a prescription-monitoring program reporting drug, then this

would be incremented by one for each compound ingredient being reported.

Used to identify the individual ingredients that make up a compounded drug.

If CDI is filled in, the NDC of DSP08 must be 99999999999
