Part-time Community Health Worker (29 hours/week)

Service Area: Prevention & Health Promotion Services – Moms & Babies First Home Visiting Program

Reports to: Infant Vitality Supervisor

Salary: CHW In-Training entry level - $13.62/hour

Certified CHW entry level - $15.01/hour

Under general supervision of the Infant Vitality Supervisor, the Community Health Worker (CHW) serves as a trained advocate from the community to reduce African American infant mortality rates and poor birth outcomes. Conducts home visits to promote early and consistent prenatal care, consistent well baby check-ups and on-time immunizations. Manages assigned caseload through outreach and home visiting services in the Moms & Babies First Program.

Job Responsibilities:

• Conducts home visits to provide culturally appropriate health and development information, addressing risk factors for poor birth outcomes

• Makes referrals and assists clients with access to appropriate community resources

• Maintains an assigned caseload, including performing required client assessments and documenting service in the Ohio Comprehensive Home Visiting Integrated Data System (OCHIDS)

• Participates in individual and peer case review with Moms & Babies First Staff

Qualifications:

• Associate’s Degree in human services or child development and two (2) years of work experience preferred; equivalent combination of education and experience may also be considered.

• Must possess or obtain Community Health Worker certification through the Ohio Board of Nursing (CHW In Training will be trained upon employment, with certification expected within 6 months of hiring)

• Valid Ohio driver’s license with an acceptable driving record and proof of auto liability insurance coverage required. Must have a vehicle to use on the job that is in roadworthy condition.

WORKING CONDITIONS: Occasional overtime required. Travel within and outside the District required, with occasional overnight travel for trainings. Occasional work at night or on weekends.
